Vocal cord dysfunction in athletes: clinical presentation and review of the literature.

Vocal cord dysfunction (VCD) is a syndrome characterized by the intermittent, abnormal paradoxical adduction of the true vocal cords during respiration resulting in variable upper airway obstruction. It is also commonly referred to as paradoxical vocal fold motion disorder. Patients with VCD usually present with intermittent shortness of breath of varying intensity, wheezing, stridor, choking, throat tightness, voice changes, or cough, and these symptoms often resolve quickly after relaxation or cessation of activity. Since first described as a distinct clinical entity in 1983, VCD remains und

[Treatment of levator ani syndrome: update and future developments].

Chronic proctalgia is defined by chronic or recurrent episodes of rectal pain or aching lasting at least 20 minutes in the absence of structural or systemic disease explanation for the pain syndrome. Digital rectal examination distinguishes between levator ani syndrome where the patient reports tenderness on palpation of the pubo-rectalis muscle and unspecified functional anorectal pain where no pain can be elicited. There is no consensus on its etiology, but chronic tension of the pelvic floor muscles is the most common view. Diagnosis is focused on excluding organic diseases potentially resp

Biofeedback therapy for rectal intussusception.

BACKGROUND: Surgery for isolated internal rectal intussusception is controversial due to high morbidity. Therefore, there is interest in other forms of treatment that are safe and effective. The aim of this study was to determine outcome and identify predictors for success of biofeedback therapy in patients with rectal intussusception. METHODS: We retrospectively evaluated the results of electromyography (EMG)-based biofeedback in 34 patients with rectal intussusception without any other major pelvic floor or colonic physiologic disorder. RESULTS: A total of 34 patients (7 men) had undergone

Outcome of laryngeal and velopharyngeal biofeedback treatment in children and young adults: a pilot study.

A relatively new management strategy for the treatment of voice disorders is the use of laryngeal (LB) and velopharyngeal biofeedback (VB). The main purpose of the present pilot study is to document the outcome of vocal and velopharyngeal performances after a well-defined LB and VB treatment. Four subjects were studied pretreatment (1 week before LB or VB treatment) and posttreatment (1 week after the LB or VB treatment). To measure and compare the effect of LB and VB, objective and subjective assessment techniques were used. Perceptual voice assessment included a perceptual rating of the voic

Biofeedback and pelvic floor exercises for the rehabilitation of urinary stress incontinence.

We investigated the effectiveness of pelvic floor muscle (PFM) exercises or biofeedback for the treatment of urinary stress incontinence (USI). Fifty patients with USI were included in this randomized, controlled, prospective study. Twenty patients were taught PFM exercises via digital palpation and instructed to perform regularly as home program. The second group of 20 patients had PFM exercises via biofeedback three times a week for 2 months. The third group of 10 patients did not have any exercises. The patients were evaluated via pad test, perineometry, digital palpation based PFM strength

Recent innovations in the management of fecal incontinence.

The importance of having a reproducible scoring system to objectively assess the severity of fecal incontinence and its response to treatment is stressed, and a number of methods are described. The approach to conservative treatment and investigation is outlined. Recent advances in the surgical management of fecal incontinence including the electrically stimulated gracilis neosphincter, sacral nerve stimulation, the artificial anal sphincter, internal anal sphincter augmentation, rectal augmentation, and antegrade colonic irrigation are described in detail.

Autogenic-feedback training exercise is superior to promethazine for control of motion sickness symptoms.

Motion sickness symptoms affect approximately 50% of the crew during space travel and are commonly treated with intramuscular injections of promethazine. The purpose of this paper is to compare the effectiveness of three treatments for motion sickness: intramuscular injections (i.m.) of promethazine, a physiological training method (autogenic-feedback training exercise [AFTE]), and a no-treatment control. An earlier study tested the effects of promethazine on cognitive and psychomotor performance and motion sickness tolerance in a rotating chair. For the present paper, motion sickness toleranc

Migraine treatment.

Migraine is a primary headache disorder characterized by recurring attacks of pain and associated symptoms. Migraine sufferers require a continuum of clinical care that depends on their disability and response to treatment. Treatment consists of: (1) prevention of attacks by avoidance of triggers; (2) the use of nonpharmacologic treatments; (3) treatment of the acute attack; and (4) long-term prophylactic therapy. Migraine is comorbid for affective disorders, epilepsy, stroke, and mitral valve prolapse. The therapy selected depends on the headache severity and frequency, the pattern of associa

Pseudodyssynergia (contraction of the external sphincter during voiding) misdiagnosed as chronic nonbacterial prostatitis and the role of biofeedback as a therapeutic option.

PURPOSE: Chronic lower urinary tract symptoms in young men are often attributed to misdiagnosed chronic nonbacterial prostatitis. We analyzed contraction of the external urinary sphincter during voiding (pseudodyssynergia) as an etiology of voiding dysfunction in men with misdiagnosed chronic prostatitis. MATERIALS AND METHODS: The video urodynamic studies of 43 men 23 to 50 years old with chronic voiding dysfunction secondary to pseudodyssynergia performed between January 1990 and June 1996 were retrospectively analyzed. Pseudodyssynergia was diagnosed based on several criteria, including el

EMG feedback-assisted postoperative rehabilitation of minor arthroscopic knee surgeries.

This study assessed the effectiveness of surface integrated electromyographic (EMG) biofeedback in the rehabilitation of 51 patients undergoing minor arthroscopic knee surgery. Prior to surgery, both control (n = 23) and treatment (n = 28) groups received verbal and written explanations of postoperative isometric exercises; the treatment group received additional instruction in the use of ambulatory biofeedback equipment during exercise. Isokinetic tests of strength at approximately two weeks post-surgery revealed that patients given EMG biofeedback during postoperative exercise demonstrated s

[Biofeedback in the treatment of fecal incontinence].

Four hundred and thirty seven consecutive patients with problems of motility were referred to, and examined at our third-level unit from 1987 to 1993. Forty seven (mean age 36.3 +/- 3.8, range 5-76 years; 25 females) fulfilled the established criteria for fecal incontinence. The mean period of total incontinence was 55.4 +/- 7.6 months (range 6 months to 21 years), with episodes of incontinence of 2.4 +/- 0.2 (range 1-7 a day). All patients were asked to complete a questionnaire in order to validate the incontinence diagnosis. A complete clinical history was obtained from each patient, and the

Long-term effects of auditory feedback training on relearned symmetrical body weight distribution in stroke patients. A follow-up study.

Long-term effects of auditory kinetic feedback on relearned symmetrical body weight distribution while rising and sitting down were studied in stroke patients. Thirty patients were re-tested on average 33 months after having trained with and without auditory feedback. Body weight distribution on the legs was measured with two force plates. At re-test there was a decrease from 48% to 39% of body weight distribution (%BWD) on the paretic leg in rising and in sitting down in the patients in the auditory feedback group (p < 0.001). In the control group the decrease was from 44 to 39% BWD on the pa

[Rehabilitation therapy in the treatment of female urinary incontinence. Our experience with 121 patients].

Rehabilitative therapy of the perineum (FKT, SEF and biofeedback) represents an efficient method in the treatment of urinary stress incontinence without resorting to a complete uro-genital prolapse, as in patients with urethral instability and/or incontinence. One hundred twenty-one women, 55 with urinary stress incontinence (USI), 20 with urge incontinence (UI) and 46 suffering from both types of incontinence, underwent rehabilitative therapy. On each patients the following tests were carried out: an internal test, a urodynamic test (uroflowmeter, PPU, cystomanometry and pessure/flow study),
