Rimegepant in airplane headache treatment: a case report.

BACKGROUND: Airplane headache is a rare condition first identified in 2004 and subsequently included in the International Classification of Headache Disorders (Headache Classification Committee of the International Headache Society in Cephalalgia 33:629-808, 2013. https://doi.org/10.1177/0333102413485658 ). Airplane headache typically presents as intense, stabbing, unilateral pain in the frontal or orbital regions, with a severity of 8-10 on the numeric rating scale. Despite its relatively low prevalence and generally nondisabling nature, the intense pain associated with airplane headache ofte

Aeromedical evaluation and successful return to flight in a military pilot with lingual thyroid: a case report and follow-up.

BACKGROUND: Lingual thyroid is a rare congenital anomaly resulting from the aberrant migration of thyroid tissue during embryonic development. Although frequently asymptomatic, its identification in military aircrew poses particular challenges given the rigorous standards of aeromedical certification. CASE PRESENTATION: A 37-year-old male military helicopter pilot was incidentally diagnosed with lingual thyroid during routine health screening. The patient remained asymptomatic throughout the clinical course. Comprehensive evaluations-including thyroid function tests, biochemical profiling, ce

Stroke in Airplane Passengers: A Study from a Large International Hub.

INTRODUCTION: Stroke in air travelers is being increasingly recognized. We report on stroke among passengers arriving at or transiting through a busy air travel hub. METHODS: The stroke database of the sole tertiary care center for stroke in a large busy international hub was interrogated. Demographic data of transit passengers, their stroke risk factors, stroke severity, National Institutes of Health Stroke Scale (NIHSS), acute stroke interventions, discharge status and outcome utilizing the Modified Raking scale (mRS) were retrieved and compared between passengers and non-passenger controls

A socio-economic analysis of increased staffing in the Norwegian helicopter emergency medical service.

BACKGROUND: The European Aviation Safety Agency (EASA) is preparing a new set of regulations that will cover working and resting periods for crew members engaged in emergency medical services with helicopters (HEMS) and aeroplanes (AEMS). Such a shared European regulatory framework has already been introduced for the majority of commercial operations with aeroplanes, whereas national regulations are still in place for helicopter operations. A possible consequence of changing the regulations on working and resting periods for helicopter operations is that current abilities to provide 24-h, cont

Fatalities of stowaways traveling in airplane wheel wells.

The earliest mention of a stowaway in the wheel well of an airplane dates back to 1947: A 30-year-old man chose this mode of travel for a tr … n the landing gear is deployed. When it appears conceivable that a stowaway may have fallen from an aircraft wheel well during landing or takeoff, an autopsy and discovery scene investigation are essential to reconstructing the course of events.

Loss of Consciousness During Single Sling Helicopter Hoist Rescue Resulting in a Fatal Fall.

INTRODUCTION: Although harness suspension trauma has been documented since the 1960s, especially in the mountaineering setting, there is lit … the risks of a chest harness need to be balanced against the patient's condition, the environment, aircraft performance, and the urgency of the rescue. CASE REPORT: We report an adult male falling 80 ft to his death while being hoisted into a rescue helicopter for a likely fractured ankle. A single rescue sling harness technique was used, but the patient …

Hypnotic responsivity and the treatment of flying phobia.

Systematic follow-up data are reported for 178 consecutive flying phobia patients treated with a single 45-minute session involving hypnosis and a problem restructuring strategy. One hundred fifty-eight (89%) of the patients completed follow-up questionnaires between six months and ten and one half years after treatment. Results showed that hypnotizable patients were over two and one half times more likely to report some positive treatment impact than those who were found to be nonhypnotizable on the Hypnotic Induction Profile. In addition, the patients' previous experiences with psychotherapy

L4-L5 compression and anterior/posterior joint shear forces in cabin attendants during the initial push/pull actions of airplane meal carts.

The aim of the present study was to assess the acute low back load of cabin attendants during cart handling and to identify working situations which present the highest strain on the worker. In a setup, 17 cabin attendants (ten females and seven males) pushed, pulled and turned a 20 kg standard meal cart (L: 0.5m × W: 0.3 m × H: 0.92 m) loaded with extra 20 kg and 40 kg, respectively on two different surfaces (carpet and linoleum) and at three floor inclinations (-2°, 0° and +2°). Two force transducers were mounted as handles. Two-dimensional movement analysis was performed and a 4D WATBAK mod

A young military pilot presents with a periocular Basal Cell Carcinoma: a case report.

INTRODUCTION: Basal Cell Carcinoma (BCC) is the most common form of skin malignancy in the UK with 75, 000 new cases per year. It commonly presents as a non-healing lesion in the H zone of the face. The significant risk factors for this condition include UV radiation exposure and a history of sunburn. The gold standard treatment for BCC is Mohs Micrographic Surgery as well a variety of traditional surgical and non-surgical options. CASE PRESENTATION: A 32 year old white male military helicopter pilot presented with a pea sized lesion which appeared highly vascularised with multiple telangiect

Motion sickness prevention by an 8-Hz stroboscopic environment during air transport.

INTRODUCTION: Previous research has shown that retinal slip can be a significant factor in causing motion sickness. Stroboscopic illumination may prevent retinal slip by providing snapshots of the visual environment that are brief enough so each image is stationary on the retina. METHODS: The purpose of this study was to determine the effectiveness of an 8-Hz stroboscopic environment as a motion sickness countermeasure for passengers during a nauseogenic flight in a helicopter. The study population was comprised of 18 motion sickness susceptible subjects. Subjects completed a motion sickness

The psychology of fear of flying (part I): a critical evaluation of current perspectives on the nature, prevalence and etiology of fear of flying.

Fear of flying, its nature, prevalence, etiology and treatment, has been the subject of a substantial quantity of research over the past 30 years. With the exception of a dated review of treatment methods however, there has been no evaluation of this expanding body of evidence, its contribution to theory and influence on clinical practice. Published research has also generally failed to apply developments in the understanding and treatment of anxiety disorders generally to fear of flying. This review provides a critical evaluation of the existing literature and what it reveals about theory and

Improving organ procurement travel practices in the United States: proceedings from the Michigan Donor Travel Forum.

There are significant risks and inefficiencies associated with organ procurement travel. In an effort to identify, quantify, and define opportunities to mitigate these risks and inefficiencies, 25 experts from the transplantation, transportation and insurance fields were convened. The forum concluded that: on procurement travel practices are inadequate, there is wide variation in the quality of aero-medical transportation, current travel practices for organ procurement are inefficient and there is a lack of standards for organ procurement travel liability coverage. The forum concluded that the

Benign paroxysmal positional vertigo in an airline pilot.

Benign paroxysmal positional vertigo (BPPV) is one of the most common types of peripheral vertigo, characterized by violent whirling vertigo after a positional change. Although the condition is termed "benign," the clinical presentation can be incapacitating for pilots in certain maneuvers. We present a case of an airline transport pilot with the complaint of vertigo for 5 d. The vertigo was aggravated by head movements when looking up or rolling over, lasting for a few seconds. The patient was diagnosed with BPPV, and he was treated with physical therapy with the use of Epley maneuver. The ai

Effects of changing gravity on anticipatory grip force control during point-to-point movements of a hand-held object.

We investigated the quality of predictive grip force control during gravity changes induced by parabolic flight maneuvers. During these maneuvers gravity varied: There were 2 periods of hypergravity, in which terrestrial gravity nearly doubled, and a 20-s period of microgravity, during which a manipulated object was virtually weightless. We determined grip and load forces during vertical point-to-point movements of an instrumented object. Point-to-point movements were a combination of static (stationary holding) and dynamic (continuous movements) task conditions, which were separately analyzed

[Is there a physician on board?].

Physicians responding to emergency calls on board airliners in intercontinental traffic may not be aware of certain legal complications which may arise. For instance, the medical practitioner may hold a license valid in one country, the air carrier may be registered in another, and the patient may be a third state national. Legislation varies between nations, as do courts decisions. Physicians may not be aware of the laws and regulations which apply or of the subtle differences between terms and interpretations used in formal language. This article contains a scenario description from a commer

Post-traumatic stress disorder in airplane cabin crew attendants.

Six cabin crew attendants who survived an airplane crash in which 47 passengers were killed were assessed for post-traumatic stress disorder and other psychological problems. Each was interviewed 8 months after the crash and completed questionnaires measuring intrusive thoughts, avoidance, depressed mood, anxiety, and fear. Questionnaires were repeated 10 months later. All six met DSM-III-R criteria for post-traumatic stress disorder, reported a wide range of symptoms, and developed a fear of flying 8 months after the crash. Depression scores were normal 18 months after the crash, but all othe
