Vesicular agenesis and cholangitis, an infrequent association. Case report.

BACKGROUND: Vesicular agenesis is a rare congenital malformation. CLINICAL CASE: Male, 65 years old, preoperative diagnosis of gallbladder agenesis, presenting with cholangitis, unsatisfactory ERCP, resolution by laparotomy, drainage and biliary exploration. DISCUSSION: The association between cholangitis and gallbladder agenesis presents few reports in the universal literature, a preoperative diagnosis reduces surgical risk, anatomical alteration represents an increased risk of iatrogenic injury during surgical exploration. CONCLUSIONS: Vesicular agenesis, a rare and even more infrequent e…

Massive biliary ascariasis: an unusual cause of acute cholangitis.

Acute cholangitis is a condition of bacterial infection following hepatobiliary tract obstruction, which signifies poor prognosis unless adequately drained. The most common cause of bile duct obstruction is choledocholithiasis, in contrast to parasitic infestation, a rare entity causing acute cholangitis nowadays. Therefore, we reported the case of a 68-year-old Thai man who presented with acute fever, intense right upper quadrant abdominal pain and jaundice for 2 days. His medical history was normal except for the history of intermittent biliary colic for a year. Endoscopic retrograde cholang…

A rare case of acute cholangitis after endoscopic ampullectomy.

Endoscopic ampullectomy is indicated for the resection of non-invasive papillary adenomas in selected patients. Cholangitis is an uncommon complication (0-2%) that may be secondary to contamination during the procedure, poor emptying of the bile duct and prosthesis dysfunction or migration. Placement of a prophylactic biliary stent after the resection is not well established. We present a rare case of acute cholangitis after endoscopic ampullectomy secondary to a biliary prosthesis obstruction, due to a pancreatic prosthesis intrusion.

Review article: pathophysiology and management of primary biliary cholangitis.

BACKGROUND: Primary biliary cholangitis (PBC), an immune-mediated disease characterised by destruction of intrahepatic bile ducts, results in progressive damage to the biliary tree, cholestasis and ultimately advanced liver disease. In the last decade, advances in practice have improved clinical care, driven novel therapeutic options and improved risk stratification tools. AIMS: To provide an overview of the disease characteristics of PBC and review a patient-centred management approach for the clinical team caring for those with PBC. METHODS: We reviewed the current literature and guideline…

Characteristics of serum chemokine profile in primary biliary cholangitis.

Although chemokines have been believed to exert a pivotal role in pathogenesis of primary biliary cholangitis (PBC), comprehensive analysis of circulating chemokine profile in PBC has been little performed. The aim of this study is to determine serum chemokine profile and to explore its association with the development and progression of PBC. Sixty PBC patients and 30 healthy controls (HC) were enrolled. The sera were detected for 14 chemokines using multiplex immunoassay. The expression of CD3 and CD68 in the portal area of liver tissues was determined by immunohistochemistry in 6 PBC patient…

Patient ownership of primary biliary cholangitis long-term management.

OBJECTIVE: Patient ownership of disease is vital in rare diseases like primary biliary cholangitis (PBC). This survey of UK members of the PBC foundation aimed to assess patients' perception of their disease management, focusing on key biomarkers and problematic symptoms. DESIGN: Registered PBC foundation members were surveyed on their experiences on their most recent clinic visit, covering the type of hospital and clinician and whether biochemical response and symptom burden were discussed, including who initiated these conversations. Respondents were also asked about their willingness to in…

Linear Scleroderma and Primary Sclerosing Cholangitis: An Undescribed Association.

Linear scleroderma (LS) is clinically characterized by the presence of sclerotic areas of skin, which develop in a linear pattern. Primary sclerosing cholangitis is a cholestatic disorder that can lead to end-stage liver disease. We present, for the first time in English literature, the case of a patient suffering from both the diseases. This highlights the fact that, even though LS has conventionally been considered to be a form of localized scleroderma, this does not necessarily imply that it is an exclusively cutaneous disease.

Acute Cholangitis Caused by Boryong Strain of Orientia tsutsugamushi.

Scrub typhus present with a variety clinical manifestations caused by Orientia tsutsugamushi, it is sometimes accompanied by severe complications. The clinical course of this complication is not well clear. Herein, we first report a case with acute cholangitis and acalculous cholecystitis caused by the Boryong genotype of O. tsutsugamushi. A 82-year-old woman presented with fever and acute abdominal pain on the right upper quadrant. Initially, the patient showed an abnormality of liver enzymes of cholestatic pattern. An acute cholangitis and cholecystitis were suspected on the enhanced compute…

Hospitalization Burden of Biliary Strictures and Cholangitis After Pancreaticoduodenectomy.

BACKGROUND: Postsurgical biliary disease in Roux-en-y and cholecystectomies has been investigated, but less literature exists regarding biliary complications after Whipple procedure (pancreaticoduodenectomy [PD]). Moreover, the hospital burden incurred after this complication has not been previously examined. The aim of this study is to assess the trends in hospitalization for biliary strictures and cholangitis after PD. MATERIALS AND METHODS: The National Inpatient Sample identified all cases with a PD and a primary diagnosis of biliary complication in 2014. Cases were identified using the I…

Early Cholangitis after Portoenterostomy in Children with Biliary Atresia.

AIMS AND OBJECTIVES: Biliary atresia (BA) is a cholangiodestructive disease of the biliary tree. The first line of treatment is a Kasai portoenterostomy (PE) following which patients may develop cholangitis. We studied the effect of early cholangitis on the outcome of PE, namely jaundice clearance and early native liver survival (NLS). METHODS: We reviewed the data of all children who developed cholangitis after PE from our prospectively maintained database of children with BA. The standardized treatment of all children in the database is described. The frequency and nature of these episodes …

Obeticholic acid in primary biliary cholangitis: where we stand.

PURPOSE OF REVIEW: This review will summarize the use of obeticholic acid (OCA) in treatment of primary biliary cholangitis (PBC). It seeks to discuss the mechanism of action, evidence for use, appropriate clinical use, and common adverse effects of OCA. RECENT FINDINGS: PBC is a chronic, progressive cholestatic liver disease that is a chronic progressive that may lead to end-stage liver disease and need for liver transplantation. Ursodeoxycholic acid (UDCA) has been the mainstay of therapy for PBC for decades. Recent research has led to the discovery that bile acids act as hormones and have …

Cancer risk, screening and surveillance in primary sclerosing cholangitis.

Primary sclerosing cholangitis (PSC) is a rare chronic inflammatory condition mainly of the large bile ducts, affecting predominantly young men, and is associated with the presence of inflammatory bowel disease. There is no known cure for PSC, which progresses to cirrhosis or death over 10-20 years. Hepatobiliary malignancy, especially cholangiocarcinoma, is a feared complication associated with poor overall survival. Screening and surveillance appear to improve overall outcomes. To capture as many relevant studies, broad search criteria were employed within the PubMed database. Given the high…

Primary Sclerosing Cholangitis and Psychological Wellbeing: A Scoping Review.

Primary sclerosing cholangitis (PSC) is a chronic liver disease resulting from the inflammation and scarring of an individual's hepatic bile ducts. With no curative treatment available and a risk of potentially severe complications and death, it is likely that those diagnosed with the illness may experience impairments in their psychological wellbeing. The aim of this scoping review is to locate, chart, and summarize all available literature on how PSC affects mental health and psychological wellbeing, as well as the factors that may or may not impact on the psychological wellbeing of those wh…
