High-resolution spiral computed tomography coronary angiography in patients referred for diagnostic conventional coronary angiography.

BACKGROUND: The diagnostic performance of the latest 64-slice CT scanner, with increased temporal (165 ms) and spatial (0.4 mm3) resolution, to detect significant stenoses in the clinically relevant coronary tree is unknown. METHODS AND RESULTS: We studied 52 patients (34 men; mean age, 59.6+/-12.1 years) with atypical chest pain, stable or unstable angina pectoris, or non-ST-segment elevation myocardial infarction scheduled for diagnostic conventional coronary angiography. All patients had stable sinus rhythm. Patients with initial heart rates > or =70 bpm received beta-blockers. Mean scan t

[The mitral prolapse syndrome. Observation of 100 cases (author's transl)].

100 patients with mitral prolapse syndrome (MPS) were studied with electrocardiogram, phonocardiogram, thorax radiography and echocardiogram. In 44 of the patients, the effort test and dynamic electrocardiogram were performed; 25 underwent hemodynamic examination, and 10 underwent selective coronarography. The most frequent symptomatologies were palpitations and vertigo; the most-registered electrocardiographic alterations were ventricular extrasystoles and altered electrical repolarization in the inferior seat. The symptomatic patients with rhythm disturbances showed reduced work capacity (RW

Suboptimal Engagement of High-Level Cortical Regions Predicts Random-Noise-Related Gains in Sustained Attention.

Interindividual variability in outcomes across individuals poses great challenges for the application of noninvasive brain stimulation in psychological research. Here, we examined how the effects of high-frequency transcranial random-noise stimulation (tRNS) on sustained attention varied as a function of a well-studied electrocortical marker: spontaneous theta:beta ratio. Seventy-two participants received sham, 1-mA, and 2-mA tRNS in a double-blind, crossover manner while they performed a sustained-attention task. Receiving 1-mA tRNS was associated with improved sustained attention, whereas th

Periodic limb movements in sleep are followed by increases in EEG activity, blood pressure, and heart rate during sleep.

PURPOSE: Periodic limb movements in sleep (PLMS) are related to arousal, sympathetic activation, and increases in blood pressure (BP), but whether they are part of the arousal process or causative of it is unclear. Our objective was to assess the temporal distribution of arousal-related measures around PLMS. METHODS: Polysomnographic recordings of six patients with restless legs syndrome were analyzed. We analyzed 15 PLMS, plus three 5-s epochs before and after each movement, for every patient. Mean values per epoch of blood pressure (BP), heart rate (HR), and electroencephalographic (EEG) po

[Clinical aspects of thalassemia (or microcythemia)].

The thalassaemias are genetic syndromes brought about by a low or nil synthesis of one or more of the main globinic chains and by consequent imbalance of the normal ratio between alpha and non-alpha chains. Three basic pictures can be distinguished: (1) microcythemia or thalassaemia minima, the expression of heterozygosis for one microcythemic gene, which includes beta, delta beta and alpha microcythemias. Subjects are healthy but very often pale and asthenic, (2) intermediate thalassaemia, very similar to thalassaemia major, though a less severe disease. It is the expression of the presence o

Neural basis of lower-limb visual feedback therapy: an EEG study in healthy subjects.

BACKGROUND: Video-feedback observational therapy (VOT) is an intensive rehabilitation technique based on movement repetition and visualization that has shown benefits for motor rehabilitation of the upper and lower limbs. Despite an increase in recent literature on the neurophysiological effects of VOT in the upper limb, there is little knowledge about the cortical effects of visual feedback therapies when applied to the lower limbs. The aim of our study was to better understand the neurophysiological effects of VOT. Thus, we identified and compared the EEG biomarkers of healthy subjects under

Impact of fasciculoventricular bypass tracts on the diagnosis and treatment of concomitant arrhythmias and cardiac diseases.

BACKGROUND: Fasciculoventricular (FV) bypass tracts (BTs) are the rarest form of ventricular preexcitation. Although they are not involved in clinically significant reentrant tachycardia, they may cause diagnostic and therapeutic confusion if not properly understood. This study aimed to assess the impact of FV BTs on the diagnosis and treatment of concomitant arrhythmias and cardiac diseases. METHODS: Twenty-two patients with FV BTs who underwent electrophysiologic (EP) study were evaluated. The prevalence of concomitant arrhythmias and cardiac diseases in FV BTs was evaluated. The mechanisms

High mobility group box 1 was associated with thrombosis in patients with atrial fibrillation.

INTRODUCTION: High mobility group box 1 (HMGB1) is a member of the HMGB family that is involved in inflammatory disease-related thrombosis. We hypothesize that HMGB1 and its downstream factors are associated with thrombosis in atrial fibrillation (AF). MATERIALS AND METHODS: Our experimental materials were the left atrial appendage (LAA) tissues from patients undergoing valve replacement. The samples were divided into 3 groups: a sinus rhythm group (n = 15), an AF(+)thrombus(-)group (n = 15), and an AF(+) thrombus (+)group (n = 15). The expression of HMGB1, Toll-like receptor 4 (TLR4), advanc

Long-term results of radiofrequency maze procedure for persistent atrial fibrillation with concomitant mitral surgery.

BACKGROUND: The radiofrequency (RF) maze procedure can effectively restore sinus rhythm in most patients with persistent atrial fibrillation (AF) and mitral disease. However, long-term results and predictors for late AF recurrence are still under investigation. METHODS: From December 1995 to November 2011, 207 consecutive patients with persistent AF and mitral disease underwent RF maze procedure and concomitant mitral surgery. The mean age was 54±12.4 year-old. Mitral surgery was performed in all patients and concomitant procedures including tricuspid surgery, aortic valve surgery, and atrial

Pretranslational regulation of rhythmic type II iodothyronine deiodinase expression by beta-adrenergic mechanism in the rat pineal gland.

It has been demonstrated that type II iodothyronine deiodinase is present in rat pineal gland, and the deiodinase activity markedly increases during the hours of darkness, primarily through beta-adrenergic mechanism. We have studied the relationship between pineal type II iodothyronine deiodinase messenger RNA (mRNA) and the deiodinase activity to elucidate the mechanisms involved in the nocturnal rise in pineal deiodinase activity. Northern analysis has demonstrated that type II iodothyronine deiodinase mRNA is expressed in rat pineal gland, and the mRNA markedly increases during the hours of

Cardiac arrhythmias: diagnosis and management. The bradycardias.

OBJECTIVE: To review the diagnosis and management of cardiac arrhythmias in a two-part presentation. DATA SOURCES: Articles and published peer-review abstracts on tachycardias and bradycardias. SUMMARY OF REVIEW: Bradycardias are caused by a failure of the sinus node to generate normal impulses or due to a defect in cardiac conduction that in turn causes a delay or failure of impulse propagation. During sleep, the heart rate may decrease to 30 beats per minute (bpm) with episodes of sinoatrial block, junctional rhythms and first and second degree atrioventricular block that occur often enoug

Isolated Left Ventricular Apical Hypoplasia Presenting as Vasovagal Syncope in a Middle-Aged Male Patient.

BACKGROUND: Isolated left ventricular apical hypoplasia (ILVAH) is a rare congenital cardiomyopathy characterized by a truncated and spherical left ventricle with compensatory right ventricle elongation. Clinical presentation is variable and often delayed. CASE SUMMARY: We report a 40-year-old man with no comorbidities who presented with a vasovagal episode. Electrocardiogram showed sinus rhythm with incomplete left bundle branch block. Echocardiography suggested dilated cardiomyopathy; however, cardiac magnetic resonance imaging (CMR) revealed the classic features of isolated left ventricula

The Long Shadow of Repair: Late-Onset Atrioventricular Block and Atrial Arrhythmias After Scimitar Syndrome and Mitral Annuloplasty.

Background and Clinical Significance: Scimitar Syndrome is a rare congenital cardiopulmonary anomaly characterized by partial anomalous pulmonary venous return, often requiring early surgical correction. It may coexist with other congenital or acquired cardiovascular anomalies, including valvular diseases such as mitral regurgitation. When surgical correction of Scimitar Syndrome is combined with mitral valve annuloplasty, the proximity to the atrioventricular node may potentially predispose patients to late-onset conduction disturbances, although causality remains speculative. Optogenetic targeting of cortical astrocytes selectively improves NREM sleep in an Alzheimer's disease mouse model.

Alzheimer's disease (AD) is a progressive neurodegenerative condition marked by memory impairments and distinct histopathological features such as amyloid-beta (Aβ) accumulations. Alzheimer's patients experience sleep disturbances at early stages of the disease. APPswe/PS1dE9 (APP) mice exhibit sleep disruptions, including reductions in non-rapid eye movement (NREM) sleep, that contribute to their disease progression. In addition, astrocytic calcium transients associated with a sleep-dependent brain rhythm, slow oscillations prevalent during NREM sleep, are disrupted in APP mice. However, at p

SMAD4 Overexpression in Patients with Sleep Apnoea May Be Associated with Cardiometabolic Comorbidities.

Obstructive sleep apnoea (OSA) is associated with several diseases related to metabolic and cardiovascular risk. Although the mechanisms involved in the development of these disorders may vary, OSA patients frequently present an increase in transforming growth factor beta (TGFβ), the activity of which is higher still in patients with hypertension, diabetes or cardiovascular morbidity. Smad4 is a member of the small mother against decapentaplegic homologue (Smad) family of signal transducers and acts as a central mediator of TGFβ signalling pathways. In this study, we evaluate Smad4 protein and

A Focus on Pharmacological Management of Catecholaminergic Polymorphic Ventricular Tachycardia.

Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a channelopathy characterized by adrenergic mediated ventricular arrhythmia. Untreated CPVT is a malignant syndrome with more than 50% of arrhythmic events and up to 25% of fatal or near-fatal cardiac events at 8 years follow-up. Prevention of sudden cardiac death starts with exclusion of competitive sports. Beta blockers (BB) are the cornerstone pharmacological therapy for the prevention of cardiac event in CPVT patients. Dose of BB should be highly tolerable, preferably nadolol. Efficiency of BB is undeniable but uncompleted. Th

Electroencephalographic modulations during an open- or closed-eyes motor task.

There is fundamental knowledge that during the resting state cerebral activity recorded by electroencephalography (EEG) is strongly modulated by the eyes-closed condition compared to the eyes-open condition, especially in the occipital lobe. However, little research has demonstrated the influence of the eyes-closed condition on the motor cortex, particularly during a self-paced movement. This prompted the question: How does the motor cortex activity change between the eyes-closed and eyes-open conditions? To answer this question, we recorded EEG signals from 15 voluntary healthy subjects who p

Perioperative management of phaeochromocytoma.

Phaeochromocytoma is a rare catecholamine producing tumour, feared for its life threatening cardiovascular disturbances during anaesthesia. Improved medical and anesthetic management resulted in reduction of perioperative phaeochromocytoma resection mortality from about 50% in the pioneer period to near 0% nowadays. Cardiomyopathy is usually reversible if managed properly. Stress related or (inverted) Tako Tsubo cardiomyopathy is a recent finding, deserving our attention. Preoperative alpha blockade should be performed to achieve cardiovascular stability and decrease uncontrolled intraoperativ
