Although sarcoid-like reactions are uncommon in colorectal cancer, they can coexist with other cancers. We herein report a 50-year-old woman with sigmoid adenocarcinoma and osseous sarcoid-like reactions that resembled metastases. Positron emission tomography-computed tomography revealed an uptake in the iliac bone and para-aortic lymph nodes. The depth of invasion indicates non-metastatic lesions. Primary aortoenteric fistula is a rare entity with high mortality. Depending on the location of the fistula, primary aortoenteric fistulas may lead to upper or lower gastrointestinal hemorrhages. OBJECTIVE: To demonstrate variable anatomy of inferior mesenteric vein and importance of preoperative vascular examination and imaging.
MATERIAL AND METHODS: We present a middle-aged patient with extremely rare variant of inferior mesenteric vein anatomy.
RESULTS: Surgical procedure was followed by uneventful postoperative period. The patient was followed-up. There were no signs of recurrence.
CONCLUSION: Analysis of imaging is important for preoperative management and minimizing the risk of intraoperative complications associated with variable anatomy.

This case report presents an unusual and challenging case of an 82-year-old female patient who presented with constipation and abdominal pain and was diagnosed with bowel perforation and hydronephrosis caused by an ingested chicken wishbone. This patient was treated with emergency laparotomy and bowel sigmoid resection. She made a good recovery and was discharged home. The patient's clinical presentation, diagnostic challenges, and successful management are discussed.
