[A Case Report of Adenocarcinoma with Pagetoid Growth(Secondary Paget's Disease)of the Anal Canal].

UNLABELLED: Differentiating primary Paget's disease from secondary Paget's disease, which is also known as Pagetoid growth of other cancers, in the perianal region is difficult. Given that the treatment strategies of both disease are significantly different, diagnosis of such conditions should be performed with caution. We report a case of secondary Paget's disease. A 76-year-old man previously visited a hospital because of erythematous plaque around the anus. It was diagnosed as extramammary Paget's disease based on skin biopsy results; consequently, perianal skin resection was performed. Pat

Anal Neoplasia in Inflammatory Bowel Disease: Classification Proposal, Epidemiology, Carcinogenesis, and Risk Management Perspectives.

Patients with inflammatory bowel disease [IBD] may develop, similarly to individuals from general population, rare cases of human papilloma virus [HPV]-related anal canal squamous cell carcinoma [SCC] and intra-epithelial precursor lesions, as well as very rare cases of anal canal adenocarcinoma. Patients with chronic perianal Crohn's disease [CD] are at substantial risk of developing SCC or adenocarcinoma from the fistula-lining epithelium, as well as SCC or adenocarcinoma arising from chronic anorectal ulcerations or strictures. Based on this lesion stratification, we provide in this review

Perianal Lesions in Children: An Updated Review.

Perianal lesions in children are common reasons for dermatology clinic visits and a well-defined approach to diagnosis and management is helpful to the practicing clinician. In this article, we review and update various etiologies of perianal lesions in the pediatric population, including infectious, papulosquamous, vascular, and neoplastic. We provide a standard initial approach to diagnosis and updates on current management. Infectious etiologies of perianal lesions discussed in this article include fungal, bacterial, parasitic, and viral. Perianal papulosquamous lesions often encountered in

[A case of carcinoma associated with anal fistula in which reconstruction using a gracilis musculocutaneous flap was performed after preoperative chemoradiotherapy].

The patient was a 47-year-old man with a 24-year history of anal fistula. A carcinoma associated with anal fistula was diagnosed after a biopsy was performed because the anal fistula was not improving. Computed tomography and magnetic resonance imaging showed that the tumor had invaded the spongy urethra. Therefore, preoperative chemotherapy(S-1 plus radiation at a dose of 60 Gy/body) was performed. Because the size of tumor decreased, we performed abdominoperineal resection with reconstruction using a gracilis musculocutaneous flap, which enabled the spongy urethra to be fully preserved. Hist

Rectal adenocarcinoma in an adult with imperforate anus at birth.

A 43-year-old man with imperforate anus surgically corrected at birth presented with bleeding per rectum and constipation. On investigations, there was an adenocarcinoma at 2 cm from anal verge involving mid and lower rectum. He underwent abdominoperineal resection. During surgery, he was found to have a rectourethral fistula. The relationship between imperforate anus, rectourethral fistula and carcinoma of rectum is disclosed.

Is histopathological evidence really essential for making a surgical decision about mucinous carcinoma arising in a perianal fistula? Report of a case.

We report an unusual case of mucinous adenocarcinoma of the anus associated with a chronic anal fistula, treated successfully by abdominoperineal resection (APR). Although multiple biopsies failed to reveal any histological evidence of malignancy, cancer was diagnosed from the mucin obtained for cytology. Subsequent histological examination of the resected specimen revealed clusters of cancer cells floating in a mucous lake, suggesting that it would have been difficult to acquire the cells in a biopsy sample. Conversely, the presence of mucin lakes and globules in specimens drained from the re

Mucinous adenocarcinoma derived from chronic perianal fistulas: report of a case and review of the literature.

Chronic perianal fistulas are a common clinical condition. However, their evolution into adenocarcinoma is rare. We report the case of a 68-year-old man with perineal and perianal chronic fistulas, who developed a perineal mass that extended proximally as a pararectal tumor. Diagnosis was confirmed by magnetic resonance imaging (MRI). Histopathological sections indicated extramucosal mucinous adenocarcinoma. No intestinal lesion was seen at endoscopic examination. The patient underwent abdominal perineal excision of the rectum without neoadjuvant or adjuvant therapy, and had a good postoperati

[Infiltrating mucinous carcinoma developed on chronic anal fistula; diagnostic and surgical difficulties].

Anal carcinoma is rare; fistula-in-ano is sometimes mentioned among other premalignant conditions. Chronic anal fistula is associated with malignant transformation to mucinous adenocarcinoma, difficult to diagnose in the anal region and associated to a poor prognosis. Malignant transformation to infiltrating colloid carcinoma was surprised during a one year period, in a patient with long-standing perianal fistula (24 years). Inflammatory changes of the perineal region and other peculiar characteristics (such as neoplastic spread following supurative pathways) were responsible for diagnostic an

[Adenocarcinoma of the anal glands].

The study aim was to report an adenocarcinoma of the anal glands revealed by an anal abscess in a 64-year-old man. Malignant transformation of an anal fistula is discussed in the genesis of this disease. Clinical symptoms are not specific. Abdomino-perineal resection of the rectum is the usual surgical treatment and adjuvant radiation therapy didn't prove its efficiency.

Properties of HPV-positive and HPV-negative anal carcinomas.

Evidence of human papillomavirus (HPV) can be found in up to 85 per cent of anal carcinomas. In the vulva, a discrete subset of HPV-positive carcinomas which show koilocytic morphology and distinct clinical features has recently been identified (warty carcinoma). The morphological and prognostic features of HPV-positive and HPV-negative anal carcinomas were compared in this study of the tumour distribution of HPV DNA. Vulval and anal neoplasia are similar in many ways and we have also looked to see if their similarity extends to 'warty' morphology in relation to HPV status. Thirty-five resecti

Anorectal malformation: a new anatomic variant resembling an H-type fistula.

A new anatomic variant of an anorectal malformation resembling an H-type fistula was observed in a newborn girl. This variant consisted of a high-grade stenosis of the anal canal below the level of the levator muscle with a blind-ending fistulous tract originating from the stenotic segment and running toward the posterior wall of the vestibulum. The embryogenesis of this malformation remains to be seen.

Colo-recto-anal intussusception. Case report.

Two cases of colo-recto-anal intussusception are reported, both in old women and with a tumour at the apex of the intussusception. Primary resection of the sigmoid colon with terminal sigmoidostomy was successful in one case. Resection with anastomosis in the other case was followed by anastomotic leakage with peritonitis, requiring revisional closure of the rectum and terminal sigmoidostomy.

[Venereal diseases in children and sexual abuse].

Sexual abuse in children is reported increasingly frequently. The abuse frequently comes to the attention of the authorities when the children present symptoms which raise the suspicion of sexual abuse. One of these symptoms may be sexually transmitted disease in the child. Previously, there was a tendency to accept the possibility that sexually transmitted disease in children could be transmitted by other means than sexual contact, eg indirectly by infected bedclothes and toilet articles. Where gonorrhoea is concerned, no documentation exists in the literature for non-sexual infection in chil

Laser surgery in colonic and anorectal lesions.

Endoscopic laser therapy for diseases of gastrointestinal tract in general, and lesions of colon, rectum, and anus in particular, is in its infancy. Advances are expected to be made to define those lesions that are amenable to lasers and those that are not. In this process, it is hoped that the relative merits and demerits of the various modes of endoscopic therapy might be brought to light. With the advent of contact Nd:YAG laser technology, it is increasingly realized that such a mode of operation could be used to great advantage in both endoscopic and open surgery related to lower-gastroint

Carcinoma of the anus in Nigerians.

In the 18-year period between 1960 and 1978, only 20 cases of carcinoma of the anus were recorded in the Cancer Registry of the University College Hospital, Ibadan, Nigeria; of these 18 were treated in the hospital. This paper is an analysis of the clinicopathologic features and outcome of treatment of ten patients whose records could be traced. There was a slight female predominance, with anal canal tumours being more common than anal margin tumors. Most cases were advanced when first seen and the commonest manifestations were passage of mucoid stool, with blood and anal pain. The tumors tend
