Early consonant production in Swedish infants with and without unilateral cleft lip and palate and two-stage palatal repair.

OBJECTIVE: To investigate consonant production at 12 and 18 months of age following early soft palate repair in infants with unilateral cleft lip and palate (UCLP), and to compare it with typically developing children without clefts. DESIGN: Randomized study with comparison group. PARTICIPANTS: Twenty Swedish infants born with UCLP and 21 without clefts (COMP) were included in a randomized trial of palatal surgery (Scandcleft project). Soft palate closure was completed at age 5 months; hard palate closure was performed in 11 of the infants with UCLP at 1 year of age (HPC) and was left open i

Clicks produced as compensatory articulations in two adolescents with velocardiofacial syndrome.

OBJECTIVE: To report perceptual and dynamic articulatory electropalatography data on clicks produced as compensatory articulations by two adolescents (S1 and S2) with velocardiofacial syndrome and velopharyngeal dysfunction. RESULTS: The perceptual analysis showed that both speakers produced click substitutions for English targets /t/, /d/, /k/, /g/, and S2 additionally produced clicks for /p/, /b/, and /tintegral/, //. The adolescents produced a range of clicks, which varied in placement (bilabial, dental, alveolar, palatal), voicing (voiced, voiceless), and nasality (nasal, nonnasal). Measu

Using a speech-generating device to enhance communicative abilities for an adult with moderate intellectual disability.

For adults with disabilities who are unable to speak, the literature recommends that intervention include augmentative and alternative communication (AAC) to improve communication and interactions with others. Some adults with moderate intellectual disabilities who exhibit limited functional speech are often overlooked as candidates for AAC interventions because they have some speech abilities. The perception is that they are too old to improve their language and communication skills. This article presents a case report of a 30-year-old woman with a moderate intellectual disability and a sever

[Assessment of dysarthria and dysphagia in ALS patients].

Swallowing and speech disorders are the dramatic consequences of bulbar and pseudo-bulbar syndrome in ALS. Evaluation is necessary to guide speech therapy and to measure the effects of treatment. This article revues the different examinations used to assess bulbar and pseudobulbar involvement in an ALS patient: oromotor assessment, evaluation of the functions with self assessment, perceptive and objective evaluation of speech disorders, fiberoptic endoscopic evaluation of dysphagia (FEES) and videofluoroscopy.

Low intensity phonological awareness training in a preschool classroom for children with communication impairments.

UNLABELLED: Phonological awareness is a term that refers to one's knowledge of the sound structure of spoken language. Children who understand that sounds in language represent the letters used in reading and writing typically learn to read more easily than children who do not. Children with language and/or speech impairments often demonstrate a lack of phonological awareness. Thus, it is important to identify problems in phonological awareness and to implement intervention programs early. The purpose of this study was to determine if a low intensity, classroom phonological awareness program i

Association between velopharyngeal function and dental-consonant misarticulations in children with cleft lip/palate.

We studied the association between velopharyngeal function and misarticulation of the dental consonants /r/, /s/ and /l/ in children with cleft lip/palate. We assessed 278 6-year-old Finnish-speaking non-syndromic children (115 girls, 163 boys) with isolated cleft palate (n= 81), cleft lip/alveolus (n= 82) or unilateral (n= 84) or bilateral (n= 31) cleft lip and palate. Auditory analysis of speech and velopharyngeal function, the presence of fistulae, previous velopharyngoplasty and speech therapy, as well as surgical technique and timing of primary palatal surgery were obtained from the hospi

The role of pharyngoplasty in congenital neurogenic speech disorders.

Children with dysarthria are often seriously handicapped by their inability to communicate. Velopharyngeal incompetence may contribute to their speech difficulties but the indications for pharyngoplasty are poorly defined. The speech of 41 children, thought to have a defect of neurological origin and referred for possible pharyngoplasty, was assessed by a speech therapist, by nasopharyngoscopy and by multiview videofluoroscopy. These investigations revealed that 16 had velopharyngeal incompetence, with a neurological aetiology, and might be expected to benefit from pharyngoplasty. Independent

Speech production following partial glossectomy.

Changes in the dimensions and patterns of articulation used by three speakers to compensate for different amounts of tongue tissue excised during partial glossectomy were investigated. Place of articulation was shifted to parts of the vocal tract congruent with the speakers' surgically altered lingual morphology. Certain metrical properties of the articulatory gestures, such as width of the sibilant groove, were maintained. Intelligibility data indicated that perceptually acceptable substitute sounds could be produced by such transposed gestures.

Automatic initial and final segmentation in cleft palate speech of Mandarin speakers.

The speech unit segmentation is an important pre-processing step in the analysis of cleft palate speech. In Mandarin, one syllable is composed of two parts: initial and final. In cleft palate speech, the resonance disorders occur at the finals and the voiced initials, while the articulation disorders occur at the unvoiced initials. Thus, the initials and finals are the minimum speech units, which could reflect the characteristics of cleft palate speech disorders. In this work, an automatic initial/final segmentation method is proposed. It is an important preprocessing step in cleft palate spee…

A novel entity of massive multifocal osteolyses in the elderly.

Osteolyses are common findings in elderly patients and most frequently represent malignant or locally aggressive bone tumors, infection, inflammatory and endocrine disorders, histiocytoses, and rare diseases such as Gorham-Stout syndrome. We here report on a novel entity of massive multifocal osteolyses in both shoulders, the right hip and left knee joint and the dens of an 83-year-old patient not relatable to any previously known etiopathology of bone disorders. The soft tissue mass is of myxoid stroma with an unspecific granulomatous inflammatory process, aggressively destroying extensive co

Auditory-Perceptual Features of Speech in Children and Adults With Down Syndrome: A Speech Profile Analysis.

Purpose The aim of this study was to determine how the speech disorder profiles in Down syndrome (DS) relate to reduced intelligibility, atypical overall quality, and impairments in the subsystems of speech production (phonation, articulation, resonance, and prosody). Method Auditory-perceptual ratings of intelligibility, overall quality, and features associated with the subsystems of speech production were obtained from recordings of 79 children and adults with DS. Ratings were made for sustained vowels (62 of 79 speakers) and short sentences (79 speakers). The data were analyzed to determine

[Repetitive transcranial magnetic stimulation. A reasonable adjuvant therapeutic method in the treatment of post-stroke aphasia?].

Repetitive transcranial magnetic stimulation (rTMS) is a non-invasive, painless method used to stimulate areas of the brain through the intact skull by means of magnetic fields. Depending on the stimulation frequency, the effect on concurrent brain areas is either inhibiting or exciting. As a result, it should be possible to inhibit compensatory hyperactivation in certain brain areas or to temporarily enhance cortical excitability. Therefore, rTMS potentially represents an adjuvant treatment for aphasia. In this article, the literature regarding rTMS as a treatment for aphasia is reviewed and

Evaluation for language and speech development in Kabuki make-up syndrome: a case report.

Kabuki make-up syndrome (KMS) is defined as a rare syndrome with mental retardation, growth deficiency and multiple anomalies of unknown cause. Cases have a characteristic facial appearance of broad and low auricles, wide forehead, broad and flattened nose root. In this article, 4-year, 10-month-old boy with speech delay reported due to characteristics of the facial appearance is considered as KMS, a rare syndrome. Otological, audiological and developmental evaluation of the patient consisted of six parameters. (1) ENT examination: normal. (2) Audiological findings were at normal hearing level

[Examination of 1,300 children for incidence and therapy of hearing disorders in pediatric speech disorders].

This study presents the results on 1305 children with speech disorders. The examination includes ENT-inspection, audiometry, speech-evaluations and if required, psychological tests. In 48% we found hearing losses, most frequently in the age up to four years. In 95% hearing losses were fluctuating about 20 decibels, caused by malfunction of the tube or by adenoids, not even in autumn and winter, but also all the year. In this case, fluctuating hearing function reduces the total hearing input over the year. In only 5% we found cochlear hearing loss. In hearing impaired children speech developmen

Automatic speech-based assessment to discriminate Parkinson's disease from essential tremor with a cross-language approach.

Parkinson's disease (PD) and essential tremor (ET) are prevalent movement disorders that mainly affect elderly people, presenting diagnostic challenges due to shared clinical features. While both disorders exhibit distinct speech patterns-hypokinetic dysarthria in PD and hyperkinetic dysarthria in ET-the efficacy of speech assessment for differentiation remains unexplored. Developing technology for automatic discrimination could enable early diagnosis and continuous monitoring. However, the lack of data for investigating speech behavior in these patients has inhibited the development of a fram

Systemic distribution and speciation of diphenylarsinic acid fed to rats.

Diphenylarsinic acid (DPAA) is an environmental degradation product of diphenylarsine chloride or diphenylarsine cyanide, which were chemical warfare agents produced by Japan during the World War II. DPAA is now considered a dangerous environmental pollutant in Kamisu, Japan, where it is suspected of inducing health effects that include articulation disorders (cerebellar ataxia of the extremities and trunk), involuntary movements (myoclonus and tremor), and sleep disorders. In order to elucidate the toxic mechanism of DPAA, we focused on the distribution and metabolism of DPAA in rats. Systemi…

"Not all my fault": genetics, stigma, and personal responsibility for women with eating disorders.

Medical researchers and clinicians increasingly understand and present eating disorders (anorexia and bulimia nervosa) as biologically-based psychiatric disorders, with genetic risk factors established by high heritability estimates in twin studies. But there has been no research on interpretation of genetic involvement by people with eating disorders, who may hold other views. Their interpretations are particularly important given the frequent presumption that biogenetic framing will reduce stigma, and recent findings that it exacerbates stigma for other mental illnesses.
