Long-term follow-up of high-pressure injection injuries to the hand.

BACKGROUND: High-pressure injection injury is an injury caused by accidental injection of substances by industrial equipment. This injury may have devastating sequelae. The goal of this study was to assess the long-term outcome of high-pressure injection injury to the hand. METHODS: In this historical prospective study, patients who had previously sustained high-pressure injection injury were examined. Assessment included measurement of grip and pinch strength, range of motion, two-point discrimination, and use of a questionnaire regarding present complaints and return to the work force. RES

Equestrian cold panniculitis in women.

We describe four patients with panniculitis attributable to a combination of cold exposure and equestrian activities. All were young, healthy women who rode horses for at least two consecutive hours per day throughout the winter. Initially, several small, erythematosus, pruritic papules appeared on the superior-lateral portions of one or both thighs. During one week, the lesions progressed to indurated, red-to-violaceous,tender plaques and nodules. Studies for cryofibrinogens and cryoglobulins were negative. The histologic picture was that of a panniculitis with prominent inflammation of veins

A new method of preventing bile duct injury in laparoscopic cholecystectomy.

AIM: Of all the complications of laparoscopic cholectecystomy, bile duct injury (BDI) is the most serious complication. The prevention of injury to the common bile duct (CBD) remains a significant concern in laparoscopic cholecystectomy (LC). Different kinds of methods have been advanced to avoid this injury but no single method has gained wide acceptance. Because of various limitations of current methodologies we began a study using cold light illumination of the extrahepatic biliary system (light cholangiography LCP) to better visualize this area and thereby reduce the risk of bile duct inju

Can Prolonged Ischemia Time in Extremity Transfer Be Resolved Using an Extracorporeal Circulation Model? An Experimental Study.

BACKGROUND: The standard method for transporting tissues during limb transplantation and replantation is cold ischemic transport (CIT). However, CIT cannot completely prevent ischemia-reperfusion injury (IRI). As an alternative, extracorporeal perfusion (ECP) methods that provide an adequate metabolic environment for ischemic tissues could be considered. In this study, we investigated the differences between CIT and ECP in terms of their effects on IRI. METHODS: An ischemia-reperfusion model was used to compare the CIT and ECP groups. This model includes a 6-hour ischemia period followed by a

Equestrian perniosis associated with cold agglutinins: a novel finding.

Reports of equestrian perniosis are rare in the literature and in the cases previously described there have been no abnormal laboratory investigations. We describe two patients with equestrian perniosis who had persistently elevated titres of cold agglutinins. We discuss the relationship of these cold agglutinins to the pathogenesis of perniosis and other related skin disorders.

Oxygenation of the Transplanted Kidney.

Although kidney oxygen tensions are heterogenous, and mostly below renal vein level, the nephron is highly dependent on aerobic metabolism for active tubular transport. This renders the kidney particularly susceptible to hypoxia, which is considered a main characteristic and driver of acute and chronic kidney injury, albeit the evidence supporting this assumption is not entirely conclusive. Kidney transplants are exposed to several conditions that may interfere with the balance between oxygen supply and consumption, and enhance hypoxia and hypoxic injury. These include conditions leading to an

Influence of cold water immersion on limb and cutaneous blood flow at rest.

BACKGROUND: Cold water immersion reduces exercise-induced muscle damage. Benefits may partly arise from a decline in limb blood flow; however, no study has comprehensively investigated the influence of different degrees of cooling undertaken via cold water immersion on limb blood flow responses. PURPOSE: To determine the influence of cold (8°C) and cool (22°C) water immersion on lower limb and cutaneous blood flow. STUDY DESIGN: Controlled laboratory study. METHODS: Nine men were placed in a semireclined position and lowered into 8°C or 22°C water to the iliac crest for two 5-minute periods

Effects of pre-ganglionic decentralization or post-ganglionic excision of the superior cervical ganglia on brain edema and heat stroke in rats.

The preventive effect of pre-ganglionic decentralization (Sympathetic trunk resectioN) or postganglionic excision (ganglionectomy) of the superior cervical ganglia on thermal injury induced brain edema or the development of heat stroke was assessed in rats. Brain edema was induced by cold or heat injury to the cortex in 24 rats. The results showed that decentralization, but not excision, of the superior cervical ganglia greatly inhibited the formation of brain edema which was subsequently induced. When heat stroke was induced by exposing 24 rats to an ambient temperature of 41 degree C, the la

Oxygen persufflation as adjunct in liver preservation (OPAL): study protocol for a randomized controlled trial.

BACKGROUND: Early graft dysfunction due to preservation/reperfusion injury represents a dramatic event after liver transplantation. Enhancement of donor organ criteria, in order to cope with the ever increasing donor shortage, further increases graft susceptibility to ischemic alterations. Major parts of post-preservation injury, however, occur at the time of warm reperfusion but not during ischemic storage; successful reperfusion of ischemic tissue in turn depends on an adequate redox and intracellular signal homeostasis. The latter has been shown experimentally to be favorably influenced by

Pain during ice water test distinguishes clinical bladder hypersensitivity from overactivity disorders.

BACKGROUND: The Bladder cooling reflex (BCR) i.e. uninhibited detrusor contractions evoked by intravesical instillation of cold saline, is a segmental reflex believed to be triggered by menthol sensitive cold receptors in the bladder wall, with the afferent signals transmitted by C fibres. The BCR is a neonatal reflex that becomes suppressed by descending signals from higher centres at approximately the time when the child gains full voluntary control of voiding. It re-emerges in adults with neurogenic detrusor overactivity as a consequence of loss of central descending inhibition, resulting f

[Acute replantation after facial amputation injuries].

Amputation injuries to the face and scalp, although uncommon, can have devastating consequences. Microvascular replantation of the amputated tissue in the facial or scalp region often yields the best functional and aesthetic outcomes. It is crucial to promptly assess amputation injuries and contact a reconstructive plastic surgery service to discuss the possibility of microvascular replantation. While awaiting surgery, the amputated tissue should be wrapped in moist gauze, placed in a sealed plastic bag, and stored in an ice water bath or a refrigerator. In the literature, the success rate for

[Return of sensitivity after digital nerve reconstruction in children: how does age affect outcome?].

BACKGROUND: The regenerative capacity after nerve reconstruction in children is believed to be superior compared to that in adults. However, the available data on this topic are limited. The aim of this work is to improve the age-dependent assessment of the prognosis after surgical treatment of peripheral nerve injuries of the hand. PATIENTS AND METHODS: 44 of 147 children with complete transections of proper and common digital nerves who were treated from 2000 to 2009 and who were currently 6 years or older, could be included for follow-up (mean time, 7.5 years). In total there were 60 nerve

Local neurological complication during carotid endarterectomy.

In a prospective study of 52 carotid endarterectomies there were 13 temporary cranial nerve injuries in 12 patients (25%); 7 hypoglossal nerve injuries (13.5%), 3 were asymptomatic (5.8%), 2 had mild speech difficulty (3.8%) and 2 had severe difficulty with deglutition and articulation (3.8%). A full recovery was made within 3 months in all 7 patients. Although 10 patients (19.2%) developed a hoarse voice, indirect laryngoscopy only indicated damage to the ipsilateral recurrent laryngeal nerve in 3 patients (5.8%). Both the voice and vocal cord movement returned to normal within 6 months in 2

Panniculitis in children.

The panniculitides include a group of disorders of varied etiology that manifest as inflamed nodules in the subcutaneous tissue. They are rarely seen in infants and children. The panniculitides of the newborn represent a unique response of the infant's fat to different injuries, and are a specific type of panniculitis that is only seen in neonates and very young infants. These specific panniculitides of children include subcutaneous fat of the newborn, poststeroid panniculitis, sclerema neonatorum, and cold panniculitis. This article reviews in detail the specific types of panniculitis of the

Material basis and mechanism of Ephedra sinica in interfering with wind-chill cold.

Ephedra sinica has a long history in medicine, i.e., as the first medicinal plant for treating wind-chill colds. However, few studies have shown its material basis and mechanism of action, so this article mainly focuses on these two issues. Rats were stimulated by fan blowing and low temperature for 7 days, following which alkaloid extract (SWJ), non-alkaloid extract (FSWJ), and polysaccharide extract (DT) from Ephedra sinica (MH) were administered for 4 days. First, the body temperature, sweat spots, activity status, and cough-related indexes were detected to screen the pharmacodynamic materi

Keep your attention closer to the ureters: Ureterolysis in deep endometriosis surgery.

Endometriosis surgery involving the ureter poses significant challenges requiring meticulous surgical techniques and vigilant postoperative care. This chapter addresses key aspects of ureterolysis techniques, intraoperative management of ureteral injuries, and postoperative care in the context of endometriosis surgery. Ureterolysis methods aim to isolate and mobilize the ureter while preserving its vascularity. Cold instruments and careful dissection are recommended to prevent thermal injury during surgery. Intraoperative tools such as indocyanine green (ICG) show promise in assessing for vasc

Radial nerve injury causes long-lasting forelimb sensory impairment and motor dysfunction in rats.

INTRODUCTION: Peripheral nerve injury is a common cause of lifelong disability in the United States. Although the etiology varies, most traumatic nerve injuries occur in the upper limb and include damage to the radial nerve. In conjunction with the well-described effects of peripheral damage, nerve injuries are accompanied by changes in the central nervous system. A comprehensive understanding of the functional consequences of nerve injury is necessary to develop new therapeutic interventions. OBJECTIVES: We sought to characterize changes in sensory and motor function and central neurophysiol
