Is Aortic Z-score an Appropriate Index of Beneficial Drug Effect in Clinical Trials in Aortic Aneurysm Disease?

Aortic Z-score (Z-score) is utilized in clinical trials to monitor the effect of medications on aortic dilation rate in Marfan (MFS) patients. Z-scores are reported in relation to body surface area and therefore are a function of height and weight. However, an information void exists regarding natural, non-pharmacological changes in Z-scores as children age. We had concerns that Z-score decrease attributed to "therapeutic" effects of investigational drugs for Marfan disease connective tissue diseases might simply reflect normal changes ("filling out" of body contour) as children age. This inve…

Simple formulas to determine optimal subclavian central venous catheter tip placement in infants and children.

BACKGROUND/PURPOSE: Optimal central venous catheter (CVC) tip location is necessary to decrease the incidence of complications related to th … ionship between that length and patient characteristics, including: age, gender, height, weight and body surface area (BSA). RESULTS: Two derived formulas using the BSA best correlated with the optimal subclavian CVC length. For the left subclavian vein approach, the optimal catheter length was 6.5 BSA+7 cm, and for the right subclavian vein approach it was 5 BSA+ …

Reflectance confocal microscopy for quantification of Sarcoptes scabiei in Norwegian scabies.

BACKGROUND AND OBJECTIVES: In vivo reflectance-mode confocal microscopy (RCM) can be used for the diagnosis of scabies. This study quantifies S. scabiei and its eggs and droppings in a patient affected by Norwegian Scabies (NS), and describes their distribution within the epidermis and in different body areas. METHODS: Different skin sites were randomly chosen in four sections (head, upper limbs, trunk and inferior limbs) of the body surface area (BSA) to acquire a total of 60 RCM z-stacks. The number of mites and eggs, the presence of droppings, as well as the minimum epidermal depth at whic…

Flexural and extensoral eruptions in dermatologic disease.

Dermatologic eruptions can be generalized or localized to specific areas of the body. Eruptions in specific body regions may suggest specific diagnosis. Recognizing such clinical patterns can facilitate the identification of the underlying pathology. In this contribution, we shall discuss those dermatologic lesions that tend to affect the flexure and extensor surfaces.

Analysis of treatment pathways and long-term prevalence of pruritus after burn - An observational study with real-world data.

INTRODUCTION: Burns heal with long-lasting scars and complications, such as persistent pruritus. To date, there is no consensus on optimal treatment. This study aims to identify prevalence and treatment patterns of burn-related pruritus using a large dataset. METHODS: A retrospective cohort study was conducted using the real-world database TriNetX. Burn patients were identified and stratified into three cohorts by extent of total body surface area (TBSA) burned: Cohort 1 (<20 %; mild), Cohort 2 (20-39 %; moderate), and Cohort 3 (≥40 %; severe). Prevalence of pruritus, eosinophilia status, and…

A comparison study of methods for estimation of a burn surface area: Lund and Browder, e-burn and Mersey Burns.

INTRODUCTION: One of the greatest challenges in burn care is the estimation of a total burn surface area (TBSA). It is especially challenging and needs to take into account the growing proportions and the age of a paediatric patient. The aims of this study is to: (1) assess the reliability of the three modalities (LB, MB, and EB) in calculating the extent of burn injuries and fluid resuscitation, and (2) compare the features in terms of usability and efficacy. METHODS: Participants were recruited from Women's and Children's Hospital (WCH), South Australia's surgical and emergency department.
