The Gut-Prostate axis in chronic Prostatitis/Chronic Pelvic pain syndrome: Mechanisms, evidence gaps, and biomarker-stratified translation.

Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a heterogeneous disorder traditionally framed around local prostate, pelvic-floor, neurological, and psychosocial factors. Emerging evidence suggests that gut dysbiosis may contribute to CP/CPPS in a subset of patients through barrier dysfunction, immune priming, microbial metabolites, and neuroimmune modulation. We propose a Dual-Hit Permissive Model in which gut-derived systemic immunometabolic priming must converge with prostate-local or pelvic permissive cues before organ-localized inflammation and chronic pain can be sustained.

Results of a Peer Navigator Program to Address Chronic Illness Among Persons Experiencing Homelessness.

People who are homeless disproportionately experience the burdens of chronic disease, have limited access to preventive care, and may be less trusting of healthcare agencies. The Collective Impact Project created and evaluated an innovative model designed to increase chronic disease screening and referral to healthcare and public health services. Trained Peer Navigators (PNs), who were paid staff with lived experiences similar to the clients served, were embedded in five agencies serving people experiencing homelessness or at risk for homelessness. Over two years, PNs engaged 1071 individuals.

Models of Telehealth Service Delivery in Adults With Spinal Cord Injuries: Scoping Review.

BACKGROUND: In Canada, approximately 86,000 people live with spinal cord injury (SCI), and there are an estimated 3675 new cases of traumatic or nontraumatic etiology per year. Most people with SCI will experience secondary health complications, such as urinary and bowel issues, pain syndrome, pressure ulcers, and psychological disorders, resulting in severe chronic multimorbidity. Moreover, people with SCI may face barriers in accessing health care services, such as primary care physicians' expert knowledge regarding secondary complications related to SCI. Telehealth, defined as the delivery

Does my patient have chronic Chagas disease? Development and temporal validation of a diagnostic risk score.

INTRODUCTION: With the globalization of Chagas disease, unexperienced health care providers may have difficulties in identifying which patients should be examined for this condition. This study aimed to develop and validate a diagnostic clinical prediction model for chronic Chagas disease. METHODS: This diagnostic cohort study included consecutive volunteers suspected to have chronic Chagas disease. The clinical information was blindly compared to serological tests results, and a logistic regression model was fit and validated. RESULTS: The development cohort included 602 patients, and the v

Reducing opioid use for chronic pain with a group-based intervention: Economic evaluation using randomised controlled trial data.

BACKGROUND AND AIMS: Long-term strong opioid use for chronic non-malignant pain (CNMP) represents a significant public health challenge, associated with reduced health-related quality of life (HRQoL) and increased mortality. The Improving the Wellbeing of people with Opioid Treated CHronic pain (I-WOTCH) trial demonstrated that a multicomponent intervention supporting opioid withdrawal in people with CNMP successfully reduced opioid consumption compared with best usual care (BUC), without adversely affecting perceived pain levels. This study aimed to assess the value-for-money of the strategie

Budget impact modeling for a single-tablet formulation of ibuprofen and famotidine for prevention of upper gastrointestinal ulcers in patients with osteoarthritis and/or rheumatoid arthritis.

BACKGROUND: Single-tablet ibuprofen/famotidine is approved by the US Food and Drug Administration for the relief of signs and symptoms of rheumatoid arthritis and osteoarthritis and to decrease the risk of developing upper gastrointestinal (GI) ulcers in patients taking ibuprofen for those indications. Currently, little is known about the cost impact of gastroprotective therapies, and an estimate of the financial consequences of adopting these therapies will be helpful to decision makers. OBJECTIVES: The goal of this study was to review a model that evaluates the expected financial impact to

Allostatic Load and Preterm Birth.

Preterm birth is a universal health problem that is one of the largest unmet medical needs contributing to the global burden of disease. Adding to its complexity is that there are no means to predict who is at risk when pregnancy begins or when women will actually deliver. Until these problems are addressed, there will be no interventions to reduce the risk because those who should be treated will not be known. Considerable evidence now exists that chronic life, generational or accumulated stress is a risk factor for preterm delivery in animal models and in women. This wear and tear on the bod

Emerging Tissue and Blood Biomarkers for Eosinophilic Esophagitis.

Eosinophilic esophagitis is a chronic, immune-mediated condition traditionally diagnosed and monitored through sedated endoscopy with biopsy. This invasive method, while essential, poses challenges for routine care. This article summarizes current research on noninvasive and minimally invasive biomarkers spanning blood, tissue, and esophageal sampling methods that aim to improve diagnostic accuracy, monitor disease activity, and guide personalized treatment. While no biomarker is yet validated for clinical use, advances in translational science and technology hold promise for integrating these

Beyond Triage: The Critical Role of Emergency Nurses in COPD Assessment and Management-Insights from Patients and Staff.

Background: Chronic Obstructive Pulmonary Disease (COPD) remains a leading cause of emergency department (ED) presentation, hospitalisation, and preventable healthcare utilisation worldwide. Although guidelines advocate coordinated, preventative, and community-based management, care within ED settings often remains reactive and crisis-driven. Nurses occupy a central role in COPD management; however, the experiential dimensions of nursing practice and its contribution to improving patient outcomes are insufficiently understood. Objectives: To explore the lived experiences of patients, nurses an

Comparing Eight Prognostic Scores in Predicting Mortality of Patients with Acute-On-Chronic Liver Failure Who Were Admitted to an ICU: A Single-Center Experience.

Limited data is available on long-term outcome predictions for patients with acute-on-chronic liver failure (ACLF) in an intensive care unit (ICU) setting. Assessing the reliability and accuracy of several mortality prediction models for these patients is helpful. Two hundred forty-nine consecutive patients with ACLF and admittance to the liver ICU in a single center in northern Taiwan between December 2012 and March 2015 were enrolled in the study and were tracked until February 2017. Ninety-one patients had chronic hepatitis B-related cirrhosis. Clinical features and laboratory data were col

Knowledge development: the roy adaptation model and bulimia nervosa.

The utility of nursing theory as a framework to understand, explain, and propose nursing research is essential to knowledge development. The purpose of this column is to provide an examination of the Roy adaptation model as a basis for knowledge development in bulimia nervosa. This chronic and debilitating disorder is complex, poorly understood, and characterized by repeated binge eating and purging episodes. A careful examination of the major concepts and theoretical linkages of the Roy adaptation model in relationship to bulimia nervosa is necessary to determine the value of this framework t

Root Cause Analysis-based Risk Management Nursing Protocol For Patients With Gastric Ulcer.

Gastric ulcer is a chronic digestive disorder characterized by recurrence, complications, and reduced quality of life. Although pharmacological treatment remains essential, variability in nursing processes, incomplete risk identification, and insufficient follow-up may hinder recovery and long-term disease control. Root cause analysis (RCA) is a structured quality-improvement method used to identify underlying causes of care-related risks and to guide targeted corrective action. This article describes a reproducible RCA-based risk management nursing workflow for hospitalized patients with gast

Physical and social wellbeing of family caregivers of persons with hepatitis B associated chronic liver disease in Ghana: a qualitative study.

BACKGROUND: Hepatitis B is one of the most common viral (HBV)infections that affect the liver. Infection with the virus may result in varying severity of liver disease which may be acute or chronic. Though most people recover from the infection, about 5 - 10% of cases lead to chronic infection. Persons who develop HBV-related debilitating liver disease will likely require informal care from family caregivers. AIM: This study sought to explore the physical and social wellbeing of family caregivers of persons with hepatitis B-associated chronic liver disease in a tertiary hospital in the Centra

Psychosomatic interplay in postmenopausal osteoporosis: from neuroendocrine mechanisms to structured psychological therapies and long-term management.

Postmenopausal osteoporosis (PMOP) is a prevalent chronic condition that severely compromises both the physical and psychological well-being of aging women. Beyond its skeletal implications, PMOP exhibits a profound psychosomatic interplay, intimately linked with elevated rates of depression and anxiety that frequently impair treatment adherence and functional recovery. This psychological vulnerability arises from a complex web of hormonal shifts, fear of fracture, and significant global sociocultural determinants that restrict care access. This comprehensive narrative review synthesizes curre
