Panic disorder in clinically referred children and adolescents.

The present study examined the frequency and characteristics of panic disorder in children and adolescents who had been referred to a pediatric psychopharmacology clinic. Of the 280 children and adolescents evaluated in this clinic, 35 were diagnosed with panic disorder using a semi-structured clinical interview (K-SADS) and other objective measures. Approximately half of the youngsters with panic disorder also met criteria for the diagnosis of agoraphobia. There was extensive comorbidity between panic disorder and other internalizing and externalizing disorders. Parents reported clinically si…

Norepinephrine transporter (NET) promoter and 5'-UTR polymorphisms: association analysis in panic disorder.

Several biochemical and pharmacological studies suggest that the catecholaminergic system involving the norepinephrine transporter (NET) is relevant for the pathogenesis of panic disorder. Three single nucleotide polymorphisms in the promoter or untranslated 5' region of the NET gene were investigated by means of RFLP analysis in a sample of 115 German patients with panic disorder and 115 matched controls. Statistical analysis failed to show association with the overall diagnosis of panic disorder. In the subgroup of patients with panic disorder without agoraphobia, however, two polymorphisms …

Are top journals biased against eating disorders topics?

OBJECTIVE: This study investigated whether there is a bias against eating disorders research among the leading psychiatric, psychological, and medical journals. METHOD: The authors performed a comparison between the number of empirical articles published about anorexia nervosa and/or bulimia nervosa and the number of articles published about panic disorder and/or agoraphobia (i.e., disorders of comparable disease burden) in 29 high-impact journals over a 5-year period (1996-2001). RESULTS: There were almost twice as many published empirical articles about panic disorder and/or agoraphobia (N…

Virtual reality: a new tool for panic disorder therapy.

The use of a multicomponent cognitive-behavioral treatment strategy for panic disorder with agoraphobia is actually one of the preferred therapeutical approach for this disturbance. This method involves a mixture of cognitive and behavioral techniques which are intended to help patients identify and modify their dysfunctional anxiety-related thoughts, beliefs and behavior. Emphasis is placed on reversing the maintaining factors identified in the cognitive and behavioral patterns. The treatment protocol includes exposure to the feared situation, interoceptive exposure and cognitive restructurin…

Social sensitivity: a shared feature of all phobias.

The prominence of a variety of social fears among types of phobia was examined. Responses of 80 agoraphobics, 25 social phobics and 35 specific phobics to Wolpe's (1983) Fear Survey Schedule were factor-analysed. Factors of social sensitivity (accounting for 24 out of 50 per cent of the variance), agoraphobia (7 per cent), blood injuries (5 per cent) and five other small specific phobic factors were extracted. On social sensitivity, agora- and social phobics overlapped; specific phobics were significantly lower than social phobics. On agoraphobia scores, agoraphobics scored significantly highe…

Carbon dioxide/oxygen challenge test in panic disorder.

The effects of a single inhalation of a 35% CO2/65% O2 gas mixture were examined in 71 patients with panic disorder with or without agoraphobia and 44 normal control subjects. Compared with the placebo condition, inhalation of air, the CO2/O2 mixture elicited a clear anxiety reaction only in panic disorder patients, who experienced a sudden rise of subjective anxiety as well as of several panic symptoms. Respiratory symptoms and the fear of dying best distinguished the patients from the control subjects. Baseline anxiety was not the key factor in explaining this differential reaction. The clin…

Psychiatric correlates of behavioral inhibition in young children of parents with and without psychiatric disorders.

Behavioral inhibition is a laboratory-based temperamental category by the tendency to constrict behavior in unfamiliar situations and assumed to reflect low thresholds of limbic arousal. We previously found behavioral inhibition prevalent in the offspring of parents with panic disorder and agoraphobia. In this report, we examined the psychiatric correlates of behavioral inhibition by evaluating the sample of offspring of parents with panic disorder and agoraphobia, previously dichotomized as inhibited and not inhibited, and an existing epidemiologically derived sample of children, followed by …

The relationship between DSM-III symptom disorders (Axis I) and personality disorders (Axis II) in an outpatient population.

The relationship between symptom disorder and personality disorder according to DSM-III was studied in 289 consecutive outpatients. It was observed that personality disorders occurred frequently among the chronic affective and anxiety disorders. The "dramatic" personality disorders were observed especially frequently among patients with cyclothymic disorder, and the "eccentric" personality disorders among patients with a diagnosis of dysthymic disorder, social phobia and agoraphobia. Dramatic personality disorder was also common among patients with simple phobia. As expected, a close correspon…

The Newcastle Anxiety Depression Diagnostic Index in relationship to the effects of monoamine oxidase inhibitors and tricyclic antidepressants.

The Newcastle Anxiety Depression Diagnostic Index (NADDI) was applied to 151 patients who received a monoamine oxidase inhibitor (MAOI) or a tricyclic antidepressant (TCA) drug. The pretreatment presence of a physical stressor and absence of agoraphobia were associated with superior MAOI effects. In women nine of the 13 NADDI items were associated with superior response to the MAOI drugs: these items comprised the absence of childhood anxiety, agoraphobia, compulsive symptoms, early awaking, and retardation, and the presence of dependent traits, neuroticism, physical stress, and panic attacks.…

Specificity in familial aggregation of phobic disorders.

BACKGROUND: To investigate whether each of three DSM-III-R phobic disorders (simple phobia, social phobia, and agoraphobia with panic attacks) is familial and "breeds true." DESIGN: Rates of each phobic disorder were contrasted in first-degree relatives of four proband groups: simple phobia, social phobia, agoraphobia with panic attacks, and not ill controls. Phobia probands were patients who had one of the phobia diagnoses but no other lifetime anxiety comorbidity. RESULTS: We found moderate (two- to fourfold increased risk) but specific familial aggregation of each of the three DSM-III-R p…

Exonic variants of the GABA(B) receptor gene and panic disorder.

The enhancement of GABAergic neurotransmission has been closely linked to antipanic drug efficacy. This is the first study to investigate a putative association of exonic sequence variants of the human GABA(B) receptor 1 (GABA(B)R1) gene and susceptibility to panic disorder. Three DNA sequence variants in exons 1a1, 7 and 11 were assessed by polymerase chain reaction-based restriction fragment length polymorphism in a case-control study among patients with panic disorder with and without agoraphobia (DSM III-R criteria) and blood donors. There was no indication of an increased vulnerability to…

Prenatal care and delivery in an agoraphobic woman. A case report.

Agoraphobia is a psychiatric illness that can cause patients to become functionally homebound. Although the obstetric care of women with agoraphobia has not been described previously, it is likely that the problem is more prevalent than recognized. A pregnant woman initially believed she would be unable to travel to the hospital for delivery. She was given prenatal care and psychotherapy at home; a safe hospital delivery was achieved. The management plan included the in-depth involvement of two physicians, physician preparation for a potential home delivery, patient education about obstetric r…

Internet-Based Acceptance and Commitment Therapy With Interoceptive Exposure for Panic Disorder: A Randomized Controlled Trial and Working Alliance Analysis.

This study's primary aim was to evaluate the efficacy of an internet-based Acceptance and Commitment Therapy (IACT) program modified to include interoceptive exposure for treating panic disorder with or without concurrent agoraphobia. Its secondary aim was to examine whether therapist- and client-rated working alliances were related to treatment outcomes. This randomized controlled trial included 79 participants, assigned to either a treatment group (n = 40) or a waitlist control group (n = 39) over 10 weeks. The study investigated the effects on panic disorder and quality of life, as well as …
