Multimodal liquid biopsy surveillance after pancreatectomy for pancreatic ductal adenocarcinoma: evidence and a confirmation‑gated, trial‑ready framework.

Relapse after curative‑intent pancreatectomy for pancreatic ductal adenocarcinoma (PDAC) is common and may become clinically apparent only after a delay in conventional imaging and CA19‑9 monitoring. Liquid biopsy offers serial access to tumor‑derived signals, but post‑operative minimal residual disease (MRD) is often near the assay limit of detection, raising risks of false positives, over‑imaging, and premature treatment escalation outside trials. We performed a scoping review of clinical studies evaluating circulating tumor DNA (ctDNA), circulating tumor cells (CTCs), and extracellular vehi…

Cholangiocarcinoma: The era of liquid biopsy.

Cholangiocarcinoma (CCA) is a highly aggressive and heterogeneous malignancy arising from the epithelial cells of the biliary tract. The limitations of the current methods in the diagnosis of CCA highlight the urgent need for new, accurate tools for early cancer detection, better prognostication and patient monitoring. Liquid biopsy (LB) is a modern and non-invasive technique comprising a diverse group of methodologies aiming to detect tumour biomarkers from body fluids. These biomarkers include circulating tumour cells, cell-free DNA, circulating tumour DNA, RNA and extracellular vesicles. Th

ccfDNA analysis for the classification of adrenocortical adenomas.

BACKGROUND: Somatic alterations are commonly observed in adrenocortical adenomas including cortisol-producing (CPA) [overt Cushing syndrome (CS) or mild autonomous cortisol secretion (MACS)], aldosterone-producing (APA), and non-functioning (NFAT) tumors. We tested whether somatic variants could be detected in circulating cell-free DNA (ccfDNA) from patients with adenomas and potentially contribute to management strategies. MATERIALS AND METHODS: We investigated 44 patients (17 CPA-MACS, 9 CPA-CS, 12 APA, and 6 NFAT). 23 healthy subjects (HS) served as controls. ccfDNA was extracted from bloo

Update from the laboratory: mechanistic studies of pathways of cancer-associated venous thrombosis using mouse models.

Cancer patients have an increased risk of venous thromboembolism (VTE). The rate of VTE varies with cancer type, with pancreatic cancer having one of the highest rates, suggesting that there are cancer type-specific mechanisms of VTE. Risk assessment scores, such as the Khorana score, have been developed to identify ambulatory cancer patients at high risk of VTE. However, the Khorana score performed poorly in discriminating pancreatic cancer patients at risk of VTE. Currently, thromboprophylaxis is not recommended for cancer outpatients. Recent clinical trials showed that factor Xa (FXa) inhib

Dynamic Precision Oncology for Real-Time Molecular Monitoring and Management in Urothelial Carcinoma.

The management of urothelial carcinoma (UC) is undergoing a paradigm shift from static anatomical staging to molecularly guided dynamic approaches that integrate time as a critical therapeutic variable. This evolution is driven by liquid biopsies, particularly circulating tumor DNA, which allow real-time tumor interrogation. We conducted this expert review to synthesize landmark evidence, enabling technologies, and implementation challenges in dynamic precision oncology for UC. In this non-systematic narrative review, we searched PubMed/MEDLINE, Embase, Web of Science, and the Cochrane Library…

A Targeted, Personalized Management Strategy in Advanced Penile Squamous Cell Carcinoma.

Penile squamous cell carcinoma (PSCC) is a rare malignancy with poor outcomes in advanced stages, with dismal response and survival rates using conventional surgical and systemic options. Additionally, the ability to detect and monitor residual disease with current imaging modalities remains difficult. Therefore, advances in multimodal management and disease monitoring are desperately needed. We present a case of advanced PSCC utilizing multimodal management informed by next-generation sequencing and circulating tumor DNA monitoring. These genomic techniques were valuable in guiding management…

Inferring tissue origin from cell-free DNA in metastatic cancer.

A major clinical challenge in metastatic cancer management is determining not only the presence of tumor-derived cell-free DNA (cfDNA) but also the tissue lineage and organ context reflected by circulating cfDNA signals. CfDNA supports such inference by retaining molecular signatures specific to its tissue-of-origin, including DNA methylation, 5-hydroxymethylcytosine (5hmC), fragmentomic profiles, nucleosome positioning patterns, and histone modifications. This review examines the biological principles underlying cfDNA-based tissue-of-origin determination, the computational frameworks used to

Analysis of cell-free DNA in lymphomas: from sample collection to genotyping and minimal residual disease monitoring.

Circulating tumor DNA (ctDNA) is increasingly investigated in lymphomas because it enables noninvasive molecular profiling, longitudinal assessment of clonal evolution, and quantification of minimal residual disease (MRD), which reflects residual tumor burden and treatment response and serves as a clinically validated prognostic biomarker. The clinical utilities of ctDNA include supporting diagnosis, enabling early detection of relapses, and resolving ambiguous imaging findings. Current approaches for ctDNA assessment in lymphomas include droplet digital polymerase chain reaction, immunoglobul…

Ultra-sensitive detection of somatic mutations in androgen receptor conferring resistance to anti-androgen therapy in prostate cancer.

Prostate cancer (PCa) is primarily driven by androgen receptor (AR) signaling, where a potent form of androgen, such as dihydrotestosterone (DHT), binds with AR to promote tumor cell proliferation and survival. Somatic mutations in AR gene are recognized as key predictive indicators of therapeutic resistance to anti-androgen therapy in advanced stages of PCa. To enable minimally invasive detection of these alterations, we developed an ultrasensitive digital droplet PCR (ddPCR) assay capable of quantifying absolute copies of AR mutant alleles relative to wild-type AR using cell-free circulating

Screening and Prevention for High-Grade Serous Carcinoma of the Ovary Based on Carcinogenesis-Fallopian Tube- and Ovarian-Derived Tumors and Incessant Retrograde Bleeding.

High-grade serous carcinoma (HGSC) is the most common and lethal subtype of ovarian carcinoma. Many HGSCs are now believed to originate in the fallopian tube epithelium; ovarian surface epithelium is another possible origin. Thus, current screening methods, i.e., ultrasonography and serum CA-125 measurements, have a limitation in their early detection. Recently, circulating biomarkers, such as tumor DNA, autoantibody, and microRNA, have been investigated to detect HGSCs. As cancer cells in the fallopian tube flow into the endometrial cavity, the detection of exfoliated cells, tumor DNA, and pr
