The effects of cognitive and perceptual load on inattentional blindness.

Inattentional blindness is often attributed to limited attentional resources. In principle, tasks that are more effortful, those that place greater demands on attention, should result in less noticing of unexpected objects. But that load on attention can take different forms, including both perceptual and cognitive components. Increasing the perceptual load by making selective attention more difficult typically increases inattentional blindness for unexpected objects, but the evidence for an effect of increased cognitive load on noticing has been more limited and mixed. In a set of five inatte…

Severe Headache and Acute Blindness: A Case of Pituitary Apoplexy.

Pituitary apoplexy is a rare, life-threatening syndrome that commonly occurs in the adenomatous pituitary gland. It presents with a myriad of severe neuroendocrine and ophthalmological signs and symptoms. Early diagnosis and prompt treatment are vital in the management of pituitary apoplexy. This case report describes a 56-year-old woman who reported to the neurology clinic with 1 week's history of visual loss, and a severe headache, on the background of a 2-year history of pituitary adenoma with poor follow-up. The patient underwent transsphenoidal surgery. She regained vision in the right ey

Important entomological events in the history of onchocerciasis ('river blindness').

This article reviews the major events that entomology has played, and still plays, in the history of onchocerciasis ('river blindness') from life-cycle discovery to the current global goal of elimination of parasite transmission in all endemic areas. The identified events include the various attempts to implement successful vector control, the important introduction of the microfilaricide ivermectin and the new improvements in laboratory-based techniques to identify infected blackflies and refine onchocerciasis epidemiology and programme monitoring. These scientific events are paralleled with …

Fulminant bilateral blindness despite initially normal neuroimaging in rhinocerebral mucormycosis.

BACKGROUND: Rhinocerebral mucormycosis is a rapidly progressive and frequently fatal invasive fungal infection that most commonly occurs in patients with uncontrolled diabetes mellitus in the setting of diabetic ketoacidosis. CASE PRESENTATION: We present a woman in her 70 s with diabetic ketoacidosis, who developed right-sided facial numbness and ptosis, followed by acute, complete bilateral vision loss within hours. Initial cranial CT and diffusion MRI were unremarkable despite rapidly evolving cranial neuropathies. This created significant diagnostic uncertainty and prompted consideration

Unraveling the genetic spectrum of inherited deaf-blindness in Portugal.

BACKGROUND: Syndromic genetic disorders affecting vision can also cause hearing loss, and Usher syndrome is by far the most common etiology. However, many other conditions can present dual sensory impairment. Accurate diagnosis is essential for providing patients with genetic counseling, prognostic information, and appropriate resources. This study aimed to describe the genetic profile of combined inherited deaf-blindness in Portugal. METHODS: This was a cross-sectional study conducted at a tertiary hospital in Portugal. Patients were identified through the national, web-based inherited retin…

[Acute blindness in a patient with metformin-associated lactic acidosis].

Lactic acidosis is a rare metabolic complication that can occur in patients with diabetes mellitus type 2 who use metformin. We discuss a 79-year old woman with metformin-associated lactic acidosis (MALA) and acute kidney injury based on gastroenteritis. Patient reported acute blindness which in literature is described as a rare presentation of a metabolic acidosis (regardless of its underlying cause). Immediate treatment with hemodialysis led to improvement of the acidosis and complete recovery of the vision. It is important that patients who use metformin are instructed to consult their heal…

Transient cortical blindness following peripheral vascular trauma: A case report.

Cortical blindness is characterized by unilateral or bilateral vision loss despite an intact pupillary reflex, full extraocular movements, and normal fundoscopic examination. Common causes include stroke, cardiac emboli, head trauma or rarely, a hypoxic-ischemic event which results to decreased perfusion to the occipital lobes supplied by the posterior cerebral artery. Imaging with computed tomography is usually diagnostic documenting stroke or embolization as well as ensuring an intact cerebral circulation. Prognosis largely depends on the etiology as most reports document an irreversible con…

Gliomatosis cerebri with blindness: A case report with literature review.

Cerebral gliomatosis (GC) is a rare diffuse infiltrative growth pattern of glioma with nonspecific clinical manifestations like visual impairment that may involve bilateral temporal lobes. Herpes simplex encephalitis (HSE) and limbic encephalitis (LE) can also lead to temporal lobe involvement. Differentiating these entities is necessary for patients with misleading presentations and imaging findings. To the best of our knowledge, this is the third case of GC presenting with blindness. The patient was a 35 years-old male in a drug rehabilitation center for heroin addiction. He presented with a…

Gata2a Mutation Causes Progressive Microphthalmia and Blindness in Nile Tilapia.

The normal development of lens fiber cells plays a critical role in lens morphogenesis and maintaining transparency. Factors involved in the … ion and apoptosis of primary lens fiber cells. The mutants exhibited progressive microphthalmia and blindness in adulthood. Transcriptome analysis of the eyes showed that the expression levels of almost all genes encoding crystallin were significantly down-regulated, while the expression levels of genes involved in visual perception and metal ion binding we …

[A case of blindness caused by mucocele of frontal sinus].

A case of blindness caused by frontal sinus mucocele was reported. The patient presented with pain and swelling in his left eye and decreased visual acuity with only light sensation for 3 days. Orbital imaging examination suggested a mass of communication in the left frontal sinus and the upper quadrant of left orbit, which was considered as a mucinous cyst involving the left superior orbital wall, surrounding the left lacrimal gland, pushing down the left superior rectus muscle, squeezing the left eyeball and protruding forward and downward. The patient was diagnosed with a left frontal sinus…

80 Years of vision: preventing blindness from retinopathy of prematurity.

Retinopathy of prematurity (ROP) is one of the leading yet preventable causes of childhood blindness worldwide. The purpose of this review is to provide a practical template for observational and treatment methods in order to reduce the overall incidence of any ROP and to improve both short-term and long-term outcomes once Type 1 ROP (treatable ROP) develops.

Subacute Blindness Revealing an Autoimmune Glial Fibrillary Acidic Protein Astrocytopathy.

Autoimmune glial fibrillary acidic protein (GFAP) astrocytopathy is an autoimmune disease of the central nervous system characterized by positive GFAP autoantibody. The most common are encephalitis, meningoencephalitis or meningoencephalomyelitis. Antibodies in cerebrospinal fluid (CSF) against GFAP are biomarkers and expressed in most cases with autoimmune GFAP astrocytopathy. Diagnosis by biopsy is not common practice and has been rarely performed in the literature. This is the particularity of our reported case of autoimmune GFAP astrocytopathy presented as opticopyramidal syndrome, all par

A woman in her seventies with acute onset of blindness.

BACKGROUND: Dural arteriovenous fistulae are among the most common causes of pulsatile tinnitus. Selective angiography can be necessary for a definitive diagnosis, but in rare cases has been reported to cause sudden cortical blindness. CASE PRESENTATION: We present a woman in her seventies for whom cerebral angiography revealed a dural arteriovenous fistula. Two hours after the angiography she experienced sudden bilateral blindness. A local cause of sudden visual loss was excluded by clinical examination, cerebral bleeding was excluded by CT scan, vascular spasms and occlusions were excluded …

Transient cortical blindness in fat embolism syndrome---a diagnostic enigma.

Fat embolism syndrome (FES) is a serious life-threatening manifestation of the fat embolism phenomenon characterized by Bergman's triad of dyspnea, petechiae and mental confusion. While fat embolization into systemic circulation is common, FES occurs in a meagre 0.05%-3% of patients having isolated long bone fractures. Though visual symptoms are commonly attributed to fat embolism retinopathy and is a later occurrence, it may not always be the case. Cortical blindness has been seldom reported in association with FES, and less so as a presenting complaint. Furthermore, no previous literature ha…
