Hypoxia in the glioblastoma microenvironment: shaping the phenotype of cancer stem-like cells.

Glioblastoma is the most common and aggressive malignant primary brain tumor. Cellular heterogeneity is a characteristic feature of the disease and contributes to the difficulty in formulating effective therapies. Glioma stem-like cells (GSCs) have been identified as a subpopulation of tumor cells that are thought to be largely responsible for resistance to treatment. Intratumoral hypoxia contributes to maintenance of the GSCs by supporting the critical stem cell traits of multipotency, self-renewal, and tumorigenicity. This review highlights the interaction of GSCs with the hypoxic tumor micr

Diffuse pontine lesions in children with neurofibromatosis type 1: making a case for unidentified bright objects.

Using an illustrative case of a presumed pontine unidentified bright object (UBO) with spontaneous lesion regression over 2 years, we review the importance of including UBOs in the differential diagnosis of children with confirmed or possible neurofibromatosis type 1 (NF1) who present with diffuse pontine enlargement and T2-weighted changes on MRI. Asymptomatic children with presumed NF1 and diffuse pontine lesions should not be treated with radiation and should not be biopsied. Prior reports of good prognosis associated with pontine glioma in patients with NF1 may have been unrecognized UBOs

Brainstem anaplastic glioma in patients with AIDS: a case report and review of the literature.

AIDS is an increasingly common diagnosis seen by neurologists and neurosurgeons alike. Although the more common brain lesions associated with AIDS are due to central nervous system lymphomas, toxoplasma encephalitis or progressive multifocal leukoencephalopathy, relatively recent clinical evidence has shown that AIDS-independent cerebral tumours can arise as well, albeit less commonly. Previous incidents have been reported with HIV and AIDS patients presenting with cerebral astrocytomas. To our knowledge, there has never been a report in the literature of a brainstem anaplastic glioma occurrin

A giant retroclival Rathke's cleft cyst.

Rathke's cleft cyst (RCC) is a rare sellar tumor that can extend to the retroclival region, resulting in brain stem compression. We report a male with short stature who experienced repeated syncope due to compression of the brain stem by a giant retroclival RCC. MRI revealed a large homogeneous sellar tumor with anterior displacement of the pituitary gland that extended to the left retroclival space. The tumor differed from ordinary RCCs because of its unusual clinical presentation, the direction of extension, and its imaging characteristics. The retromastoid surgical approach was used for cys

[A case of cavernous angioma in the lower pons, showing subacute onset of unilateral cranial nerve palsy and segmental sensory disturbance].

We report a 54-year-old man with right abducent nerve palsy, right facial nerve palsy, and left segmental sensory disturbance, which progressed for 2 weeks. He was found to have cavernous angioma in the lower pons. When he visited our hospital, he had right facial palsy, sensory disturbance of left half of the face and left upper limb, and diplopia. He had suffered right abducent nerve palsy 5 years previously and had recently developed hypertension. Neurological examination further revealed right abducent nerve palsy, right peripheral facial nerve palsy, sensory impairment of the left half of

Pontine mass presenting as anxiety, obsessions, and severe eating problems: a case report.

Psychiatric symptoms are rarely reported as presenting symptoms in brainstem gliomas in children, with anxiety being the most common one. An 8-year-old girl patient had loss of appetite, weight loss, and difficulty in swallowing severe enough to warrant parenteral nutrition and hospitalization. Psychiatric examination revealed ego-dystonic obsessions related with choking and compulsory religious rituals. Symptoms partially responded to psychotropic treatment. However, because of unremitting hiccups and left-sided weakness, brain imaging was conducted, and magnetic resonance imaging revealed a

Application of stem cell transplant for brain tumors.

Brain tumors are the second most common malignancy in children and the most common solid tumor. The majority of children are treated with surgery alone or in combination with radiation and/or chemotherapy. Recently investigators have used high dose chemotherapy with autologous stem cell rescue (ASCR) in patients with malignant brain tumors. This approach has been most successful in chemosensitive tumors including medulloblastoma, supratentorial primitive neuroectodermal tumors (SPNET) and central nervous system germ cell tumors (CNS GCT). In addition, the use of high dose chemotherapy has enab

Multiple modes of PRC2 inhibition elicit global chromatin alterations in H3K27M pediatric glioma.

A methionine substitution at lysine-27 on histone H3 variants (H3K27M) characterizes ~80% of diffuse intrinsic pontine gliomas (DIPG) and inhibits polycomb repressive complex 2 (PRC2) in a dominant-negative fashion. Yet, the mechanisms for this inhibition and abnormal epigenomic landscape have not been resolved. Using quantitative proteomics, we discovered that robust PRC2 inhibition requires levels of H3K27M greatly exceeding those of PRC2, seen in DIPG. While PRC2 inhibition requires interaction with H3K27M, we found that this interaction on chromatin is transient, with PRC2 largely being re

Pathology of midline brain tumors. Immunocytochemical tumor markers and classificatory aspects.

Our understanding of the pathology of human brain tumors has considerably increased in recent years thanks to investigation of nervous tissue markers which can be demonstrated by immunocytochemical techniques in sections of neurosurgical tissue specimens. This development is presented here in a review of the most important entities among midline brain tumors. Detection of neural differentiation markers such as glial fibrillary acidic protein (GFAP), S 100 protein (S 100 p), neurofibrillary proteins and neuron-specific enolase (NSE) greatly contributed to clarifying the histogenesis and differe

Microsurgical Resection of Brainstem Cavernous Malformations in Older Adults: A Multicenter, 30-Year Experience.

BACKGROUND AND OBJECTIVES: Microsurgical resection is the only curative intervention for symptomatic brainstem cavernous malformations (BSCMs), but the management of these lesions in older adults (≥65 years) is not well described. This study sought to address this gap by examining the safety and efficacy of BSCM resection in a cohort of older adults. METHODS: Records of patients who underwent BSCM resection over a 30-year period were reviewed retrospectively. Baseline characteristics and outcomes were compared between older (≥65 years) and younger (<65 years) patients. RESULTS: Of 550 patien

Long-term analysis of ABI auditory performance in patients with neurofibromatosis type 2-related schwannomatosis.

PURPOSE: This retrospective monocentric study aimed to evaluate long-term auditory brainstem implant (ABI) function in patients with neurofibromatosis type 2, and to investigate the prognostic factors for ABI use. METHODS: Between 1997 and 2022, 27 patients with at least five years of follow-up underwent implantation with 32 ABIs. At 1- and 5-years post-implantation and at last follow-up, ABIs were classified as used or non-used and the size of the ipsilateral tumor was recorded. For patients who used their ABIs, we assessed speech perception (disyllabic words, MBAA sentences) in quiet condit

The Brainstem Cavernoma Case Series: A Formula for Surgery and Surgical Technique.

Background and Objectives: Cavernous malformations (CM) are vascular malformations with low blood flow. The removal of brainstem CMs (BS) is associated with high surgical morbidity, and there is no general consensus on when to treat deep-seated BS CMs. The aim of this study is to compare the surgical outcomes of a series of deep-seated BS CMs with the surgical outcomes of a series of superficially located BS CMs operated on at the Department of Neurosurgery, College of Tuebingen, Germany. Materials and Methods: A retrospective evaluation was performed using patient charts, surgical video recor

Delta Histogram Analysis of Synthetic MRI for Evaluating Grade and Cellular Proliferation of Meningioma.

Purpose To investigate the potential of delta histogram analysis of synthetic MRI (SyMRI) for predicting meningioma grade and cellular proliferation. Materials and Methods In this prospective study from February 2022 to October 2023, consecutive participants scheduled for conventional MRI and SyMRI were enrolled. Histogram parameters (HPs) were obtained with whole-tumor volume analysis. Delta histogram parameters, including absolute (dα = postcontrast HP - precontrast HP) and relative delta HPs (dα% = [postcontrast HP - precontrast HP]/precontrast HP), where α = T1, T2, or proton density (PD),

[Orbicularis oculi reflex in computerized tomography verified lesions of the posterior cranial fossa].

The blink reflex elicited by electrical stimulation was studied in 60 patients with lesions of the brainstem and cerebellum (ischemic lesions, non-traumatic bleedings or tumors), which were diagnosed by computertomography. The relationship between localized morphological changes in computertomography and changes in blink reflex has been studied. 46 patients (77%) had a pathologic blink reflex. Usually the blink reflex was normal in patients with a tumor of cerebellum. In bleedings or tumors of the brainstem a good relationship existed between localization and extension of the lesion and the pa

Surprising complete response of intramedullary spinal cord metastasis from breast cancer: a case report and literature review.

PURPOSE: Intramedullary spinal cord metastases (ISCM) are considered rare but their incidence is rising. Most ISCM cases are recognized to occur in patients with stage IV lung cancer or breast cancer (BC). METHODS: We report a rare case of cervical BC-ISCM in 60-year-old woman, documented by magnetic resonance imaging and positron emission tomography-computed tomography and treated with volumetric modulated arc therapy with daily image-guided radiation therapy (VMAT/IGRT). RESULTS: An unexpected clinical and radiologic complete response in absence of neurologic side effects was recorded 4 mo

Isolated transient blindness and isolated oculomotor nerve palsy secondary to single mesencephalic metastases: report of 2 cases.

PURPOSE: Metastatic lesions of midbrain are quite rare, and only unusually they produce isolated cranial nerve palsy. METHODS: We report two cases of 2nd and 3rd cranial nerves impairment, produced by solitary midbrain metastases. RESULTS: In both patients Magnetic Resonance (MR) allowed a correct diagnosis, subsequently confirmed by histology. CONCLUSIONS: The metastatic involvement of midbrain would be included in the differential diagnosis of neuro-ophthalmic conditions to avoid the risk of misdiagnosis and consequently delay in management of this special population. MR is crucial for ea

Malignant peripheral nerve sheath tumor of the vestibulocochlear nerve and brainstem: multimodality treatment with survival of 27 months. A case report and review of the literature.

BACKGROUND AND IMPORTANCE: Malignant peripheral nerve sheath tumors are the most common malignant mesenchymal tumors of soft tissues, but they are very rare when found to arise from a cranial nerve and when not in association with neurofibromatosis. These tumors are highly malignant and carry a poor prognosis with survival usually less than 6 months. CLINICAL PRESENTATION: The authors report the case of a 23-year-old female with no history of phakomatoses, previous irradiation, or known genetic disorders, who presented with a malignant peripheral nerve sheath tumor of the vestibulocochlear ne
