Vietnam seeks help expanding voluntary surgical contraception.

Recent surveys by the Vietnamese Ministry of Health suggest that 60% of married women desire no more children. Yet only 2% of currently married women and less than 1/2 of 1% of their partners use sterilization. Underscoring the high unmet need for effective family planning, over 1 million abortions (legal in Vietnam for the past 20 years) are performed annually. This rate corresponds to 1 abortion for every live birth. The Ministry of Health has recently welcomed a variety of organizations, including AVSC, whose assistance can help expand the country's family planning programs. Sorely lacking

A comprehensive review on post-wildfire impacts in California: environmental exposure, health risks, and resilience strategies.

California's wildfires have intensified in frequency and severity, driven by climate change, prolonged droughts, and historical fire suppression policies. This PRISMA-guided systematic review synthesises empirical research from 2007 to 2024 on post-wildfire environmental exposures, health risks, and resilience strategies in California. Database searches (Scopus, Web of Science, PubMed) identified 47 peer-reviewed studies that met the inclusion criteria. Empirical evidence suggests that wildfire-derived smoke, characterised by increased concentrations of fine particulate matter (PM2.5), volatil

Devastating "DANA" Floods in Valencia: Insights on Resilience, Challenges, and Strategies Addressing Future Disasters.

BACKGROUND: The unprecedented DANA floods that struck Valencia, Spain on October 29, 2024, caused catastrophic human, economic, and ecological damages, resulting in 223 fatalities, the displacement of 15,000 residents, and long-term health and environmental consequences and incurred financial losses estimated over 50 billion euros. Vulnerabilities in urban planning, ambiguous command and control oversight, delayed warnings, and insufficient preparedness underscored systemic gaps in the flood preparedness and management. ANALYSIS AND POLICY OPTIONS: Urgent actions, including stricter urban zon

Applying CFIR to assess multi-level barriers to PrEP delivery in rural South Africa: Processes, gaps and opportunities for service delivery of current and future PrEP modalities.

Despite established efficacy for oral pre-exposure prophylaxis (PrEP) in reducing HIV incidence, multi-level barriers within the health system, clinics, and the processes that shape practice have hindered service delivery and subsequent population-level effects. We applied the Consolidated Framework for Implementation Research (CFIR) to assess the context of PrEP delivery for adolescent girls and young women (AGYW) in rural South Africa and identify the factors supporting and impeding PrEP implementation to develop strategies to improve PrEP delivery. Between 2021 and 2022, we conducted in-dep

Structural Barriers to HIV Prevention and Services: Perspectives of African American Women in Low-Income Communities.

BACKGROUND: African American women are at a disproportionate HIV risk compared with other U.S. women. Studies show that complex structural and social determinants, rather than individual behaviors, place African American women at greater risk of HIV infection; however, little is known about women's views of what puts them at risk. AIMS: This study sought to comprehend the perceptions of African American women living in low-income housing regarding the factors that influence both their personal sexual health behaviors and use of HIV prevention services. METHODS: We conducted seven focus group

Community-based interventions to support aging in place and functional independence in older adults: a systematic review of randomized controlled trials.

BACKGROUND: As the global population ages, ensuring the autonomy and wellbeing of older adults living at home has become increasingly important. Supporting independence is central to aging in place, which requires access to appropriate health and social resources. In this context, community-based public health interventions may play an important role in addressing physical, cognitive, and psychosocial challenges associated with aging. OBJECTIVE: This systematic review aimed to synthesize evidence from randomized controlled trials examining community-based public health interventions that prom

Consulting the Community on Strategies to Strengthen Social Capital for Community Disease Control.

Excluding communities in planning and implementing research maximizes internal risks that are otherwise visible and avoidable when there is adequate community consultation. Communities might not meaningfully use research results if majority of the researched people have minimal or no participation in information generation and dissemination. However, effective participation of researched communities in research is key to transferring knowledge to action. Using a qualitative approach, the study identified barriers to, and relevant strategies for improving health research uptake, particularly fo

'I told him I'm gonna get it': intimate partner birth control communication among homeless young women.

Homeless youth have disproportionately high rates of unintended pregnancy and STIs. Enhancing communication between sexual partners can improve sexual health outcomes, yet little is known about this topic among homeless youth; therefore, this study aimed to examine how homeless youth communicate with their partners about birth control. In-depth semi-structured interviews regarding intimate partner birth control communication were conducted with 10 homeless young women aged 14-22 years following their completion of a comprehensive sexual health program (Wahine Talk). We transcribed the intervie

HIV/AIDS and the African-American Community 2018: a Decade Call to Action.

HIV/AIDS disproportionately affects African-Americans more than any other racial or ethnic group in the USA. Currently representing only 12% of the US population, African-Americans now comprise close to half of the total reported HIV/AIDS cases in the USA according to the Centers for Disease Control and Prevention since the initial reporting of HIV/AIDS. In this paper, we examined the prevalence and current direction of the HIV/AIDS epidemic in the African-American community especially in comparison to our first call to action in 2008. The situation remains dire and broader attention is necess

Not-For-Profit Hospital Community Benefit: Hospital Prioritization of Opioid Use Disorder and Self-Pay Prices for Medications Prescribed for Opioid Use Disorder.

CONTEXT: Not-for-profit hospitals are required to provide community benefits (CB) in exchange for tax-exempt status, yet CB spending remains the predominant measure for assessing whether hospitals meet these obligations. Newly available hospital price transparency data offer an opportunity to evaluate whether hospitals' stated CB health priorities align with their financial practices. OBJECTIVE: To examine whether not-for-profit hospitals that prioritize opioid use disorder (OUD) in CB planning are more likely to comply with price reporting for medications commonly prescribed for OUD treatmen

Mental healthcare practices from entry to release across Southeastern jails.

BACKGROUND: Individuals with mental illnesses are disproportionately incarcerated in jails, which have become de facto mental health institutions across the US. Yet there is limited research describing mental healthcare practices from entry to release among multiple jails and states. METHODS: We conducted 34 semi-structured interviews with jail healthcare personnel across five Southeastern states and used the Framework Method to guide analyses. RESULTS: We report results on challenges and practices related to mental health staffing, screening, additional evaluations and services, and dischar

Evaluating a tailored communication tool to increase advance care planning among Alaska Native and American Indian people in primary care: A type 1 hybrid effectiveness-implementation cluster randomized trial protocol.

The older Alaska Native/American Indian (AN/AI) population is increasing faster than their U.S. peers overall, yet AN/AI peoples are less likely to receive palliative care, including advance care planning (ACP). ACP aims to align care with patient priorities and can improve health-related quality of life for people with serious illness and their families, including a reduction in psychological distress, physical symptoms, unwanted treatments, and cost. ACP ideally includes documentation of advance directives to guide medical decisions when patients cannot speak for themselves. AN/AI peoples ar

Person-Centred Plans From the Perspective of Persons-Supported in a Community Care Setting: A Qualitative Study.

BACKGROUND: In partnership with PHSS, a community care agency in London, Ontario, we describe the person-centred planning model. Person-centred plans (PCPs) are mandated in the community care sector and created annually through discussions with the person-supported, staff, family/friends and community members. PCPs are individualised, integrated into daily activities and contribute towards larger goals. METHODS: We interviewed 18 persons-supported and one family member. Participants were individuals with developmental, medical, or complex physical needs and supported by PHSS. We explored goal

Development and Uptake of Long-Acting Reversible Contraception Services in Rwanda, 2009-2016.

Background: Long-acting reversible contraception (LARC) is highly effective at preventing pregnancy. However, in sub-Saharan Africa, LARC education for clients is relatively limited and providers are often not skilled in their insertion. Before 2009, only 1% of family planning clients in Rwanda received an LARC. Materials and Methods: We trained Rwandan government clinic nurses to promote, insert, and remove copper intrauterine devices (IUDs) and hormonal implants. Training started in two large urban clinics, and those nurses trained three successive waves of clinic nurses. Initial LARC promot
