Non-auditory stimulation in adult cochlear implant users.

This is a retrospective case review of all 163 adults to have received multichannel cochlear implants on the Manchester University/Manchester Royal Infirmary programme between 1988 and 1998. The aims were to investigate the incidence of unwanted non-auditory effects of electrical stimulation (NAS) of the cochlea, and to try to identify any factors that seemed to be related to these effects. The effectiveness of programming strategies in eliminating unwanted effects was also studied. Most of the devices were Nucleus CI 22M or CI 24M. The remainder were Med-el Combi 40 or Combi 40+. NAS occurred

A concept for a research tool for experiments with cochlear implant users.

APEX, an acronym for computer Application for Psycho-Electrical eXperiments, is a user friendly tool used to conduct psychophysical experiments and to investigate new speech coding algorithms with cochlear implant users. Most common psychophysical experiments can be easily programmed and all stimuli can be easily created without any knowledge of computer programing. The pulsatile stimuli are composed off-line using custom-made MATLAB (Registered trademark of The Mathworks, Inc., http://www.mathworks.com) functions and are stored on hard disk or CD ROM. These functions convert either a speech s

The magnetless Clarion cochlear implant in a patient with neurofibromatosis 2.

We present our experience using the Clarion magnetless multichannel cochlear implant with a woman profoundly deafened following bilateral acoustic neuromata as a consequence of neurofibromatosis 2 (NF2). The right neuroma had been previously removed without an attempt at neural preservation. On the left, however, a posterior fossa approach had been taken with the aim of preserving hearing. Although the left cochlear nerve appeared to be undamaged at the end of the operation, no hearing thresholds could be elicited on post-operative audiometry, because of damage either to the cochlear nerve or

A prospective study of the cost-utility of the multichannel cochlear implant.

CONTEXT: Prior clinical studies have indicated that cochlear implantation provides benefits to individuals with advanced sensorineural hearing loss who are unable to gain effective speech recognition with hearing aids. OBJECTIVE: To determine the cost per quality-adjusted life-year (QALY) for adults receiving multichannel cochlear implants. DESIGN: Prospective 12-month multicenter study using preference-based quality-of-life measures and total cost determinations, comparing profoundly hearing-impaired adult subjects with and without cochlear implants. SETTING: Hospital-based and patient-res…

A model of educational resource use by children with cochlear implants.

We have tracked patterns of use of educational and rehabilitative resources as part of an initial assessment of cost-benefit ratios of cochlear implants in children. Forty-two children with cochlear implants in the Listening Center at Johns Hopkins program of aural rehabilitation have served as our study cohort to develop the measures to be assessed. An educational resource matrix stratifies school setting (residential vs special education vs non-specialized "mainstream" setting) and levels of rehabilitative support (speech and language therapy and interpreter use) to map past and current use

[Cochlear implant in inner ear abnormalities and footplate malformation].

BACKGROUND: Malformations of the cochlea can cause deafness and otogenic meningitis. Cochlear implantation in children with such malformations, which are sometimes quite complex, require special attention before, during and after surgery. PATIENT: The concept developed for these patients at Hannover Medical School is illustrated in a patient with severe bilateral inner ear malformation and bilateral stapes defect. Implantation of a cochlear implant was performed in this 2-year-old girl. A large endostal sack originating from the inner ear, which had caused meningitis twice, was observed in th…

Objective electrophysiological measures from cochlear implant patients.

Various objective electrophysiological techniques have been used to supplement the behavioral measurements for cochlear implant patients. This report summarizes various studies of these measurements, including stimulus artifact, averaged electrode voltages, electrical stapedius reflexes, electrical auditory brain stem responses, electrical middle latency responses, and cortical auditory responses. These techniques have been shown to provide valuable information in the evaluation of device and electrode integrity in addition to correlated values to behavioral thresholds and comfort levels.

Intraoperative Cochlear Nerve Monitoring for Vestibular Schwannoma Resection and Simultaneous Cochlear Implantation in Neurofibromatosis Type 2: A Case Series.

BACKGROUND: Neurofibromatosis type 2 (NF2) often results in profound hearing loss and cochlear implantation is an emerging hearing rehabilitation option. However, cochlear implant (CI) outcomes in this population vary, and intraoperative monitoring to predict cochlear nerve viability and subsequent outcomes is not well-established. OBJECTIVE: To review the use of intraoperative electrically evoked cochlear nerve monitoring in patients with NF2 simultaneous translabyrinthine (TL) vestibular schwannoma (VS) resection and cochlear implantation. METHODS: A retrospective review was performed of 3…

Severe and persistent facial nerve stimulation after cochlear implantation in a patient with cochlear-facial dehiscence: a case report.

Generally, cochlear implants (CIs) are effective in helping patients improve their hearing performance; however, some patients have poor hearing performance owing to facial nerve stimulation (FNS), which is often associated with cochlear anomalies. We report a case with a normal cochlea and severe and persistent FNS owing to cochlear-facial dehiscence (CFD) that affected the CI outcomes. Preoperatively, a careful review of the computed tomography images before CI surgery is necessary not only for patients with otosclerosis and inner ear malformations but also for patients with normal cochlear

Perceptual organization of Mandarin consonants in cochlear implant users.

This study provides a systematic characterization of Mandarin consonant perception in adult cochlear implant (CI) users and benchmarks the resulting perceptual organization against a condition-matched hearing-aid (HA) dataset and a previously reported normal-hearing (NH) reference. Thirty-five Mandarin-speaking CI listeners completed a 21-alternative closed-set identification task using 21 /C(i)ā/ syllables presented in quiet. Data were analyzed using accuracy, hierarchical clustering, multidimensional scaling, and feature-based information transmission. CI listeners achieved moderate overall

The Audiological Benefits of the CROS System for Cochlear Implant Users.

INTRODUCTION: When a unilateral cochlear implant (CI) is implanted in a bilaterally deaf person they may still experience, along with significant benefits, certain limitations. The use of a contralateral routing of signal (CROS) system, which transmits the signal from the deaf side to the side with the speech processor, creates an opportunity to reduce these limitations and improve hearing performance in difficult acoustic conditions. In this study, a wireless CROS solution from Advanced Bionics specifically designed for CI speech processors is investigated. METHODS: Speech-in-noise tests bas

Ten years of research on the sound quality of cochlear implants.

This report summarizes data collected over the span of more than a decade from four laboratories on the sound quality of cochlear implants. Sixteen projects (ten published and six unpublished) are summarized; two video and eleven audio files accompany the summaries. Fourteen of the projects used single sided deaf patients (SSD) fit with an implant (CI) as listeners; one project used SSD-CI patients with preserved hearing (HP) in their implanted ear; one project used conventional CI patients. Approximately a third of the SSD-CI patients could create close matches to CI sound quality. The matche

Pulse timing dominates binaural hearing with cochlear implants.

Although cochlear implants (CIs) provide valuable auditory information to more than one million profoundly deaf patients, these devices remain inadequate in conveying fine timing cues. Early deaf patients in particular struggle to use interaural time differences (ITDs) for spatial hearing and auditory scene analysis. Why CI patients experience these limitations remains controversial. One possible explanation, which we investigate here, is that the stimulation by clinical CIs is inappropriate, as it encodes temporal features of sounds only in the envelope of electrical pulse trains, not the pul

Understanding the Process of Integration in Binaural Cochlear Implant Configurations.

OBJECTIVES: Cochlear implant (CI) users with access to hearing in both ears (binaural configurations) tend to perform better in speech perception tasks than users with a single-hearing ear alone. This benefit derives from several sources, but one central contributor may be that binaural hearing allows listeners to integrate content across ears. A substantial literature demonstrates that binaural integration differs between CI users and normal hearing controls. However, there are still questions about the underlying process of this integration. Here, we test both normal-hearing listeners and CI

The everyday speech environments of preschoolers with and without cochlear implants.

Children who receive cochlear implants develop spoken language on a protracted timescale. The home environment facilitates speech-language development, yet it is relatively unknown how the environment differs between children with cochlear implants and typical hearing. We matched eighteen preschoolers with implants (31-65 months) to two groups of children with typical hearing: by chronological age and hearing age. Each child completed a long-form, naturalistic audio recording of their home environment (appx. 16 hours/child; >730 hours of observation) to measure adult speech input, child vocal

[Indications for Cochlear Implants in Children: Where is the Journey Heading?].

The indications for cochlear implantation in children have been gradually expanded during the last 30 years. This concerns both the age at implantation and the audiological criteria, unilateral or bilateral treatment, the treatment of unilateral deafness and of children with multiple disabilities. Due to the improvement in hearing preservation and new technical developments, it is to be expected that the indication limits will continue to shift in the coming decades.

Barriers to Early Progress in Adult Cochlear Implant Outcomes.

OBJECTIVES: Adult cochlear implant (CI) recipients obtain varying levels of speech perception from their device. Adult CI users adapt quickly to their CI if they have no peripheral "bottom-up" or neurocognitive "top-down" limiting factors. Our objective here was to understand the influence of limiting factors on the progression of sentence understanding in quiet and in noise, initially and over time. We hypothesized that the presence of limiting factors, detected using a short test battery, would predictably influence sentence recognition with practical consequences. We aimed to validate the t

Vocal Emotion Perception in Children Using Cochlear Implant.

The significance of emotional prosody in social communication is well-established, yet research on emotion perception among cochlear implant (CI) users is less extensive. This study aims to explore vocal emotion perception in children using CI and bimodal hearing devices and compare them with their normal hearing (NH) peers. The study involved children aged 4-10 years with unilateral CI and contralateral hearing aid (HA), matched with NH peers by gender and listening age. Children were selected using snowball sampling for the CI group and purposive sampling for the NH group. Vocal emotion perc
