The relationship of alcohol intake and urticaria is complex. The mechanism of action may be related to a hypersensitivity reaction to ethyl alcohol or to the other components of alcoholic drinks. Urticaria may also result from the pharmacological effects of alcohol or from the histamine content of the drink. The authors review the literature about hypersensitivity to alcohol and to the different components of alcoholic drinks, especially added compounds. The appropriate sensitivity tests for making this diagnosis are described.

Using criteria for the diagnosis of alcoholism established by the National Council of Alcoholism and by scores on the Self-Administered Alcoholism Screening Test, we compared the prevalence of alcoholism in a group of 99 patients hospitalized for the treatment of psoriasis with that in an age- and sex-matched control group hospitalized for the treatment of other dermatologic disorders. Alcoholism was diagnosed in 11 psoriatic patients and 3 control patients (0.05 less than p less than 0.10); 10 of the 11 psoriatic patients were men, and 1 of the 3 control patients was a man (p less than 0.05).

Alcoholic polyneuropathy is a common disorder, characterized by nonspecific clinical, electromyographic, and pathologic features. Nerve conduction studies and needle electromyography disclose findings consistent with a generalized, sensorimotor, axonal degenerative, polyneuropathy. Alcoholic polyneuropathy may be suspected when such findings occur in the setting of long-standing alcoholism and malnutrition. Serious infections appear to occur more frequently in alcoholic patients. However, whether this is due to the effects of alcohol per se or to the other frequent complications of alcoholism--nutritional deficiencies, cirrhosis, and poor hygiene--has not been determined. The host factors that may underlie an increased frequency of infection in alcoholics and the clinical infectious syndromes associated with alcoholism are described.

Available evidence implicates alcohol consumption as a major risk factor for almost all types of injury. An exceptionally strong relationship is noted between alcohol and motor vehicle accidents--particularly single-vehicle crashes. Recognition of the association between alcohol and trauma is important not only for proper care and for treatment of the underlying alcoholism, but to stimulate enactment of preventive measures aimed at reducing the risk of alcohol-associated injuries.
