Fulminant hepatic failure has been reported in patients with chronic congestive heart failure. Two patients in whom jaundice and coma followed cardiac surgery were studied to find the possible etiology. Clinical, biochemical, and histologic evaluations revealed low cardiac output; elevated levels of bilirubin, lactic dehydrogenase, serum glutamic oxaloacetic transaminase, and alkaline phosphatase; prolonged prothrombin time; and centrizonal necrosis of hepatocytes with sinusoidal dilatation. No other possible causes of hapatic dysfunction and coma were identified. We conclude that acute postop

During the past eight years, 46 of the 106 patients who underwent mitral valve replacement were associated with tricuspid insufficiency. No surgical correction was performed (14 cases) in cases of slight tricuspid insufficiency. Tricuspid annuloplasty (11 cases) or valve replacement (21 cases) was employed according to the severity of insufficiency. In the non-repair group, the mortality rate was fairly low (21 per cent), but the postoperative status was the least satisfactory by the NYHA classification. Tricuspid insufficiency was significantly reduced only in two of these 14 cases after the

The authors report the cases of two cirrhotic patients without cardiac failure who underwent peritoneo-venous bypass (Leveen valve). On the two cases an acute pulmonary oedema (APE) serious but not fatal was noted at the 36e hour. This one was associated with a rise of cardiac output and a moderate rise of the pulmonary wedge pressure (15-18 mm Hg). The administration of thiamine did not correct these perturbations but small doses of furosemide corrected the APE. Despite the low frequency of the cases of APE after Leveen bypass and their mechanism not definitevely known, the authors recommend

Heart failure with reduced ejection fraction is a syndrome consisting of symptoms (dyspnoea, fatigue, swelling) and/or signs of congestion (pulmonary crackles, oedema). It is caused by structural and/or functional pathologies, most commonly ischaemic heart disease, entailing elevated cardiac filling pressures and can result in low cardiac output. Medical treatment has evolved during the recent decades as outlined in this review, and a 4-pillar treatment strategy is recommended including a renin-angiotensin-aldosterone system blocker or sacubitril/valsartan, a betablocker, a mineralocorticoid a

Aortic stenosis is a congenital or acquired reduction in the area of the aortic valve, resulting in obstruction of the blood flow from the left ventricle to the aorta. Aortic stenosis accounts for 2-5% of all congenital heart defects and is a potentially life-threatening disorder. In adults aortic stenosis represents 34% of all valvular heart diseases. Degenerative etiology is present in 80% of cases. Patients with mild aortic stenosis are usually asymptomatic. Symptoms of the disease occur along with the disappearance of effective compensatory mechanisms. These are symptoms of low cardiac out

This noninvasive echocardiographic study of cardiac performance in 27 patients with chronic renal failure and vascular access documented the magnitude of forward cardiac output to be 65 per cent of the observed index. Hemodialysis resulted in acute hemodynamic changes, including an average fall in the cardiac index of 57 per cent, presumably because of a 40 per cent decrease in left ventricular filling volume. In patients in whom the vascular access had functioned longest, a time related cardiac decompensation is suggested by a cardiac index of significantly less than normal, p, 0.0013, with o

Mechanical support options for children and small adolescents with acute left ventricular failure are often limited to extracorporeal life support (ECLS) and subsequent left ventricular assist device implantation. We report a case of a 3-year-old child weighing 12 kg with acute humoral rejection after cardiac transplantation not adequately responding to medical therapy who presented in persistent low cardiac output syndrome. We successfully stabilized the patient by implanting a Impella 2.5 device via a 6-mm Hemashield prosthesis on the right axillary artery. The patient was bridged to recover

Cardiogenic shock (CS) is a life-threatening condition characterized by end-organ hypoperfusion and hypoxia primarily due to cardiac dysfunction and low cardiac output. Unfortunately, the mortality and morbidity associated with CS have remained high despite notable advances in heart failure management. Treatment should be carefully guided by hemodynamics assessment. Although inotropes, vasopressors, mechanical circulatory support, and catheter intervention for critical valve lesion are not always recommended, they are helpful in selected patients. Early diagnosis, accurate hemodynamic assessme

Acute pernicious or sho-shin beri-beri is characterised by haemodynamic and biochemical features which include hypotension, high cardiac output, low peripheral vascular resistance, lactic acidosis and high mixed venous content. The disorder is a metabolic emergency and requires immediate treatment with intravenous thiamine. Three cases are described which were successfully treated with intravenous thiamine (500 mg intravenously, followed by 100 mg 8-hourly for 24 hours then 100 mg daily) without using catecholamines or sodium bicarbonate. Digoxin was used in two cases to manage cardiac dysfunc…

Between 1980 and 1992, 84 patients over 30 years were underwent operative closure of atrial septal defect (ASD). Preoperative complaints and lower cardiac function were more frequent in the older than in the younger, but the clinical symptom of both generation improved significantly after surgery. Only a 67-year-old patient died after surgery due to low cardiac output syndrome. No long-term death occurred. The incidence of atrial fibrillation was increased postoperatively compared with preoperatively. Therefore, we recommend early surgical repair for aged ASD before thirties, if possible.

A patient with rheumatic mitral stenosis and previous cerebral embolism had a myocardial infarction during cardiac catheterisation. She later developed severe mitral regurgitation one year after open valvotomy and at valve replacement was found to have a papillary tumour of the mitral valve. Unexplained low cardiac output occurred four days after operation. Postmortem examination showed thrombotic occlusion of the xenograft prosthesis, a complication not previously seen with tissue valves. Both of these rare events were suggested by the patient's clinical course and could have been diagnosed w

Right heart failure (RHF) is a major cause of mortality in patients using left ventricular assist devices (LVADs). Vericiguat, an oral soluble guanylate cyclase stimulator, has recently been used to treat chronic heart failure (HF) with a reduced ejection fraction (EF) and is expected to improve RHF owing to its pulmonary vasodilatory and inotropic properties in patients with an LVAD. A 62 year old woman with advanced HF due to cardiac sarcoidosis was transferred to our center with HF recurrence and left ventricular (LV) thrombosis. Since her status became inotropic-dependent post-thrombectomy

Congenital heart disease (CHD) is the most common birth defect, and up to 50% of infants with CHD require cardiovascular surgery early in life. Current clinical practice often involves thymus resection during cardiac surgery, detrimentally affecting T-cell immunity. However, epidemiological data indicate that CHD patients face an elevated risk for infections and immune-mediated diseases, independent of thymectomy. Hence, we examined whether the cardiac defect impacts thymus function in individuals with CHD. We investigated thymocyte development in 58 infants categorized by CHD complexity. To a

Veno-arterial extracorporeal life support (ECLS) may be indicated in patients with refractory heart failure. The list of conditions in which ECLS is successfully used is growing and includes cardiogenic shock following myocardial infarction, refractory cardiac arrest, septic shock with low cardiac output and severe intoxication. Femoral ECLS is the most common and often preferred ECLS-configuration in the emergency setting. Although femoral access is usually quick and easy to establish, it is also associated with specific adverse haemodynamic effects due to the direction of blood flow and acce

Clinical and echocardiographic haemodynamic evaluations of response to volume expansion are described in 12 preterm neonates aged < 7 days presenting without cardiac dysfunction and with a low cardiac output. They received 10% albumin solution (20 ml kg-1) for 3 h. Measurements were made before infusion, at volumes 5, 12.5 and 20 ml kg-1 and 1 h later. All infants increased significantly their cardiac output (CO) (from a median of 177 to 283 ml kg-1 min-1). The rise of CO decreased with the volume infused. The index of systemic vascular resistance (SVR = ratio of mean arterial pressure to the

Percutaneous ventricular assist devices (pVAD) decrease mortality in cardiogenic shock (CS) patients, but at a high adverse event rate. Novel pVADs need to improve cardiac power output (CPO) to improve efficacy and reduce profile to reduce adverse events. Venstramedical has developed an impeller with nitinol and polymer which can collapse for low-profile insertion and expand in the body to provide optimal CPO. We have also developed a unique 9 French (9Fr) delivery system. In vitro and in vivo testing have been performed to develop this device. Head-flow (HQ) tests generate 5.0 L/min with a pr

We studied in 20 anesthetized dogs (11-15 kg) the effect of volume loading and dopamine in the period between 90 and 120 min after injection of E Coli endotoxin (1.5 mg/kg). Changes in red-cell distribution were estimated from changes in regional radioactivity emitted by autologous 99mTc tagged red cells. Four groups of five animals were formed by randomization, such that effects of dopamine (15 micrograms/kg/min) and isotonic gelation solution (40 ml/kg/30 min) could be analyzed separately and in combination. Ninety minutes after endotoxin a significant decrease in cardiac output, arterial, a

1. Xamoterol (Corwin, Carwin, Corwil, Xamtol, ICI 118,587) is a beta-adrenoceptor partial agonist which is of benefit in the chronic treatment of heart failure (e.g. The German and Austrian Xamoterol Study Group, 1988). These results contrast with those obtained with other beta-adrenoceptor drugs, prenalterol and pirbuterol which were unsuccessful on chronic dosing (Currie et al., 1984; Glover et al., 1985). 2. Unlike prenalterol and pirbuterol, xamoterol has no significant agonist activity at the beta 2-adrenoceptor and has shown no tachyphylaxis in animals or man. 3. The overall action of xa

BACKGROUND: Cross-sectional epidemiological and clinical research suggests that lower cardiac index is associated with abnormal brain aging, including smaller brain volumes, increased white matter hyperintensities, and worse cognitive performances. Lower systemic blood flow may have implications for dementia among older adults.
METHODS AND RESULTS: A total of 1039 Framingham Offspring Cohort participants free of clinical stroke, transient ischemic attack, and dementia formed our sample (age, 69±6 years; 53% women). Multivariable-adjusted proportional hazard models adjusting for Framingham Str
