Monostotic fronto-orbital fibrous dysplasia with convulsion--case report.

A 28-year-old man presented with monostotic fronto-orbital fibrous dysplasia associated with convulsions. Signs of meningeal irritation were observed. Computed tomography (CT) showed right frontal sinusitis, and destruction from the inner to outer table with expansion of the diploic space. T1- and T2-weighted magnetic resonance imaging showed an abnormal low-intensity mass, with heterogeneous gadolinium enhancement. Although the meningitis resolved, signs of infection continued for 2 months due to sinusitis. Treatment of the right frontal sinusitis was undertaken, accompanied by open biopsy. T

[Familial febrile convulsions is supposed to link to human chromosome 19p13.3].

OBJECTIVE: To localize the familial febrile convulsion (FC) genes on human chromosomes. METHOD: For 63 FC pedigrees, tetranucleotide repeat markers D19S253 D19S395 and D19S591 on the short arm of chromosome 19, as well as dinucleotide repeat markers D8S84 and D8S85 on the long arm of chromosome 8 were genotyped. Transmission disequilibrium test (TDT) and Lod score calculation were carried out. The data were processed by PPAP software package. RESULTS: All the alleles in every locus of FC probands and normal controls were in Hardy-Weinburg balance. Transmission disequilibrium was found on D8S

The Louisiana Medicare Electro-Convulsive Therapy Quality Improvement Project.

The first collaborative statewide psychiatric quality improvement project, initiated by Louisiana Health Care Review, Inc, focused on the practice of inpatient electro-convulsive therapy (ECT) among Medicare patients. Data from the project indicated that the majority of Medicare patients receiving ECT were white females living at home, in good physical health, in their seventies, and with a diagnosis of major depression or major depression with psychotic features. The quality indicators for the project included: (documentation of) monitoring of cognitive side effects, monitoring of depressive

[Non-convulsive paroxysmal disorders in exogenous-organic diseases of the brain].

Examination of 273 patients with exogenous-organic diseases of the brain revealed nonconvulsive paroxysmal disorders of traumatic, toxic, infectious, radioactive and combined origin in 112 cases (41.0%). Such disorders were characterised by pronounced polymorphism and presented with viscero-vegetative (36.6%), affective (27.7%), psychosensory (19.6%), sensory (15.2%), ideatoric (11.6%) paroxysms as well as with twilight states of consciousness (16.1%), absence seizures (10.7%), narcolepsy (2.7%), catalepcy (1.8%) and the states of "déjà vu" and "jamais vu" (5.4%). In most of the patients such

[Intractable vomiting, convulsions and megaloblastic anemia: anamnesis, key to diagnosis].

In July 1996 a 43-year-old illiterate Hispanic woman presented with uncontrollable vomiting, palpitations and confusion. In 1994, despite several hospitalisations in other medical centres where a cerebral CT-scan, oesogastroduodenoscopy, colonoscopy and abdominal ultrasound were performed, no satisfactory diagnosis could be found. A psychiatric origin was finally considered. On admission, the laboratory findings showed severe metabolic alkalosis with associated hypokalaemia, confirmatory evidence of vomiting. The ECG showed tremendous P waves (5 mV) in the standard derivations, which can be ex

Mapping of cerebral metabolic activation in three models of cholinergic convulsions.

Glucose utilization of four cerebral cortex and 35 subcortical regions (CGU) was analyzed in three models of cholinergic seizures induced by the following compounds: 1) soman (pinacolylmethylphosphonofluoridate) an organophosphorus cholinesterase inhibitor, 100 microg/kg SC after pretreatment with pyridostigmine 26 microg/kg IM (n = 6); 2) physostigmine, a carbamate cholinesterase inhibitor, 1.31 mg/kg infused IV over 75 min (n = 6); and 3) pilocarpine, a direct cholinergic agonist, 30 mg/kg SC (n = 6). Physostigmine and pilocarpine were preceded by 3 mmol/kg LiCl IP 20 hrs earlier. Animals in

Epilepsy diagnosis and localization in patients with antecedent childhood febrile convulsions.

PURPOSE: A history of febrile convulsions (FC) is often obtained in patients presenting for surgical treatment of temporal lobe epilepsy (TLE), but it is not clear that preferential temporal localization of epilepsy is associated with antecedent FC. METHODS: We prospectively inquired about FC and their characteristics in all patients presenting to an epilepsy clinic through a patient questionnaire and interview. We studied the incidence of antecedent childhood febrile convulsions in relation to epilepsy diagnosis. RESULTS: FC were reported by 133 of 1005 study patients (13.2%). TLE was more

Long-term intellectual and behavioral outcomes of children with febrile convulsions.

BACKGROUND: Hospital-based studies have reported that children with febrile convulsions have subsequent mental retardation and behavior problems. In contrast, population-based studies have reported a better outcome. METHODS: We identified 398 children with febrile convulsions among 14,676 children enrolled in the Child Health and Education Study, a national population-based study in the United Kingdom of children born in one week in April 1970. The children were comprehensively assessed at the age of 10. After excluding 16 children who had neurodevelopmental problems before their first febril

[The clinical study of the first febrile convulsion in children with brain-damage].

Forty-nine patients with cerebral palsy, mental retardation, or other congenital neurological disorders who had experienced febrile convulsions and had no previous nonfebrile seizures were presented. They were followed for 1.6 years to 15 years (mean: 6.8 years) after the initial febrile convulsion. The incidence of subsequent epilepsy (two or more afebrile seizures) was 39%, and 80% of them developed epilepsy within 2 years after the first febrile convulsion. The paroxysmal discharges on EEG recorded prior to or after the first febrile convulsion did not predict the occurrence of later epilep

[A 83 year-old woman with dementia, gait disturbance, and convulsion].

We report a 83 year-old woman with dementia. She was apparently well until December of 1993 when she was 81-year-old. At that time, she was operated or her cataract. Her post operative course was uneventful, however, shortly after her operation, she had an onset of memory loss and abnormal behavior. She showed a fluctuating course in her mental disturbance. In 1995, her dementia worsened with nocturnal agitation. She was admitted to our service on June 12, 1995. She was alert and her blood pressure was 140/100 mmHg. She showed recent memory loss and disorientation to time. Motor wise, she was

The role of vasopressin, somatostatin and GABA in febrile convulsion in rat pups.

In order to further elucidate a possible role of neuropeptides and GABA in the pathogenesis of febrile convulsions, we studied changes of immunoreactive-arginine vasopressin (IR-AVP), IR-somatostatin (IR-SRIF) and gamma-aminobutyric acid (GABA) in the rat brain after febrile convulsions induced by ultra-red light (UR). Male Wistar rats at 16 days of age irradiated with UR developed generalized convulsions after 4.9 +/- 0.5 min irradiation. Six rats were killed by microwave irradiation 3 min after UR irradiation prior to convulsion development, and 29 rats were killed either 0 min, 2 h, 6 h, 24

Cerebrospinal fluid purine metabolites after complex febrile convulsions.

Adenosine monophosphate, inosine monophosphate, inosine, adenosine, guanosine, adenine, guanine, hypoxanthine, xanthine and uric acid were determined in cerebrospinal fluid (CSF) of 15 children after complex febrile seizures (CFS) and in 27 after simple febrile seizures (SFS), and compared with those in a control group of 63 children. There was no statistically significant difference between the groups for any of these metabolites, suggesting that CFS and SFS neither significantly disturb the metabolism of nucleotides, nucleosides or bases nor significantly deplete neuron adenosine triphosphat

Convulsions in childhood shigellosis and antimicrobial resistance patterns of shigella isolates.

Drug resistance patterns of 68 shigella strains were investigated prospectively in Kayseri during a period of approximately two years. The resistance was highest with ampicillin (58.8%) followed by co-trimoxazole (50%) and ampicillin-sulbactam (13%). Only 2.8 percent of cases were resistant to gentamicin, and all serogroups were sensitive to ceftriaxone. We conclude that in children with severe shigellosis, treatment with ceftriaxone is effective and better than ampicillin and co-trimoxazole for obtaining a clinical cure. We followed 18 children who experienced convulsions associated with shig

Corticosteroids enhance convulsion susceptibility via central mineralocorticoid receptors.

Recently, interest in the roles of central nervous system mineralocorticoid receptors (MR) and glucocorticoid receptors (GR) has increased. … h increased severity of ethanol, pentobarbital, and diazepam withdrawal. Further work with chemical convulsants suggests that MR mediate excitatory effects of corticosteroids on convulsion susceptibility. The circadian rhythm in convulsion susceptibility varies with the circadian rhythm of plasma corticosterone levels and MR binding. The types of convulsions …

Hyponatremia, convulsions and neuroleptic malignant syndrome in a male with schizoaffective disorder.

OBJECTIVE: The aim of the study was to highlight the diagnostic difficulties occurring when hyponatremia presents concurrently with neuroleptic malignant syndrome (NMS). Diagnostic issues in the differentiation between hyponatremia and NMS are discussed. CLINICAL PICTURE: A case of dilutional hyponatremia (DH) secondary to psychogenic polydipsia (PP) associated with NMS occurring in a male with schizoaffective disorder is described. The clinical picture was complicated by convulsions, perhaps due to clonazepam withdrawal and possible lithium toxicity. TREATMENT: The patient was treated with

[Therapeutic approach to non-convulsive status epilepticus in developmental period].

The episodes of non-convulsive status epilepticus in children are more difficult to discover and to classify than in adult patients. Using ictal clinical and EEG criteria during continous monitoring of 28 patients, aged from 3 to 16 years, 59 episodes of non-convulsive status epilepticus were classified into typical (8), atypical absence status (27) and complex partial status (24). The rational emergent antiepileptic treatment was based on the findings during clinical and EEG monitoring. Whenever necessary, benzodiazepine sensitivity test was applied in order to help the differentiation betwee

Molybdenum-cofactor deficiency: an easily missed cause of neonatal convulsions.

Intractable seizures in the neonatal period may be caused by molybdenum-cofactor deficiency, an inborn error which combines the deficiencies of sulphite oxidase and xanthine dehydrogenase. The neurological symptoms of molybdenum cofactor and isolated sulphite oxidase deficiencies are identical. Two new cases are reported, and the literature on neonatal convulsions due to molybdenum-cofactor and sulphite deficiencies is reviewed. Because of the high incidence of neonatal convulsions a search for this deficiency is advocated in each case of unexplained refractory neonatal convulsions. Diagnosis
