A rare case of diffuse pigmented villonodular synovitis is described. Because of unspecific symptoms as oedema and pain and uncertain relation with trauma the diagnosis was delayed for almost two years, until a MRI showed diffuse hypodense infiltrative lesions involving soft tissue structures around the ankle. Treatment was a radical open synovectomy of the ankle and subtalar joints and tenosynovectomy of the tendons around the ankle.

Total ankle arthroplasty is a technically challenging reconstruction, with soft tissue complications posing potential significant morbidity, especially when the anterior ankle soft tissue envelope is not pristine. Alternate approaches to the ankle for arthroplasty may need to be sought in unique cases. The author describes a posterior surgical approach for total ankle arthroplasty.

BACKGROUND: Periprosthetic osteolysis in total ankle arthroplasty (TAA) is a substantial problem. We report the incidence and characteristics of periprosthetic osteolysis and its association with clinical outcomes after TAA using the HINTEGRA ankle system.
METHODS: Between May 2004 and April 2010, 126 primary TAA were performed on 115 patients. We excluded 27 ankles with a follow-up of less than 24 months; thus, 99 ankles in 90 patients with a mean follow-up of 40.8 (range, 24-89) months were included in the study. Pain and clinical outcomes were assessed using the visual analog scale and the…

Ankle sprains are common injuries in childhood and are generally harmless. However, fractures of the ankle joint are rare, but complications (persistent rotational or axial deformity, alteration of growth of the distal tibia or fibula, or joint deformity) can lead to serious problems during growth. The clinical relevance of growth disturbances at the distal tibia or fibula depends on the age at the time of fracture more than on the fracture type, severity of dislocation, or interponated material in the fracture gap. Both stimulation and inhibition of growth are possible. Inhibition of growth a…

BACKGROUND: Ankle arthrodesis is an accepted treatment for patients with advanced disabling tibiotalar arthritis, mostly in osteoarthritis, rheumatoid, and posttraumatic arthritis. No detailed reports have been published regarding the use of arthroscopy for the treatment of the end-stage hemophilic ankle. The purpose of this article is to report the results of arthroscopic ankle arthrodesis in hemophilic arthropathy of the ankle.
METHODS: Ten patients (10 ankle joints) who underwent arthroscopically assisted ankle arthrodesis for the treatment of end-stage hemophilic A arthritis were enrolled…

Chronic ankle instability (CAI) after ankle sprains has been shown to cause foot and ankle disability. Although rehabilitation programs for patients with CAI have been performed in related studies, few researchers have studied overall performance including pain, balance, and isokinetic torque. The purpose of this study was to determine if a short-term rehabilitation program that addressed range of motion (ROM), functional muscular performance, and neuromuscular control, can improve the foot/ankle disability index (FADI), FADI-Sport, ROM, star excursion balance test (SEBT), and isokinetic torqu…

BACKGROUND: This study aimed to clarify the intermediate followup results of reduction without soft tissue repair in open ankle dislocations without fracture.
METHODS: We retrospectively reviewed cases of open isolated ankle dislocations without associated fracture. Five male patients treated between January 2005 and July 2009 were enrolled in this study. Debridement and reduction with primary wound closure of the skin were performed without suture of the capsule or repair of the ligaments. The outcome was evaluated by the American Orthopaedic Foot & Ankle Society score (AOFAS). The time peri…

Ankle arthroscopy is a valuable tool in the treatment of certain intra-articular fractures involving the ankle, as it provides the ability to address osteochondral injury and aids in the direct visualization for joint reduction through minimal intervention. It can sometimes be complimented by a more minimally invasive approach to fracture reduction and internal fixation. It should be noted that to perform arthroscopically assisted minimally invasive fracture approaches, the surgeon must have significant experience with traditional open techniques.

Coronal plane deformity has been found to be one of the main risk factors for poor clinical results, higher complication rates and failure of total ankle replacements. Initially, many authors considered a malalignment of more than 10° to be a contraindication for total ankle replacement, however, several publications later underlined the usefulness of the distinction of different etiologies of hindfoot malalignment. This subsequently led to suggestions for additional procedures in order to avoid early implant failure. The aim of the present article is to illustrate the different causes of varus…

BACKGROUND: Chronic lateral ankle instability occurs in 10% to 20% of people after an acute ankle sprain. Initial treatment is conservative but if this fails and ligament laxity is present, surgical intervention is considered.
OBJECTIVES: To compare different treatments, conservative or surgical, for chronic lateral ankle instability.
SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register, the Cochrane Central Register of Controlled Trials, MEDLINE, EMBASE, CINAHL and reference lists of articles, all to February 2010.

BACKGROUND: Wound breakdown of the operative incision is commonly encountered as a complication following ankle replacement surgery. Healing problems can progress to full thickness necrosis of the skin and deeper tissues jeopardizing the ultimate retention of the implants leading to compromised patient outcomes.
MATERIALS AND METHODS: The medical records of 57 consecutive primary total ankle arthroplasties (TAA) were retrospectively reviewed after observing a higher than expected rate of wound-healing problems consistently involving the central third of the operative incision.
RESULTS: The r…

BACKGROUND: Ankle syndesmosis fixation is often employed with the utilization of many variable methods and implants. Complications secondary to trans-syndesmotic fixation have been described, yet the proximity of a single trans-syndesmotic screw to the perforating branch of the peroneal artery (PBPA) has not.
MATERIALS AND METHODS: Sixteen cadaveric legs received a single trans-syndesmotic screw using standard AO technique. The PBPA was identified and the following distances were measured using photographic images and morphometric software: the tip of lateral malleolus to the PBPA, the tibial…

This report presents an adolescent wakeboarder with persistent pain after conservative treatment of a Salter Harris type II distal fibular fracture. On physical examination the pain was localised over the anterior inferior tibiofibular ligament. Additional imaging revealed that this syndesmotic impingement was caused by a loose body caught in the syndesmotic area. The loose body was successfully removed from the anterior inferior tibiofibular ligament by anterior ankle arthroscopy. With functional aftertreatment, the patient had resumed full sports activities after 8 weeks. At 1-year follow-up…
