Epidemiology and Outcomes Associated with PR Prolongation.

PR prolongation is defined primarily as delayed conduction through the AV node, but can also signify delayed electrical impulse propagation through any part of the conduction system. The prevalence of PR prolongation ranges from 1% to 5% in patients younger than 50 years, with increasing prevalence, after the sixth decade of life and in patients with organic heart disease. Contemporary studies have documented increased risk of atrial arrhythmias, heart failure, and mortality in patients with PR prolongation. Future studies are needed to more accurately risk stratify elderly patients with PR pr

A conversion CRT strategy combined with AVJA may be a perspective alternative for heart failure patients with persistent atrial fibrillation.

Heart failure (HF) combined with persistent atrial fibrillation (AF) often coexist and may promote the pathological conditions of cardiac dysfunction, leading to poor prognosis. Cardiac resynchronization therapy (CRT) combined with atrioventricular junction ablation (AVJA) is a highly effective treatment for HF patients with underlying AF who either have failed or are not suitable for catheter ablation. The CRT-AVJA combination therapy can improve clinical outcomes in HF patients. Currently, clinical CRT methods are categorized into biventricular pacing (BVP) - based and conduction system paci

Long-Term Outcome of Cardiac Denervation Procedures: The Anatomically Guided Septal Approach.

BACKGROUND: Functional bradycardia is a challenging condition that affects a healthy population. Ganglionated plexus ablation has emerged as … a negative atropine test after ablation. Twenty-eight (77.7%) patients presented immediate sings of atrioventricular node denervation, with a shortening of 15.6% of mean Wenckebach cycle length and 15.9% of the mean AH interval. All heart rate variability parameters showed a significant reduction after 12 months, enduring after 18 months. Thirty (83.3%) patients remain …

Regular ventricular rate and "reverse bigeminy" in 3:2 Wenckebach periodicity.

One of the more common causes of bigeminy at the ventricular level is type 1 second-degree atrioventricular (AV) block with 3:2 conduction ratio. In 3:2 Wenckebach, the shorter cycles reflect the consecutively conducted impulses and the longer cycles coincide with the blocked P waves. Theoretically, however, depending on the degree of conduction delay between the first and second transmitted impulses, other types of spacing of the QRS complexes may become possible. In this retrospective study of 180 patients who underwent electrophysiologic studies for symptomatic arrhythmias, atrial pacing-in

Atrioventricular conduction disturbances in a young patient with Fabry's disease without other signs of cardiac involvement.

A 30 year-old male patient with a history of Fabry's disease, was referred to hospital with symptoms of dizziness, hypotension and weakness. Fabry's disease had been diagnosed 2 years before, based on angiokeratoma and hypohidrosis on physical examination and complete lack of alpha-galactosidase A on laboratory examination. The ECG on admission demonstrated sinus bradycardia, with a poor response to atropine administration. Echocardiograms on admission and 2 years before were normal, as well as Holter ambulatory ECG recording. Subsequent electrophysiological study demonstrated mild AV conducti

Selection of pacing mode after interruption of atrioventricular conduction for atrial fibrillation: observations from the PA3 clinical trial.

The optimal pacing mode for patients with paroxysmal atrial fibrillation (AF) following AV junction ablation remains the subject of some debate. Recent clinical trials have not demonstrated a superior advantage of maintenance of sinus rhythm over the rate control approach. However, clinical trials in pacemaker populations have demonstrated that physiologic pacing reduces the probability of paroxysmal and persistent AF compared to ventricular pacing. In the second phase of the PA(3) study, patients were randomized to DDDR versus VDD pacing in a cross over study design. Of the 67 patients random

[Transient atrioventricular conduction 1:1 in a patient with common atrial flutter following the administration of adenosine triphosphate].

A patient with atrial flutter and 2:1 atrioventricular conduction and acceleration to 1:1 conduction after administration of a single i.v. dose of 10 mg adenosin triphosphate (ATP) is presented. Despite the fact that ATP is a very useful drug for the treatment of paroxysmal supraventricular tachycardia. Its use as a diagnostic tool in atrial flutter must be carefully considered and the possibility of transient acceleration of AV conduction must be taken into account.

[An experimental study on the selective ablation of AV node].

26 dogs were divided into two equal groups. Each dog was anesthetized, and a tetrapolar electrode was inserted transvenously to the area of AV junction. Unipolar leads of HBE were recorded with each of the four electrodes. The electrode where the highest H wave was recorded was considered to the contiguous to the His bundle, and the first or the second electrode proximal to the above-mentioned one was considered as in the vicinity of the AV node. These electrodes were used as cathodes, and electric discharges were performed with an external defibrillator. Two dogs in each group died of VF duri

Atrial fibrillation resulting from cardiac trauma.

Reports in the literature concerning isolated traumatic lesions of the conductive system of the heart following blunt injury to the thorax are rare. A 69-year-old woman who was a front-seat passenger developed disturbances of conduction with atrial fibrillation after blunt injury to the thorax. She died 7 days later. Autopsy and subsequent histological investigation of the conductive system of the heart showed isolated contusion in the right atrium close to the atrio-ventricular node.

Electrophysiologic characteristics of concealed bypass tracts: clinical and electrocardiographic correlates.

Twelve of 60 consecutively studied patients undergoing electrophysiologic study for paroxysmal supraventricular tachycardia had atrioventricular (A-V) bypass tracts functioning as the retrograde limb of the reentrant circuit. None had evidence of preexcitation in the surface electrocardiogram, but in two patients anterograde preexcitation could be produced by pacing from the coronary sinus. In all 12 patients with concealed bypass tracts the retrograde atrial activation sequence or effect of left bundle branch block aberration during the tachycardia, or both, confirmed the left-sided bypass tr

Association Between Bradyarrhythmia Requiring Permanent Pacemaker Implantation and Epicardial Adipose Tissue in Elderly Patients.

BACKGROUND: Aging is one of the most significant risk factors for sinus node (SN) and atrioventricular node (AVN) dysfunction. Epicardial adipose tissue (EAT) is reported to promote myocardial fibrosis and influence intracardiac conduction through the release of inflammatory cytokines and direct tissue infiltration. The SN and AVN are in contact with EAT, which may affect the SN function and AVN conduction. The purpose of this study was to examine the association between bradyarrhythmia and EAT. METHODS: We enrolled 103 patients with bradyarrhythmia requiring permanent pacemaker (PM) implanta…

Indications for the use of pacing pulmonary artery catheters in cardiac surgery.

Several varieties of pulmonary artery catheters (PACs) with pacing capabilities are now available. Although specific recommendations for prophylactic perioperative placement of pacemakers have been offered previously, the authors believe that those recommendations warrant further examination, taking into consideration the availability of new pacing modalities. Toward this end, the use of pacing PACs in cardiac surgical patients was prospectively examined. In 600 consecutive adult patients with PACs placed prior to cardiopulmonary bypass (CPB), the cardiac anesthesiologist recorded if a pacing
