B.fragilis is an obligate anaerobic commensal colonizing human intestines and carries number of physiological functions. Sinuses and fistulae of the third branchial cleft or pouch are rarely encountered. We have cared for a child with a history of multiple neck masses and abscesses, in whom the causative factor seems to have been an unrecognized branchial cleft sinus. This case suggests the need for contrast radiography of the hypopharynx in children with recurrent neck abscess, and dramatizes the need to exercise all reasonable diagnostic options before proceeding with surgery of newly presenting neck masses in children.

Over the period 1981-92, 32 bacterial isolates were referred to the Microbiological Diagnostic Unit from infected dog-bite wounds and 10 isolates were submitted from blood cultures after dog bites or close contact with dogs. Eleven cases of bacteremia in diabetic patients with infected lower extremities at Rancho Los Amigos Hospital (RLAH) were observed over a 34-mo period. Familial Mediterranean fever (FMF) can present cutaneous symptoms. In the reported case, infiltrating perineal and scrotal inflammation were attributed to FMF and treated by systemic medication with colchicine. The poor outcome of this conservative approach and pathognomonic axillary dermatological findings allowed the diagnosis of acne inversa to be made. Knowledge of this clinical picture possibly including genital manifestations is crucial, as early excision of all affected regions is the therapy of choice that enables healing.

Odoribacter splanchnicus was recently reclassified from the genus Bacteroides. We present the first case of Odoribacter splanchnicus bacteremia following appendicitis. The species was identified using MALDI-TOF mass spectrometry and later confirmed with 16S rRNA sequencing. The patient was successfully managed with surgery and antibiotic administration for two weeks.

Most episodes of acute suppurative parotitis are caused by Staphylococcus aureus. Reported here is a patient who had acute parotitis associated with anaerobic sepsis caused by Bacteroides melaninogenicus and Peptostreptococcus micros. Acute parotitis should be included in the ever-widening spectrum of clinical infections caused by anaerobic bacteria.
