GLP-1 and Dual GIP/GLP-1 Receptor Agonists in Pediatric Obesity: From Neuroendocrine Mechanisms to Clinical Application.

Pediatric obesity is a chronic, multifactorial neuroendocrine disease in which disrupted gut-brain signaling, hypothalamic appetite regulation, adipose tissue dysfunction, and altered inter-organ communication sustain positive energy balance. Incretin-based therapies translate this biology into mechanism-based treatment, yet their developmental implications remain incompletely defined. This narrative review integrates evidence on GLP-1 and GIP receptor signaling with pediatric trial data and clinical implementation, with particular attention to developmental modifiers of gut-brain signaling an

Swallowable Gastric Balloon: As a Noninvasive Option for Weight Loss.

Background: Obesity is a multifactorial disease that has had an increase in its prevalence. Its treatment requires a multidisciplinary team to prepare the patient for bariatric surgery. But not all patients are willing to undergo a surgical procedure. Other options to surgery were developed, such as the swallowable gastric balloon, a novel device that allows weight loss (WL) due to a restrictive function without anesthesia, endoscopy, or surgery and is completely ambulatory. Methods: A descriptive, retrospective, and observational study was presented. A total of 497 obese patients of both sexe

Autophagy in the corpus luteum correlates with tissue growth in pregnant rats.

The developmental activation of the corpus luteum (CL) structurally and functionally is critical for the temporally regulated establishment, maintenance, and termination of pregnancy in rats. In this study, we have investigated the possible involvement of autophagy in the regulation of the CL during pregnancy in rats. The expression ratio of microtubule-associated protein light chain 3 (LC3)-II/-I, a widely used indicator of autophagic activity, in the CL remained relatively stable until day 15 of pregnancy. Subsequently, it progressively increased until day 21, and then declined until day 3 p

The contributions of weight problem perception, BMI, gender, mood, and smoking status to binge eating among college students.

College student participants (N=1063; 77.8% response rate) completed a web-based survey assessing demographics, depression, anxiety, body image, cigarette smoking, and weight history. Among overweight participants, 42.6% of those who believed they were overweight admitted to binge eating, while only 30.1% who did not feel overweight did so (p<.05). Among non-overweight participants, 43.2% of those who believed they were overweight admitted to binge eating, while only 32.9% of those who did not feel overweight did so (p<.05). Weight Problem Perception (WPP) mediated the contribution of BMI on b

Weight gain after adjuvant chemotherapy in patients with early breast cancer in Istanbul Turkey.

Weight gain is a well-known and unwanted complication of adjuvant chemotherapy in breast cancer. We observed that the female Turkish cancer patients frequently gain weight with adjuvant treatment of breast cancer and planned to examine the magnitude of this problem in early breast cancer patients treated at our hospital. A total of 176 early breast cancer patients who received their adjuvant systemic therapy in Marmara University Hospital between 2003 and 2007 are included in the study. We recorded their weight before and after chemotherapy and also a year after chemotherapy to find out whethe

Very low energy ketogenic diet vs. Mediterranean diet for MASLD: Superior steatosis reduction in a randomised pilot study.

BACKGROUND & AIMS: Weight loss is the cornerstone of treatment for metabolic dysfunction-associated steatotic liver disease (MASLD). This pilot study compared the efficacy and safety of a ketogenic very low energy diet (VLED) vs. a Mediterranean diet (MD) in improving hepatic steatosis and liver histology in individuals with overweight or obesity and MASLD. METHODS: We conducted a pilot randomised controlled trial in adults with histologically confirmed MASLD and a BMI of 27-35 kg/m2. Participants were assigned to either a 12-week VLED (3,151 kJ/day) or MD program (8,950 kJ/day) and monitored

Nutritional and lifestyle supportive care recommendations for management of obesity with GLP-1 - based therapies: An expert consensus statement using a modified Delphi approach.

BACKGROUND: Liraglutide, semaglutide and tirzepatide have transformed the management of obesity. However, dose-related gastrointestinal effects, obesity-associated nutritional insufficiencies, and poor long-term adherence may limit their long-term health benefits. Despite a recent joint advisory summarizing nutritional and lifestyle supportive care priorities with these therapies, there is still a significant lack of direct evidence to guide clinical practice, making consensus-based recommendations necessary. METHODS: The consensus statement development was based on an initial scoping review

Bioimpedance Spectroscopy-Guided Ultrafiltration Normalizes Hydration and Reduces Intradialytic Adverse Events in Hemodialysis Patients.

Chronic fluid overload as well as excessive fluid removal are associated with increased morbidity and mortality in hemodialysis (HD) patients. The clinical method to probe the dry weight is often inaccurate and the bioimpedance spectroscopy (BIS) is shown to improve the accuracy. We compared the impact of BIS and clinical methods to guide ultrafiltration (UF) in a randomized controlled study on the intradialytic complications and blood pressure control in prevalent HD patients. Fifty patients on maintenance HD were randomized to BIS method (BIS-group) and clinical method (CL-group) to guide UF

Study protocol, rationale and recruitment in a European multi-centre randomized controlled trial to determine the efficacy and safety of azithromycin maintenance therapy for 6 months in primary ciliary dyskinesia.

BACKGROUND: Clinical management of primary ciliary dyskinesia (PCD) respiratory disease is currently based on improving mucociliary clearance and controlling respiratory infections, through the administration of antibiotics. Treatment practices in PCD are largely extrapolated from more common chronic respiratory disorders, particularly cystic fibrosis, but no randomized controlled trials (RCT) have ever evaluated efficacy and safety of any pharmacotherapeutics used in the treatment of PCD. Maintenance therapy, with the macrolide antibiotic azithromycin, is currently widely used in chronic resp

The methyl-CpG-binding protein 2 (Mecp2) regulates the hypothalamic mitochondrial function and white adipose tissue lipid metabolism.

OBJECTIVE: The neuroepigenetic factor Mecp2 regulates gene expression and is thought to play a crucial role in energy homeostasis. Body weight is regulated at the hypothalamic level, where mitochondrial energy metabolism is necessary for its proper functioning, allowing the hypothalamus to respond to peripheral signals to maintain energy balance and modulate energy expenditure through the sympathetic nervous system. Since the mechanism by which genetic and environmental factors contribute to regulating energy balance is unclear, this study aims to understand the contribution of gene-environmen

Optimizing phenobarbital dosing in critically ill patients with refractory and superrefractory status epilepticus using a population pharmacokinetic model.

OBJECTIVE: Current weight-based dosing fails to account for pharmacokinetic variability in refractory and superrefractory status epilepticus (RSE, SRSE). However, understanding pharmacokinetics in critically ill patients with varying degrees of organ dysfunction can improve both safety and efficacy. Hence, this study aims to quantify key pharmacokinetic variabilities to enable individualized dosing in RSE and SRSE. METHODS: Patients with RSE and SRSE admitted to a neurointensive care unit of a tertiary academic center were retrospectively screened for therapeutic drug monitoring (TDM) samples

Gut microbiota alteration in a mouse model of Anorexia Nervosa.

BACKGROUND & AIMS: Anorexia Nervosa is a severe disease depending on both biological, psychological and environmental factors. The gut microbiota has recently been proposed as one of the biological factors potentially involved in the onset or maintenance of Anorexia Nervosa. To unravel the potential role of the gut microbiota in this disease, we characterized the dysbiosis occurring in a mouse model of Anorexia and correlated bacteria level changes with different physiological parameters such as body weight, food intake or levels of hypothalamic neuropeptides. METHODS: We used the Activity-Ba

The natural history of the obese strain of chickens--an animal model for spontaneous autoimmune thyroiditis.

Chickens of the Obese strain (OS) are hereditarily affected with spontaneous autoimmune thyroiditis that resembles Hashimoto's thyroiditis of humans in clinical, histopathological, serological, and endocrinological aspects. In this review, the natural history of the OS, reflecting the development and maintenance of the stock and its improvement of productivity over many years at Cornell University, is summarized. To underline the value and usefulness of this animal model, the concept of the multifactorial pathogenesis of autoimmune diseases, which was mainly established at the University of In

Low-Dose Tirzepatide for Obesity: Comparative Efficacy of 2.5 mg Versus 5 mg in Non-Diabetic Japanese Adults.

BACKGROUND: Obesity is a major global health issue. While tirzepatide demonstrates robust weight reduction in clinical trials, data comparing low maintenance doses (2.5 vs. 5 mg) in non-diabetic adults with obesity remain limited. This study evaluated the comparative efficacy and safety of 2.5 versus 5 mg tirzepatide combined with lifestyle intervention. METHODS: This multicentre, prospective, non-randomised cohort study enrolled non-diabetic Japanese adults (18-65 years) with BMI ≥ 30 kg/m2 or ≥ 27 kg/m2 with comorbidities. Patients initiated tirzepatide at 2.5 mg weekly; after 4 weeks, the

Risk factors for weight gain during methadone maintenance treatment.

BACKGROUND: Weight gain was reported during methadone maintenance treatment (MMT). However, its relation to eating habits and specific risk factors, including methadone dose or serum level, was limited. The aims of this study were to characterize risk factors for weight gain and to study current eating habits, food preferences, and nutrition knowledge. METHODS: Patients with available measures of weight and height (body mass index [BMI]) at admission to MMT and at follow-up, when methadone serum levels were determined (after 1 year or when stabilized) (N = 114), were studied (using the Addict

Effect of dietary L-arginine supplementation on the expression of vascular endothelial growth factor A, its receptors, and nitric oxide system components in the pancreas of streptozotocin-induced diabetic rats.

T1DM is characterized by elevated blood glucose levels, insulin insufficiency, and alterations in vessel formation. Therefore, the expression of insulin, active caspase-3, inflammatory factors such as TNF-α and NF-κB, and components of the NO and VEGFA systems were investigated in the pancreas of STZ-induced diabetic rats. Blood glucose levels and body weight were also measured. Additionally, the effects of L-arginine supplementation on the same parameters were evaluated. The body weight of the STZ and STZ + L-arginine groups was lower than that of the control and L-arginine groups. L-arginine

Bifidobacterium longum subsp. longum dipro-O: a potential therapeutic agent for ameliorating metabolic disorders in high-fat diet-induced obesity mouse model.

BACKGROUND: Excessive fat intake results in lipid metabolic disorders accompanied by inflammation and other complications. However, the effectiveness of drug interventions for metabolic disorders is not ideal, owing to their inherent limitations. Here, we introduce the probiotic Bifidobacterium longum subsp. longum dipro-O, which ameliorates metabolic disorders without any side effects. METHOD: C57BL/6J mice were fed a 60% kcal high-fat diet (HFD) for eight weeks to induce obesity, and then dipro-O intervention was administered for nine weeks.
