Serial assessment of echocardiographic measures in Friedreich ataxia.

BACKGROUND: Cardiomyopathy is the most common cause of death in Friedreich ataxia (FRDA), but the relationship between genetic factors, disease course, echocardiographic measures, and clinical outcomes is not completely defined. METHODS: Data on echocardiographic results and disease status were obtained from records from a large cohort of subjects followed at the Children's Hospital of Philadelphia (CHOP) or other locations. We compared clinical features with long term clinical outcomes and echocardiographic markers of cardiomyopathy using correlation and linear regression. RESULTS: Overall,…

Case Report: Ataxia telangiectasia with severe hemorrhagic cystitis.

BACKGROUND: Ataxia telangiectasia (AT) is a rare autosomal recessive genetic disorder caused by variants in the ataxia-telangiectasia mutated (ATM) gene. AT is characterized by progressive cerebellar degeneration, telangiectasia, immunodeficiency, cancer susceptibility, and radiosensitivity. This report presents a case of classic AT complicated by severe hemorrhagic cystitis, a rare clinical manifestation. Genetic analysis revealed novel variants in the ATM gene. CASE PRESENTATION: A 12-year-old Han Chinese boy presented with recurrent gross hematuria that progressed in frequency and severity…

Synaptophysin autoantibodies mediate synaptic dysfunction in cerebellar ataxia.

Cerebellar ataxias (CAs) encompass a wide spectrum of genetic and sporadic origins, of which a portion is driven by immune-mediated pathomechanisms. While some patients harbor known autoantibodies, assisting diagnosis and treatment, many remain seronegative. Here, we identify synaptophysin (SYP) as an autoantigen in CA by serum IgG staining on primate tissue, combined with human protein array and cell-based assay (CBA). SYP is abundant in presynaptic vesicles and is transiently exposed on the cell surface. Out of 43 patients with CA with synaptic serum reactivity on cerebellar sections, SYP an…

RAB24 Missense Variant in Dogs with Cerebellar Ataxia.

Hereditary ataxias are a highly heterogenous group of diseases characterized by loss of coordination. In this study, we investigated a family of random-bred dogs, in which two siblings were affected by a slowly progressive ataxia. They presented with clinical signs of progressive cerebellar ataxia, hypermetria, and absent menace response. The MRI revealed generalized brain atrophy, reduced cortical demarcation, hypoplastic corpus callosum, and cerebellar folia thinning, highly suggestive of a neurodegenerative disorder. We sequenced the genomes of the two affected dogs and their unaffected par…

Impulsivity in cerebellar ataxia: an online, multidimensional assessment.

While considered a motor control structure, the cerebellum contributes to non-motor functions, including impulsivity. However, whether it contributes to impulsivity in a domain-specific manner remains unknown. Studies on cerebellar ataxia (CA), a common model for cerebellar dysfunction, typically have small sample sizes, limiting robustness. In a multicenter cross-sectional study, we investigated the cerebellum's role in various forms of impulsivity by comparing large cohorts of CA to age- and education-matched neurotypical healthy (NH) controls. Additionally, to examine the ability to identif…

Oxidative Stress and Antioxidant Therapies in Friedreich's Ataxia.

The pathogenesis of Friedreich's ataxia (FRDA) remains poorly understood. The most important event is the deficiency of frataxin, a protein related to iron metabolism and, therefore, involved in oxidative stress. Studies on oxidative stress markers and gene expression in FRDA patients have yielded inconclusive results. This is largely due to the limited number of studies, small sample sizes, and methodological differences. A notable finding is the decreased activity of mitochondrial respiratory chain complexes I, II, and III, as well as aconitase, in endomyocardial tissue. In contrast, numerou…

Survival in Brazilian Patients with Friedreich´s Ataxia.

Friedreich's Ataxia (FRDA) is a progressive condition leading to reduced life expectancy in European/North American cohorts, but little is known about Latin American cohorts. Herein, we assessed FRDA survival data from a large Brazilian reference center (UNICAMP). We conducted a retrospective study including patients with FRDA followed at UNICAMP between 1998 and 2025. For those patients who died, we recorded age at death. For those alive or lost to follow-up, we considered the age at last visit. Potential prognostic markers (sex, age at onset, presence of cardiomyopathy and diabetes) were exp…

CSF Markers of TG6 Autoimmunity in Gluten Ataxia.

Gluten ataxia (GA) is the primary neurological manifestation of gluten sensitivity, characterised by loss of Purkinje cells throughout the cerebellar cortex and rooted in autoimmunity to transglutaminase 6 (TG6). Previous studies have shown the contribution of serum anti-TG6 antibodies to disease progression; however, it remains unclear where these antibodies are produced and how they gain access into the brain parenchyma. This study aims to provide an immunological assessment of the CSF in patients with GA to better define the humoral response contributing to disease pathophysiology. In this …

Spinocerebellar Ataxia Type 10 (SCA 10) in Brazil.

Spinocerebellar ataxia type 10 (SCA10) is an autosomal dominant inherited ataxia caused by the expansion of ATTCT pentanucleotide repeats in intron 9 of the ATXN10 gene. This rare form of SCA has predominantly been observed in individuals of Indigenous American and East Asian descent. Notably, in Mexico and the southern Brazilian states of Paraná and Santa Catarina, SCA10 is identified as the second most prevalent type of spinocerebellar ataxia. Initially, the phenotype described in Mexico featured a combination of cerebellar ataxia and epilepsy-a presentation also observed in other Latin Amer…

Generalized Cerebellar Ataxia of Acute Onset: Case Report.

In this case report, we describe a male patient, aged 65 years, with previous arterial hypertension and atrial fibrillation, with irregular use of losartan, presenting with sudden onset of dizziness, dysarthria, and bilateral ataxia, 5.5 h before the admission to the neurological emergency department. The cranial magnetic resonance imaging revealed hyperintensities on T2/FLAIR sequences, and diffusion restriction in the territory irrigated by both superior cerebellar arteries, alongside with left pontine, compatible with infarct. The CT angiography of the cervical and cerebral arteries reveale…

Vestibular Loss and Cerebellar Ataxia: A Practical Approach.

Cerebellar ataxia has been the remit of neurology but increased awareness of genetic disorders affecting both cerebellar and vestibular function has led to a rise in their reported prevalence. It is not uncommon for such patients to be seen in otolaryngology clinics. This review explores the underlying etiologies of patients presenting with vestibular loss accompanied by signs of cerebellar ataxia and provides a practical approach to diagnosis. We provide a comprehensive overview of common conditions that can manifest with both vestibular dysfunction and cerebellar ataxia, acutely or chronical…

Genetic Determined Iron Starvation Signature in Friedreich's Ataxia.

BACKGROUND: Early studies in cellular models suggested an iron accumulation in Friedreich's ataxia (FA), yet findings from patients are lacking. OBJECTIVES: The objective is to characterize systemic iron metabolism, body iron storages, and intracellular iron regulation in FA patients. METHODS: In FA patients and matched healthy controls, we assessed serum iron parameters, regulatory hormones as well as the expression of regulatory proteins and iron distribution in peripheral blood mononuclear cells (PBMCs). We applied magnetic resonance imaging with R2*-relaxometry to quantify iron storages …

ITPR1: The missing gene in miosis-ataxia syndrome?

The association of early-onset non-progressive ataxia and miosis is an extremely rare phenotypic entity occasionally reported in the literature. To date, only one family (two siblings and their mother) has benefited from a genetic diagnosis by the identification of a missense heterozygous variant (p.Arg36Cys) in the ITPR1 gene. This gene encodes the inositol 1,4,5-trisphosphate receptor type 1, an intracellular channel that mediates calcium release from the endoplasmic reticulum. Deleterious variants in this gene are known to be associated with two types of spinocerebellar ataxia, SCA15 and SC…
