Cranial diabetes insipidus caused by giant cell arteritis.

A 78 year old woman was admitted with symptoms of diabetes insipidus and was found to have a high ESR, the cause of which was uncertain. Five months later she presented with blindness, and cranial arteritis was confirmed by temporal artery biopsy. The endocrine abnormality returned to normal after steroid therapy. The association between these two conditions is discussed.

Pulmonary artery obstruction due to giant cell arteritis.

Giant cell arteritis is often referred to in the context of polymyalgia rheumatica with temporal artery involvement. There are, however, more malignant forms of presentation of this necrotizing arteritis involving either the great vessels of the aorta or, occasionally, the pulmonary arteries. Our case relates to giant cell arteritis presenting as pulmonary artery obstruction in a patient without polymyalgia rheumatica or extensive aortic or proximal great vessel involvement.

Giant-cell arteritis. Histological, immunohistochemical and electronmicroscopic studies.

Biopsies from the temporal artery of 32 patients suspected of giant-cell arteritis were evaluated retrospectively by light microscopy, histochemical, and immunohistochemical methods, as well as by transmission electron microscopy (TEM). At the clinical follow-up the 32 patients included four clinical groups: temporal arteritis (8 patients), polymyalgia rheumatica (10 patients), rheumatoid arthritis (4 patients), and a group of miscellaneous diseases unrelated to inflammatory rheumatic diseases (10 patients). There were a number of similarities between age-related alterations in the arteries an…

[Visceral giant cell arteritis similar to polyarteritis nodosa].

Biopsy from the small and large intestines of a 60-year old female patient with intestinal tract ulcerations helped to reveal angiitis similar to polyarteritis nodosa but with progressive proliferation of giant multinuclear cells in the inflammatory infiltrate rich in eosinophils. As much as 20-31% eosinophils were found in peripheral blood. Postmortem examination also confirmed the presence of angiitis in the kidneys, liver and thalamus. The position of arteritis between polyarteritis nodosa and visceral giant cell arteritis is discussed.

Juvenile temporal arteritis. Biopsy study of four cases.

Two young adults (aged 21 and 22 years) and two children (aged 7 and 8 years) complained of an unsightly, soft, painless unilateral nodule i … microaneurysmal disruption of the media. Whether the lesions represent a juvenile form of temporal arteritis, an unusual form of localized polyarteritis nodosa, or Kimura disease (subcutaneous angiolymphoid hyperplasia with eosinophilia) remains conjectural.

Macular Arteritis: A Report of Two Pediatric Cases.

We report two pediatric cases of macular arteritis presenting as asymptomatic retiform pigmented macules on the legs. Biopsy showed deep dermal-subcutaneous lymphocytic arteritis with fibrin deposition and luminal narrowing, with a negative infectious and autoimmune workup. Both patients remained well with stable lesions at 1 and 7 years of follow-up. These cases add to the limited pediatric literature and support macular arteritis as an infrequent benign cutaneous arteritis, helping avoid unnecessary investigations and aggressive treatment.

Bilateral Ocular ischemia in a patient with Takayasu arteritis.

OBJECTIVE: To describe a rare case of bilateral ocular ischemia as the initial presentation of Takayasu arteritis and highlight the role of early recognition and treatment in preventing irreversible vision loss. METHODS: A woman in her 50s presented with a gradual, progressive diminution of vision, initially in the left eye and later involving the right. Comprehensive ophthalmic evaluation, systemic examination, fundus fluorescein angiography, and CT angiography of the head and neck were performed. Laboratory investigations, including ESR, CRP, and ANA, were obtained. The American College of …

A glossary of signs and symptoms of giant cell arteritis.

OBJECTIVES: This study seeks to create consensus-based definitions of signs and symptoms of giant cell arteritis (GCA) for use by health care professionals, primarily in research settings. METHODS: Core definitions of signs and symptoms of GCA were extracted from 11 randomised controlled trials of GCA previously reviewed in a systematic literature review conducted in the context of the development of response criteria for GCA. This information was supplemented by definitions from other sources, such as rheumatology textbooks. A 2-round Delphi was performed within an international task force (…

Transorbital ultrasound in the diagnosis of giant cell arteritis.

OBJECTIVES: The objective of this study was to prospectively evaluate the diagnostic efficacy of transorbital ultrasound (TOS) in patients newly diagnosed with giant cell arteritis (GCA), presenting with visual symptoms. METHODS: Patients with newly diagnosed, untreated GCA were examined using TOS, assessing central retinal artery flow velocity [peak systolic velocity (PSV), end-diastolic velocity (EDV), resistance index (RI)], and optic nerve diameter (OND). Vascular ultrasound was conducted to evaluate the superficial temporal arteries, their branches, facial, axillary, carotid, and vertebr…

Lingual Necrosis in the Setting of Giant Cell Arteritis.

Giant cell arteritis (GCA) is a medium-to-large vessel vasculitis most commonly affecting the aortic arch and carotid branches. Lingual necrosis is a rare complication of GCA caused by lingual artery vasculitis due to ischemia. A delay in diagnosis can result in irreversible complications such as tongue amputation. Our case involves an 83-year-old female patient whose hospital admission for evaluation of stroke-like symptoms was complicated by the development of tongue necrosis, requiring percutaneous endoscopic gastrostomy tube placement for severe odynophagia and dysphagia. The patient was t…

Unusual overlap of systemic sclerosis with Takayasu arteritis.

Overlap syndromes are diseases that meet the criteria of two or more rheumatic diseases. In this case report, a woman in her 20s presented with a constellation of symptoms, including skin thickening, Raynaud's phenomenon, hypertension, absent pulse in both lower limbs with bilateral renal artery bruit. The antinuclear antibody profile revealed Scl-70 positivity. CT thorax identified early interstitial lung disease, and nailfold capillaroscopy showed severe capillary loss. CT angiogram features were suggestive of Takayasu arteritis. Notably, there have been only four documented cases of systemi…

Outcomes of aortic surgery in patients with Takayasu arteritis.

OBJECTIVE: To investigate the presentation, aortic involvement, and surgical outcomes in patients with Takayasu arteritis undergoing aortic surgery. METHODS: We queried our surgical database for patients with Takayasu arteritis who underwent aortic surgery from 1994 to 2022. RESULTS: There were a total of 31 patients with Takayasu arteritis who underwent aortic surgery. Patients' median age at the time of diagnosis was 35.0 years (interquartile range, 25.0-42.0). The majority were female (n = 27, 87.0%). Most patients (n = 28, 90.3%) were diagnosed before surgery, and 3 patients (9.6%) were …

Rare Presentations of Takayasu Arteritis: A Case Series.

Takayasu arteritis (TA) is a form of large vessel vasculitis that may lead to fibrosis, stenosis, or aneurysm formation of vessels. Its presentation varies depending on the arterial beds involved. We report 3 cases out of around 150 cases of TA with rare initial presentations of brachial plexopathy caused by an axillary artery aneurysm, complicated type A intramural hematoma, and renal artery aneurysms along with abdominothoracic TA presenting as a pulsatile abdominal mass.

Pyoderma Gangrenosum as a Presenting Feature of Takayasu Arteritis.

Takayasu arteritis (TA) is a large vessel vasculitis that involves the aorta and its major branches. The disease has a female preponderance, and it presents with a wide variety of symptoms including skin manifestations, mainly ulcerative nodules, pyoderma gangrenosum, and erythema nodosum-like lesions. We report a case of a 50-year-old female who presented to the outpatient department with multiple ulcerative lesions over both upper extremities and chest. On physical examination, the patient had pulseless upper limbs. Laboratory investigations revealed positive antinuclear antibodies (ANA) and…

Cerebrovascular Ischemic Events in Patients With Takayasu Arteritis.

BACKGROUND: Takayasu arteritis (TA) is a large vessel vasculitis that may complicate with cerebrovascular ischemic events. The objective was to describe clinical and vascular features of TA patients with cerebrovascular ischemic events and to identify risk factors for these events. METHODS: We analyzed the prevalence and type of stroke/transient ischemic attack (TIA), factors associated with cerebrovascular ischemic events, and stroke-free survival in a large cohort fulfilling the American College of Rheumatology or Ishikawa criteria of TA. RESULTS: Among 320 patients with TA (median age at …
