Spinal anesthesia in children with isobaric local anesthetics: report on 307 patients under 13 years of age.

BACKGROUND: Spinal anesthesia in expert hands is an excellent method for children for appropriate surgery. The aim of this study was to evaluate the effects of spinal anesthesia with isobaric solutions in 307 consecutive cases from May 2001 to August 2002. METHODS: In this prospective study, 307 patients from 0 to 12 years of age were scheduled for spinal anesthesia with enantiomeric mixture of bupivacaine (S75 : 25R) 0.5% or racemic bupivacaine 0.5% or lidocaine 2% without glucose, for surgery compatible with the technique. The following were assessed: latency of analgesia, motor block, maxi

Acute cauda equina syndrome after total knee arthroplasty as a result of epidural anesthesia and spinal stenosis.

An unusual complication of epidural anesthesia performed for routine total knee arthroplasty is presented. Epidural catheter placement or removal resulted in an acute cauda equina syndrome in a patient with asymptomatic high-grade lumbar spinal stenosis at L3-4. The case is presented along with a literature review.

Comparison of three catheter sets for continuous spinal anesthesia in patients undergoing total hip or knee arthroplasty.

BACKGROUND AND OBJECTIVES: Continuous spinal anesthesia (CSA) with microcatheters may be complicated because of technical problems. In elderly patients, some of the problems may be solved by using thicker catheters. A recent invention, involving a catheter-over-needle system, may prevent leakage of cerebrospinal fluid (CSF) and may also improve the prediction of the intrathecal position of the catheter tip. METHODS: This study included 90 patients undergoing primary hip or knee replacement, randomly allocated into 3 groups, with 30 patients in each group: Group EC, a 24-gauge epidural cathete

[Hypothesis concerning the anatomical basis of cauda equina syndrome and transient nerve root irritation after spinal anesthesia].

OBJECTIVE: Cauda equine syndrome is a rare neurological complication associated with subarachnoid anesthesia, and particularly with the use of 5% hyperbaric lidocaine and small gauge catheters. Our aim was to study a possible anatomical factor that might impede adequate dilution of local anesthetic and explain the development of cauda equine syndrome and transitory radicular irritation. MATERIAL AND METHOD: The spinal dura matters and their contents from two male human cadavers were examined after organs had been extracted for transplantation. Both men had recently died at ages 56 and 65 year

Prevention of hypotension during spinal anesthesia: a comparison of intravascular administration of hetastarch versus lactated Ringer's solution.

This study was designed to compare the efficacy of 6% hetastarch to that of crystalloid administration in reducing the incidence and severity of hypotension during spinal anesthesia. Forty ASA grade I patients scheduled for postpartum tubal ligations under spinal anesthesia were randomly allocated to receive either 500 mL of hetastarch solution or 1000 mL of lactated Ringer's solution prior to spinal anesthesia. Spinal anesthesia was managed identically in both groups by anesthesiologists who were unaware of the type of fluid administered. The incidence of hypotension was 11 of 21 (52%) in the

Hemodynamic response and change in organ blood volume during spinal anesthesia in elderly men with cardiac disease.

Aging and disease may make the elderly patient with cardiac disease particularly susceptible to hypotension during spinal anesthesia. We studied 15 men, 59-80 y old, with histories of prior myocardial infarction (n = 9), congestive heart failure (n = 2), and/or stable myocardial ischemia (n = 11) given spinal anesthesia with 50 mg lidocaine in dextrose. Technetium-99m-labeled red blood cell imaging estimated left ventricular ejection fraction (EF) and changes in blood volume in the abdominal organs and legs. Arterial and pulmonary artery catheters provided hemodynamic measurements. Sensory blo

The incidence and associated factors of hypotension during spinal anesthesia at tertiary hospital: a prospective cross-sectional study.

BACKGROUND: Spinal anesthesia is currently the most commonly used technique for procedures below the umbilicus, although it is safe. However, the most common complication associated with this technique is the onset of hypotension after administration of spinal anesthesia. METHOD: This institution-based, cross-sectional study included 402 patients. The study was conducted on patients who underwent surgery under spinal anesthesia from 30 June 2023 to 30 November 2024, at a tertiary hospital in central Ethiopia, Hossana, Ethiopia. Study participants were selected using a systematic random sampli

Caesarean section under spinal anesthesia for a mother with dilated cardiomyopathy in a resource-limited setting: a case report.

INTRODUCTION AND IMPORTANCE: Peripartum cardiomyopathy is an uncommon form of cardiomyopathy that develops in the latter stages of pregnancy or in the first few weeks following delivery. Anaesthetic management of caesarean section of a parturient with dilated cardiomyopathy is challenging due to its nature of impaired ventricular contractility coupled with cardiovascular changes during pregnancy. This is significantly worse in a resource-constrained environment with restricted access to medications and equipment. CASE PRESENTATION: We report a case of a 31-year-old multigravida parturient pre

Spinal Anesthesia for Open Appendectomy in a Patient With Cystic Fibrosis and Coagulation Abnormalities: A Case of Tailored Anesthetic Management.

Cystic fibrosis (CF) is a genetic disorder that primarily affects the respiratory and gastrointestinal systems, often leading to significant perioperative challenges due to compromised lung function, recurrent infections, and chronic respiratory failure. Managing anesthesia in patients with CF requires careful consideration, particularly because of the increased risk of respiratory complications with general anesthesia (GA). Neuraxial anesthesia, such as spinal anesthesia, presents an alternative that can reduce the likelihood of postoperative pulmonary issues, including respiratory depression

Spontaneous Spine Fracture in Patient with Ankylosing Spondylitis under Spinal Anesthesia: A Case Report and Review of the Literature.

Factures in ankylosing spondylitis (AS) patients tend to occur due to the absence of motion between vertebrae, poor bone quality, and a long lever arm that generates extension force. However, most patients have a history of at least minor trauma. The aim of this report was that a vertebral fracture in a patient with AS can be caused not only by minor trauma, but also by position changes or maintenance of position for examination due to structural weakness. A 75-year-old woman with AS visited her local hospital on foot for back pain. She usually had back pain. However, she had increased back pa

Using Lumbar X-Ray to Facilitate Modified Taylor's Approach of Spinal Anesthesia in an Elderly Patient With Scoliosis.

In geriatric patients scheduled for hip or knee surgery, neuraxial anesthesia is a safe and effective anesthesia method and may be a better option than general anesthesia. Unfortunately, establishing neuraxial anesthesia is not always easy in this group of patients. Anatomical abnormalities, such as spinal stenosis, scoliosis, and narrowed interspaces, contribute to the difficulties that anesthesiologists face while performing these procedures. The classic Taylor's approach targets the widest interspace, L5-S1, as the needle insertion site and accordingly has an increased success rate in diffi
