Functional outcome after redo below-knee amputation.

BACKGROUND: Nonhealing below-knee amputation (BKA) necessitates redo BKA or conversion to above-knee amputation (AKA). Outcomes after redo BKA remain poorly defined. We hypothesized that in selected patients redo BKA results in favorable functional outcomes. METHODS: Patients undergoing redo BKA at a single institution during 4 years were reviewed. A concurrent cohort proceeding directly to ipsilateral AKA served as comparative controls. RESULTS: Of 138 BKAs, 23 (17%) failed to heal primarily. Baseline demographics and medical comorbidities were similar between groups. Clinicians selected 14

Flow-through use of the osteomusculocutaneous free fibular flap.

A case of a forearm defect resulting from a high-velocity firearm injury was reconstructed by flow-through free transfer of the osteomusculocutaneous fibular flap harvested from the remaining stump of the patient's left leg that was amputated below the knee. The dimensions of the bone defect and damage to anatomic structures of more than one type (including ulnar artery, ulnar bone, and overlying soft tissue) were the reasons for the treatment approach with a composite tissue transfer including vascularized bone. Preoperative radiographic and angiographic examination revealed that the amputati

[Treatment of deafferentation pain by high-frequency intervention on the dorsal root entry zone].

From August 1980 to May 1981, high-frequency lesions of the dorsal root entry zone of the spinal cord were performed on 35 patients with chronic deafferentiation pains. Among them were 15 patients with traumatic transverse cord lesions, 5 with non-traumatic transverse lesions and 7 with cervical root tears or traumatic brachial plexus lesions, 6 with stump or phantom pain after amputation, and 1 each with sciatic paralysis or spinal arachnopathy. Treatment results were best in complete transverse lesions, cervical root avulsion and brachial plexus lesion, less so for stump or phantom pain of t

Amputation following meningococcemia. A sequela to purpura fulminans.

In five patients with purpura fulminans following meningitis, gangrene of the extremities developed. Four patients required amputations of the lower extremities and two patients of the upper extremities. The gangrene is caused by disseminated intravascular coagulation. In two patients epiphyseal damage and subsequent angular deformities developed. The orthopedic surgeon should be consulted early because aponeurectomy may save an extremity. Some method of temporary skin coverage should be considered at the time of initial debridement and aponeurectomy. Early skin grafts are frequently rejected

Amputation-Site Soft-Tissue Restoration Using Adipose Stem Cell Therapy.

Soft-tissue deficits in amputation stumps can lead to significant pain and disability. An emerging treatment option is stem cell-enriched fat grafting. This is the first study assessing the potential for this treatment modality in lower extremity amputation sites. In this prospective cohort study, five injured military personnel suffering from pain and limited function at amputation sites were recruited. Fat grafting enriched with stromal vascular fraction was performed at amputation sites to provide additional subcutaneous tissue padding over bony structures. Outcomes measures included compli…

Carcinoma of the cervical stump.

Treatment, prognosis, and complication rate were retrospectively studied in 46 patients with cervical stump cancer. There were no differences in clinical stage distribution and histopathological findings between these patients and patients with cervical cancer of the intact uterus. However, 43 patients were treated by radiation therapy and only 3 patients had a cervical amputation. The 5-year survival rate was 62%, which is comparable to that of cervical cancers. Late complications were found in 22 patients: 3 (7%) had symptoms of severe radiation reactions of the bladder (necrosis or vesicova

Subarachnoid Block-Induced Deafferentation Pain Successfully Treated with Pentazocine.

Deafferentation pain induced by subarachnoid block (SAB) is rare, but it can appear in the form of recurrent phantom lower limb pain, new acute-onset stump pain in amputees, lower limb pain in patients with tabes dorsalis, and neuropathic pain. We have previously reported that thiopental is an effective treatment for deafferentation pain induced by therapeutic SAB applied to treat neuropathic pain of central origin. Here, we report the case of an amputee who developed new stump pain in his lower limb immediately after subarachnoid tetracaine was administered prior to appendectomy. A 51-year-ol

Immunohistochemistry indicates that persistent inflammation determines failure of tail, limb and finger regeneration in the Lizard Podarcis muralis.

BACKGROUND: The presence of white blood inflammatory cells in injured tissues and their effect on the process of organ regeneration in lizards has been assessed on tail, limb and digits. METHODS: The present immunohistochemical survey analyzes the occurrence of CD68-labeled cells in lizard organs uncapable of regenerating tissues that exhibit strong inflammatory activity. RESULTS: This marker mainly identifies macrophages and mast cells present in large number within tissues of injured limbs and digits. Also a high inflammation is associated with amputated tails that do not regenerate, deriv

Prevention of painful neuromas by epineural ligatures, flaps and grafts.

Neuroma formation at the proximal end of a divided nerve is a common problem in peripheral nerve surgery. Forty-eight nerve endings in 23 patients with traumatic or post-elective surgery amputation stumps were capped with either an epineural ligature, epineural flaps or an epineural graft. Each technique was used on 16 nerve endings. After at least 6 months follow-up, pain at the nerve endings was assessed by tapping the treatment sites and asking the patients to score their pain on a visual analogue scale (VAS) from 0 to 10. The mean VAS scores were 5.18 for epineural ligatures, 4.25 for epin…

Selection of patients for through-the-knee amputation.

Forty-six adult patients had a through-the-knee amputation (disarticulation of the knee) in a four-year period. Thirty-four of the patients had peripheral vascular insufficiency and were judged to lack the potential for using a prosthesis functionally, although the evaluation indicated that they had the potential for healing of the wound at the below-the-knee level of amputation. At a minimum follow-up of one year, the amputation wound had healed in thirty of these patients, and no joint contracture had developed. Two patients died in the first postoperative month, and two had failure to heal

Current achievements with topical negative pressure to improve wound healing in dehiscent ischemic stumps of diabetic patients: a case series.

Negative pressure wound therapy has been increasingly used either as a primary or as an adjunctive therapeutic measure to treat a variety of recalcitrant wounds during the past years. It is thought to act by creating a local environment that promotes cell proliferation, angiogenesis, and granulation tissue formation, leading to accelerated wound healing to the point of spontaneous closure or reducing the wound size to facilitate significantly further surgical reconstruction. This case series presents our preliminary experience with the use of a Topical Negative Pressure system in the treatment

Extensive MRSA-positive sternoclavicular septic arthritis with uncommon origin: a case report.

BACKGROUND: Sternoclavicular joint (SCJ) septic arthritis is a rare entity, representing less than 1% of all bone infections. It is generally associated with hematogenous spread from sources such as skin or soft tissue infections, intravenous drug use, or central venous access devices. Methicillin-resistant Staphylococcus aureus (MRSA) is an uncommon but aggressive pathogen in this setting. CASE DESCRIPTION: We report the case of a 52-year-old male with a history of type 2 diabetes mellitus with poor glycemic control, hypertension, hypercholesterolemia, coronary artery disease, and prior lowe

Incidence and characteristics of patients with hand ischemia after a hemodialysis access procedure.

The purpose of this study was to determine the clinical characteristics of chronic renal failure patients who developed hand ischemia in the limb carrying the dialysis angioaccess. A retrospective review of the charts of 352 patients who underwent 409 upper extremity arteriovenous access, and who were subsequently diagnosed as steal syndrome, was performed at the Emory University Hospital between February 1992 and January 1997. Hand ischemia occurred after 13 of 299 arteriovenous grafts (4.3%) and after 2 of 110 direct forearm arteriovenous fistulas (1.8%). Six patients developed ischemic mani

Bilateral lower limb amputations as a result of landmine injuries.

Landmine explosions cause most of the war injuries in the battlefield. Amputations resulting from severe injuries reveal serious problems despite the improvements in surgery. Bilateral lower limb amputations have more impact than unilateral on social life. Some 29 cases with lower limb amputations due to landmine injuries were treated in the Department of Orthopaedics and Traumatology, Gülhane Military Medical Academy between January 1992 and December 1996. Amputation levels were as follows: 1 case had hip disarticulation and a trans-femoral amputation, 6 had bilateral trans-femoral amputation

Achilles tendon allograft augmentation for biomechanical residual limb pain following below-knee amputation.

Chronic residual limb pain after below-knee amputation may result from inadequate distal soft-tissue padding, leading to poor prosthetic tolerance and impaired function. We describe a novel technique using Achilles tendon allograft augmentation to treat biomechanical stump pain. Four patients underwent revision below-knee amputation with Achilles tendon allograft placement between August 2023 and February 2025. The mean age was 51 years, and the average follow-up was 4 months. All experienced complete pain resolution and successful return to prosthetic use without complications or additional r

Hand replantation using loupe magnification in a resource constrained environment: Case report.

Hand replantation is a common surgical procedure worldwide. However, this practice is underdeveloped in many resource-constrained countries in part due to a lack of surgical microscopes. We present a patient successfully managed using loupe magnification. A 17-year-old patient presented with an amputated right hand secondary to a chaff cutter. After an 8-hour surgical procedure, the amputated hand was successfully re-attached to the stump using loupes. The patient's functional recovery was satisfactory after two years of follow-up. In conclusion, replantation of extremities can be successfully

Stent Graft Expulsion Outside the Skin: A Rare Case.

Patients who undergo stent grafting may present to the ED some time after the procedure with various related symptoms. The most common of these are stent graft occlusions or hematoma, although infection or abscess also may develop. In this case report, a 58-year-old man presented to the ED with a purulent wound on the stump of an amputated leg and a foreign body protruding from the wound site. The patient had a history of stent insertion with femoropopliteal bypass 11 years before this incident and an above-the-knee amputation because of stent occlusion 8 years prior. This wound had appeared w

The "turn-up" plasty: one solution to a complex problem.

This case study presents a patient with severe osteomyelitis of the femur, which did not respond to vigorous antibiotic therapy. Compromised kidney function, malnutrition, and copious drainage from an open wound led the patient and physician to select a seldom used procedure, the "turn up" plasty to treat the problem. This procedure provides a stump for an above-the-knee prosthesis providing the patient with more optimal function than if the femur and lower leg were amputated. The procedure involves removal of the infected femur, foot amputation, knee disarticulation and turning the lower leg

Prevention of symptomatic neuroma in traumatic digital amputation: A RAND/UCLA appropriateness method consensus study.

INTRODUCTION: The appearance of a symptomatic neuroma following finger amputation is a devastating consequence for patient's quality of life. It could be cause of chronic neuropathic pain. The prevention of neuroma formation is a challenging effort for hand surgeons. The biological mechanisms leading to neuroma formation are mostly unknown and different preventing procedures have been tried without certain results. In this paper, a panel of Italian hand surgeons have been asked to express appropriateness about potentially preventive techniques of neuroma formation following the RAND/UCLA appro

Rehabilitation Following Multiple Limb Amputation: A Case Report.

Multiple limb amputations are uncommon and may result from various causes, including trauma, metabolic disorders, severe burns, purpura fulminans, and drug use. Such amputations pose substantial physical, emotional, and social challenges, necessitating a comprehensive and multidisciplinary rehabilitation approach. This report discusses the rehabilitation of a 30-year-old male patient with multiple limb amputations, including a right transradial amputation, a left transhumeral amputation, and a left transfemoral amputation, following a railway accident. The patient was managed with a multidisci
