One hundred normal lateral skull radiographs were studied and those of ten patients with basilar impression attending Kenyatta Hospital, Nairobi. The mean shortest distance of the odontoid tip to McGregor's basal line was 1.2 +/- 2.28 mm below the basal line (range 6 mm below to 3 mm above basal line), in normals and 9 +/- 2.7 mm (6-14 mm) above basal line in patients. The mean basal angle was 113 degrees +/- 7 degrees (102 degrees-133 degrees) in normals and 122 degrees +/- 6 degrees (113 degrees-125 degrees) in patients. The mean nasion-basion-opisthion angle was 162 degrees +/- 4 degrees (1

C1 laminectomy is a well-established posterior decompression procedure for craniovertebral junction stenosis. However, disruption of the ring structure of the atlas may alter load distribution across the anterior arch and lateral masses, occasionally resulting in atraumatic anterior arch fractures. Although several cases have been reported following bilateral C1 posterior arch resection or foramen magnum decompression, most developed weeks to months after surgery. In contrast, anterior arch fracture occurring during the acute postoperative period after unilateral C1 laminectomy has rarely been

BACKGROUND: Chiari I malformation (CIM) is a disorder characterized by caudal displacement of the cerebellar tonsils below the foramen magnum. It is often associated with syringomyelia and occasionally with hydrocephalus. CIM is commonly treated by posterior fossa decompression with or without removal of the posterior arch of C1 and duraplasty, but the treatment for infants with symptomatic CIM is not well established. We present a case of symptomatic CIM in an infant that was successfully treated with a ventriculoperitoneal shunt (VPS) and discuss the importance of the pathophysiology in mana

OBJECTIVE: Atlantoaxial instability may result from various pathologic conditions and operative treatment may be required to correct the deformity, provide stability and prevent neurological deficits. We presented our clinic's experience using C1-C2 fusion with polyaxial screw and rod fixation for C1 and C2 instability for various reasons.
METHODS AND MATERIAL: Four patients with atlantoaxial instability were operated using polyaxial C1 lateral mass and C2 lateral mass or pedicle screws. The mean age of the patients was 44+/-14,07 years.
RESULTS: Satisfactory screw placement was achieved in

STUDY DESIGN: Biomechanical comparison of 5 atlantoaxial posterior arthrodesis techniques.
OBJECTIVE: To assess the relative value of different posterior wire constructs when one or two transarticular screws are used.
SUMMARY OF BACKGROUND DATA: A combination of Gallie or Brooks techniques and 2 posterior transarticular screws has been shown to be effective for atlantoaxial arthrodesis. Anatomic constraints may preclude the insertion of a transarticular screw unilaterally or bilaterally.
METHODS: Ten adult human cadaveric upper cervical spine specimens were used. The specimens were tested i

Atlantoaxial stabilization has evolved from simple posterior wiring to transarticular screw fixation. In some patients, however, the course of the vertebral artery (VA) through the axis varies, and therefore transarticular screw placement is not always feasible. For these patients, the authors have developed a novel method of atlantoaxial stabilization that does not require axial screws. In this paper, they describe the use of this technique in the first 10 cases. Ten consecutive patients underwent the combined C1-3 lateral mass-sublaminar axis cable fixation technique. The mean age of the pat

We reviewed the findings of CT and myelogram of cases of os odontoideum. The diagnosis was confirmed by conventional tomogram in all these cases. Four cases were further confirmed by trans-oral decompression. Cartilage was found between the os odontoideum and the odontoid process during operation in these 4 cases. Four of them had no history of trauma and 2 of them had an associated anomaly; one was Down's syndrome, and the other was barrel chest and congenital dislocation of hips. CT findings of os odontoideum in these cases were a constriction and/or a gap of bony structure between the os od

The authors have observed 5 cases of atlanto-axial instability in children aged from 18 months to 8 years. All the cases were seen after severe trauma and presented with abnormal neurological signs. All patients were first treated conservatively and were operated on secondarily by atlanto-axial fusion using graft and wiring for persistent radiological instability between the atlas and axis. In two cases instability was due to an atlanto-axial subluxation with rupture of the transverse ligament; one of these was reduced by traction. In one case there was an anterior subluxation without fracture

Over a 23-month period, 25 patients aged 11 to 74 years presented to our Level I trauma center with odontoid lateral mass asymmetry of 2 to 5 mm on properly centered AP open-mouth X-rays: 32% of patients were asymptomatic, 68% had cervical pain, and 32% had limited range of motion. Patients with cervical spine fractures or dislocations and those with fixed deformity were excluded. The clinical significance of asymmetry was determined utilizing dynamic axial CT scanning of the occiput (C0), atlas (C1) and axis (C2) with the head neutral and with 15 degrees to 30 degrees active rotation. Occult traumatic lesions of the cervical spine usually involve the smaller, posterior elements of the vertebrae. Chronic neck pain associated with these lesions is intermittent and of variable severity, but with patterns that are fairly characteristic of the osseous lesions. Similarly, the long-term radiographic sequelae occur in fairly predictable patterns. The original osseous injury characteristically involves a fracture deformity and frequently some instability. There is a closed five point suspension system at each of the levels of the lower cervical spine, so that each of these five poin

During 1 year 6 patients with dislocated fractures of C-1 and C-2 vertebrae (fractures of the odontoid process) and 1 patient with a pseudarthrosis of the odontoid process have been treated. 6 patients have been treated surgically by internal fixation of the arch of C-1 and C-2 spinous process by wiring and plastic material (Palacos). No complications occurred. The advantage of the method was early mobilization of the patients and reduction of hospitalization. Clinical and radiographic follow-up studies showed stable fusion on Flexion-extension X-rays and boney union of the fracture of the odo

The inflammatory involvement of cervical spine ligaments in patients with rheumatoid arthritis (RA) with atlantoaxial subluxation was studied. Biopsy specimens were obtained from the ligament structures between the posterior arch of the atlas and the spinous process of C2 during atlantoaxis stabilizing operations. Inflammatory cells in cryostat sections were stained in situ using monoclonal hybridoma antibodies applied in a sensitive avidin-biotin-peroxidase complex (ABC) method. In 4 of 8 ligament specimens, focal inflammatory cell infiltrates consisting mainly of T11 + T lymphocytes (77 +/-

BACKGROUND: Various methods have been used to stabilize the vertebra in cervical fractures, including the use of various orthoses and surgery. However, it is not cleared which type of orthosis is more suitable for the subjects with cervical fractures to best immobilize the vertebra and to decrease the associated side effects. Therefore, the aim of this study was to evaluate the efficiency of various orthoses based on the available literature.
MATERIALS AND METHODS: A search was done in some databases include PubMed, ISI Web of Knowledge, EBSCO, Embasco, and Google Scholar. The search was done

Approximately one-third of all cervical spine injuries involve the craniocervical junction (CCJ). Composed of the occiput and the first two cervical vertebrae, this important anatomic landmark, in conjunction with an intricate ligamentous complex, is essential to maintaining the stability of the cervical spine. The atlantoaxial joint is the most mobile portion of the spine, predominantly relying on the ligamentous framework for stability at that level. As acute onsite management of trauma patients continues to improve, CCJ injuries, which often lead to death onsite where the injury occurred, a

BACKGROUND: Simultaneous C1 and C2 fractures are increasingly common, but contemporary series are limited.
METHODS: All patients with traumatic fractures of both C1 and C2 admitted to an academic trauma center from 2012 to 2022 were retrospectively analyzed. Multivariable regression was used to identify characteristics relevant to management and outcomes.
RESULTS: Of 103 patients identified, most were ≥80 years old (52.4%), sustained ground-level falls (80.6%), and had minor associated injuries (median Injury Severity Score 1); a 28.2% had a 1-year mortality rate. Landells type 1 fractures w

BACKGROUND: Several bone grafting techniques for posterior atlantoaxial arthrodesis have been reported. The techniques of placing a cancellous morselized bone graft (MBG) on decorticated surfaces of the atlantoaxial complex and securing a structural iliac bone graft (SBG) between C1 and C2 have been used widely. The aim of the present study was to compare the outcomes of these 2 bone grafting techniques for atlantoaxial arthrodesis.
METHODS: The data from 64 patients with reducible atlantoaxial dislocation treated using posterior C1-C2 screw-rod fixation and fusion were retrospectively review

Dumbbell C1 and C2 schwannomas are rare and have a distinctive presentation and anatomical features. To study the clinical characteristics of these tumors, we reviewed the microsurgical management of 18 patients with dumbbell C1 and C2 schwannomas by the far lateral approach. Data regarding clinical manifestations, radiological findings and surgical results were analyzed retrospectively. Total and subtotal resection of the tumor was achieved in 15 and three patients, respectively. At the time of discharge, 12 patients showed improvement while five patients remained the same. The average follow

Pedicle screw instrumentation of the upper cervical spine is rarely performed in trauma surgery because of the risk of damaging neurovascular structures. We report successful treatment of an unstable hangman's fracture with posterior pedicle screw fixation using Iso-C3D fluoroscopy-based computer navigation guidance. Postoperative computed tomographic images confirmed accurate placement of the pedicle screws. The navigation system is useful, especially in an unstable upper cervical spine injury where the likelihood of change in the inter-segmental relationship is maximal before and after posit
