Disk fragment relocation is commonly limited to the anterior epidural space, although posterior epidural movement of a sequestrated disk piece to the posterior epidural space is infrequent. We present an uncommon case of dorsal extradural sequestration of lumbar disk herniation. A 77-year-old man presented with severe leg pain, low back pain, and urinary incontinence. Deep tendon reflexes were inattentive at the knee and ankle, and the motor power in terms of ankle dorsiflexion and great toe dorsiflexion was 2/5 in both lower limbs. There was hypoesthesia in the S1, S2, and S3 dermatomes. Magn

BACKGROUND AND PURPOSE: Cape Town is the center of an HIV-tuberculosis coepidemic. This study's aim was to highlight the importance and to describe the MR imaging features of tuberculosis in acute myelopathy and cauda equina syndrome in HIV-positive adults. To accomplish this we retrospectively reviewed the MR imaging and clinico-pathologic findings of HIV-positive patients presenting to our hospital with recent onset paraplegia and sphincter dysfunction over a 4-year period, 2008-2011.
MATERIALS & METHODS: MR imaging, CD4 count, and CSF analysis and pathology were correlated in 216 cases.

OBJECTS: We report a case of epidural cerebrospinal fluid (CSF) leak after lumbar puncture caused by CSF dissecting into the spinal epidural space. The incidence of this phenomenon may be higher than suspected, although most cases may remain asymptomatic.
MATERIALS AND METHODS: A 4-year-old girl with new-onset seizure underwent a diagnostic lumbar puncture, the results of which were normal; 3 h later, she began experiencing severe low-pressure headaches and lower back pain, bilateral lower extremity weakness, numbness, and pain, and urinary retention when upright. Spinal MRI demonstrated exte

Posterior epidural migration (PEM) of free disc fragments is rare, and reported PEM patients usually presented with radicular signs. An uncommon case involving a patient with cauda equina syndrome due to PEM of a lumbar disc fragment is reported with a review of the literature. The patient described in this report presented with an acute cauda equina syndrome resulting from disc fragment migration at the L3-L4 level that occurred after traction therapy for his lower back pain. The radiological characteristics of the disc fragment were the posterior epidural location and the ring enhancement. A…

BACKGROUND: Cauda Equina syndrome (CES) is a potentially devastating condition and is treated usually with urgent open surgical decompression of the spinal canal. Currently, the role of minimally invasive discectomy (MID) as an alternative surgical technique for CES is unclear.
OBJECTIVE: The purpose of this study was to compare clinical outcomes following MID and open laminectomy and discectomy for the treatment of CES.
METHODS: The study cohort included patients that underwent surgery due to CES at our institute. Patients' outcomes included: surgical complications, length of hospitalizatio…

BACKGROUND: Most studies recommend urgent decompression (e.g., within 48-72 h) of the symptomatic onset of a cauda equina syndrome. As patients in our area typically underwent >3 months delayed surgery for cauda equina syndromes due to disc disease/stenosis, we asked whether surgery was still worthwhile.
METHODS: This was a retrospective analysis of 12 patients (2012-2018) who underwent delayed surgical decompression for cauda equina syndromes secondary to lumbar disc herniations and/or degenerative lumbar canal stenosis.
RESULTS: After a mean postoperative duration of 8.22 months, nine pati…

A 9 month old Simmental fattening bull was presented with signs of a cauda equina syndrome. An epidurographic examination was made after thorough clinical, neurological and routine radiographic examination. The cause of the cauda equina compression--a spindle cell sarcoma--was revealed by the pathologic-histologic examination. This tumor caused the compression of the cauda equina and pathologic transverse fracture of the body of the first sacral vertebra and cranial epiphyseal fracture of the 1st and the 3rd sacral vertebra as well as the separation of the arcus from the first and second sacra…

PURPOSE: Individuals with spinal cord injury, cauda equina syndrome or spina bifida often face challenges in maintaining a healthy lifestyle. A coordinated lifestyle care pathway beginning in the rehabilitation center and continuing into primary care may support the conditions needed for sustainable lifestyle behavior. This study aimed to identify determinants to inform the development and implementation of a lifestyle care pathway.
METHODS: We conducted semi-structured interviews with 19 healthcare professionals and 24 individuals with spinal cord disorders. Separate topic guides were develo…

Ewing sarcoma (ES) is a rare primary neoplasm in the lumbar adult spine and may mimic a benign tumor. In this case, after a patient's three-month history of lower back pain and rapidly progressing leg numbness and weakness, magnetic resonance imaging (MRI) showed a mass in the third lumbar vertebra. At a two-month follow-up, imaging showed a fracture, compression and lesion enlargement. Decompression and fixation confirmed ES, and the patient began combined radiotherapy and chemotherapy. Two months postoperatively, residual ES was suspected on MRI. The patient underwent a second surgery, and h

This paper reports a rare case of a lumbosacral dislocation associated with symphysis pubis separation and cauda equina syndrome. A 39-year-old male who diagnosed traumatic lumbosacral dislocation underwent an open reduction without fusion. After an open reduction and internal fixation of a symphysis pubis separation, a missed lumbosacral dislocation was diagnosed and an open reduction was performed without fusion. Due to the symphysis pubis separation, the patient was not allowed weight bearing for 3 months, which then began from wheel chair ambulation. At the 6-month follow up evaluation, th…

INTRODUCTION: Cauda equina syndrome (CES) is a rare but devastating spinal surgical emergency, most commonly caused by lumbar disc herniation. Delayed diagnosis or decompression may result in irreversible neurological and sphincter dysfunction.
MATERIALS AND METHODS: This retrospective single-Center case series included eight consecutive patients with acute CES who underwent emergency surgical decompression over 1 week at a tertiary spine care center. Clinical presentation, radiological findings, surgical management, and 6-month post-operative outcomes were evaluated.
RESULTS: The cohort inc…

BACKGROUND: Cauda equina syndrome (CES) with sexual dysfunction is an uncommon but serious complication of lumbar disc herniation. Giant lumbar disc herniations, defined as occupying more than 50% of the spinal canal, can cause severe neural compression. Delayed decompression in CES is traditionally associated with poor neurological and autonomic recovery.
OBSERVATIONS: A 36-year-old man presented with a 1-year history of low back pain and progressive radiculopathy, followed by 3 months of incomplete CES manifested by urinary dysfunction, constipation, saddle hypoesthesia, sexual dysfunction,…

INTRODUCTION AND IMPORTANCE: Cauda equina syndrome is a rare spinal emergency due to compression of the cauda equina nerve roots which present with bladder and/or bowel dysfunction, saddle anesthesia, and sexual dysfunction with or without a neurological deficit in the lower limbs and requires urgent surgical decompression through open methods or new minimally invasive procedures like Unilateral Biportal Endoscopic Discectomy.
CASE PRESENTATION: A 38-year-old serving army personnel presented with history of urinary incontinence, constipation and paresthesia of left lower limb for 1 week, whic…

Spinal epidural abscess is a severe, generally pyogenic, infection of the epidural space of spinal cord or cauda equina. The swelling caused by the abscess leads to compression or vascular disruption of neurological structures that requires urgent surgical decompression to avoid significant permanent disability. We share a rare case of Klebsiella pneumoniae spinal epidural abscess secondary to haematogenous spread of previous lung infection that presented late at our centre with cauda equina syndrome that showed good short-term outcome in delayed decompression. A 50-year old female presented w…
