We have experienced 59 jaundiced patients with unresectable carcinoma of the biliary tract or pancreas in our institute for 7 years and 6 months. A side-to-side anastomosis between a bile duct in the segment III of the liver and jejunum by Roux-en Y manner was preferred as a palliative internal drainage operation in 13 cases out of them. 112 patients with biliary calculi and destined for cholecystectomy were sonographically investigated in a prospective study carried immediately before the operation as well as between the third and fifth, and between the seventh and ninth postoperative day. Forty-six patients in whom sphincteroplasty was performed by an original technique are presented. In 43 cases the indication was oddian stenosis (associated or not with cholelithiasis or hepatic hydatid disease). Only in three patients a "security" operation was performed. In the 46-year period from September 1, 1932 to September 1, 1978, 11,808 patients were operated on for nonmalignant biliary tract disease. In 80.1% of these patients, the disease was considered chronic, and in 19.9%, acute inflammation was superimposed on the existing condition. A report is given of 72 patients suffering from carcinoma of the extrahepatic biliary ducts. Carcinoma of the gallbladder usually grow fast invading the surrounding tissue. INTRODUCTION: Biliary tract tumors comprise a group of poor prognosis malignancies, most of them being diagnosed at an advanced stage. Standard treatments include surgery with curative intent and adjuvant chemotherapy. Even so, the benefit obtained is marginal, and relapses are frequent.
AREAS COVERED: Here, we made a review of the literature trying to summarize all trials related to adjuvant therapy, as well as those yet to come. We also focused on detecting the unmet needs in this field, which may represent a change in the current strategy.
