Standard laparoscopic trocars for the treatment of tension pneumothorax: a superior alternative to needle decompression.

BACKGROUND: Needle thoracostomy (NT) is a commonly taught intervention for tension pneumothorax (tPTX) but has a high failure rate. We hypothesize that standard 5-mm laparoscopic trocars may be a safe and more effective alternative. METHODS: Thirty episodes of tPTX and 27 episodes of tension-induced pulseless electrical activity (PEA) were induced in five adult swine using thoracic CO2 insufflation via balloon trocar. Tension was defined as a 50% decrease in cardiac output. Chest decompression was performed with 5-mm laparoscopic trocars for the treatment of both tPTX with hemodynamic comprom

Effects of imposed acid-base derangement on the cardiovascular effects and pharmacokinetics of bupivacaine and thiopental.

BACKGROUND: By changing physicochemical properties such as effective lipophilicity, changes in blood pH could alter the distribution, elimination, and effects of weakly ionizing drugs. The authors examined the outcome of imposed acid-base derangement on cardiovascular effects and myocardial and whole body pharmacokinetics of bupivacaine, a weak base, and thiopental, a weak acid. METHODS: Intravenous infusions of rac-bupivacaine HCl (37.5 mg) or rac-thiopental sodium (250 mg, subanesthetic dose) were administered over 3 min to previously instrumented conscious ewes with normal blood pH, acidem

Cardiovascular and pharmacodynamic effects of high-dose fentanyl in newborn piglets.

To understand better the hemodynamic effects of fentanyl anesthesia on the developing newborn, the authors studied the changes in cardiac output and its four determinants (preload, afterload, heart rate, and contractility) and plasma fentanyl kinetics in newborn piglets following the administration of high-dose fentanyl with or without atropine premedication. Twenty-five healthy farm piglets were divided into four groups. Hemodynamic studies were conducted on five who received 50 micrograms/kg intravenous fentanyl, five controls who received only 0.01-0.03 mg/kg intravenous atropine, and nine

High frequency percussive ventilation in burn patients: hemodynamics and gas exchange.

High frequency percussive ventilation (HFPV) is a recent ventilatory mode, which combines conventional cycles with high frequency percussions. HFPV was initially instituted as salvage therapy after acute respiratory failure following smoke inhalation injury achieving in each case a dramatic improvement of blood oxygenation, PaCO(2) and ventilatory pressures. This study investigates the influence of HFPV on hesmodynamics, blood oxygenation and ventilatory parameters in eight stable ICU burn patients requiring artificial ventilatory support during a postoperative period following traumatic injur

The mineralocorticoid receptor antagonist finerenone enhances cardiovascular recovery upon food intake normalization in obese mice.

BACKGROUND: Patients with obesity exhibit high prevalence of heart failure with preserved or mildly reduced ejection fraction (HFpEF or HFmrEF). Recently, the mineralocorticoid receptor antagonist finerenone demonstrated benefits in this population. Glucagon-like peptide-1 analog aided weight loss also showed benefits. We hypothesized that finerenone would improve cardiovascular parameters when combined with weight loss in previously obese mice. METHODS: Mice were fed a high-fat diet (HFD) for 24 weeks, and were compared to mice fed a HFD for 16 weeks before returning to a standard diet for a

[Clinical pathway for cardiogenic shock. A proposal from ANMCO Tuscany].

Cardiogenic shock is a clinical syndrome characterized by hypotension and hypoperfusion due to the inability of the heart to provide adequate cardiac output. It is an infrequent clinical condition still burdened by high mortality rates. In patients with cardiogenic shock rapid diagnosis, multiparameter monitoring and timely therapeutic strategies with pharmacological agents or mechanical circulatory support are necessary to provide adequate peripheral tissue perfusion and to improve outcome. Recent investigations reported lower mortality rates to be associated with clinical pathways based on a

Operations for improving left ventricular diastolic function.

PURPOSE OF REVIEW: With a combination of fetal catheter interventions and postnatal catheter and surgical interventions, the left ventricle in patients with hypoplastic left heart can undergo significant increase in size and remodeling to support cardiac output. However, some will have persistent high left atrial pressures resulting in pulmonary edema and high pulmonary artery pressures, that is, diastolic dysfunction. RECENT FINDINGS: The manuscript details the medical and surgical strategies to diagnose and treat diastolic dysfunction in this patient population to promote biventricular circ

Myocardial enzyme release from ischemic isolated perfused working rat heart.

Isolated working rat hearts were made ischemic for 5, 10, and 30 minutes respectively. After the ischemic period, all hearts were perfused in a retrograde nonworking way for 30 minutes. During the 5 first minutes of ischemia, there was a marked fall of cardiac output and coronary flow. A significant release of GOT was seen and this was more marked after longer periods of ischemia. Addition of adrenaline to the perfusate increased the enzyme release. Pacing at 400/minute, high preload, high afterload, acidosis, or alkalosis did not alter enzyme release. Glycogen, ATP and CrP levels were depress

Myocardial Na+/H+ exchanger-1 (NHE1) content is decreased by exercise training.

The effect of exercise training on myocardial Na(+)/H(+) exchanger-1 (NHE1) protein expression was examined. Adult female Sprague-Dawley rats were randomly divided into sedentary (S; n = 8) and exercised (E; n = 9) groups. Twenty-four hours after the last exercise bout, hearts were weighed and connected to an isolated perfused working heart apparatus for evaluation of cardiac functional performance. Heart weight and heart weight/body weight from E rats was significantly increased by 7.1 and 7.2 % (P < 0.05), respectively, compared with S hearts. The E hearts displayed 15 % greater cardiac outp

MRI findings in multifetal pregnancies complicated by twin reversed arterial perfusion sequence (TRAP).

BACKGROUND: Twin reversed arterial perfusion sequence (TRAP) is a rare complication in multifetal monochorionic pregnancies in which a normal "pump" twin provides circulation to an abnormal acardiac co-twin, resulting in high-output cardiac dysfunction in the pump twin. OBJECTIVE: To define fetal MRI findings of TRAP sequence. MATERIALS AND METHODS: Fetal MR images were retrospectively reviewed in 35 pregnancies complicated by TRAP sequence. Abnormalities of the pump twin, acardiac twin, umbilical cord, placenta and amniotic fluid were reviewed. RESULTS: Acardiac twins were classified as: a

Cardiovascular disease in Sickle cell: mechanisms, diagnostics and therapeutic advances.

Cardiovascular complications are increasingly recognised as a major driver of morbidity and early mortality in patients with sickle cell disease (SCD), yet they remain underdiagnosed and underappreciated. This contemporary review synthesises current knowledge across a spectrum of cardiovascular manifestations-including myocardial dysfunction, pulmonary hypertension, cardiac iron overload, arrhythmias, myocardial infarction, stroke and sudden death-with emphasis on their unique pathophysiological mechanisms in SCD. We highlight emerging diagnostic tools such as cardiac magnetic resonance with T

Case report of percutaneous intervention for extensive thrombosis after the Fontan procedure in a 14-year-old.

BACKGROUND: Thromboembolic complications remain a significant cause of morbidity following the Fontan procedure, driven by the unique haemodynamics of the Fontan circulation. CASE SUMMARY: We present a case of a 14-year-old patient with single-ventricle physiology who underwent an extracardiac Fontan procedure. Post-operatively, he developed low cardiac output requiring extracorporeal membrane oxygenation (ECMO) support. Despite successful weaning from ECMO, hepatomegaly, pleural effusion, and ascites persisted. Further investigation revealed a significant thrombus within the Fontan conduit.

Successful Transcatheter Atrial Septal Defect Closure Prior to Coronary Artery Bypass Grafting Using Anti-Congestive Therapies and Intraaortic Balloon Pumping in a Patient with Severe Ischemic Cardiomyopathy and Triple-Vessel Coronary Artery Disease.

In patients with an atrial septal defect (ASD) and left ventricular (LV) dysfunction associated with coronary artery disease (CAD), to avoid the development of acute left heart failure (HF) and an increase in myocardial oxygen consumption following ASD closure, it is conceivable that coronary artery revascularization should be performed prior to ASD closure. We report the case of a 67-year-old man with a large secundum ASD and LV ejection fraction of 15.6% resulting from severe ischemic cardiomyopathy and triple-vessel CAD, both of which contributed to biventricular HF characterized by high le

Cardiac performance and morphology in end-stage renal disease.

Patients with end-stage renal disease (ESRD) experience a variety of hemodynamic and metabolic abnormalities that predispose to alterations in cardiac performance and morphology. High cardiac output related to renal anemia, hypertension, volume overload, and the arteriovenous fistula (in patients on hemodialysis) predispose to eccentric left ventricular (LV) hypertrophy. Hypertension, aortic stiffness, and aortic stenosis predispose to concentric LV hypertrophy. Most ESRD patients have a hybrid form of LV hypertrophy. LV hypertrophy is commonly accompanied by LV diastolic dysfunction. LV systo

Long-term results after surgical repair of total anomalous pulmonary venous connection--hemodynamic evaluation of pulmonary venous obstruction with isoproterenol infusion.

To evaluate late pulmonary venous obstruction following surgical repair in patients with total anomalous pulmonary venous connection, which may be disclosed at high cardiac output but not at rest, a hemodynamic study was performed using isoproterenol infusion. The study included 7 patients in NYHA class I, aged from 5 to 12 years (mean 6 years), who had undergone surgical correction in their early infancy (mean 79 days). After a routine catheterization protocol, isoproterenol was infused at a rate of 0.01 microgram/kg per min. On average, cardiac index increased from 4.8 to 8.1 L/min per m2. T
