Prolonged jaundice in newborns is associated with low antioxidant capacity in breast milk.

In breastfeeding newborns who are otherwise healthy, the mechanism of prolonged jaundice remains unclear. The aim of this study was to inves … tiology of the jaundice included complete blood count, peripheral blood smear, blood typing, direct Coombs test, measurement of serum levels of total and direct bilirubin, tests for liver and thyroid function (TSH, free T4, total T4), urine culture and measurement of urine reducing substances, and determination of glucose 6 phosphate dehydrogenase enzyme leve …

Adverse reactions to drugs: relationship to immunopathic disease.

Immunopathic disease resulting from drug treatment occurs when drugs interact with lymphoid cells and induce an immunological reaction. Drug … ons exemplified by reactions to procaine amide (lupus erythematosus) and alpha methyldopa (positive Coombs test result). Drug reactions must be interpreted in terms of modern immunology, with involvement of both the B and T lymphocyte systems. Inherited predisposition exists, probably dependent both on immune response genes and on the rate of enzymatic handli …

Investigation of a neonate with blocked D phenomenon: resolving D type using serologic and molecular methods.

This article addresses the diagnostic challenges encountered and resolution of a neonate with blocked D phenomenon, where maternal anti-D had masked the D sites on the neonate's red blood cells (RBCs), initially resulting in false-negative D typing. This article emphasizes the importance of integrating serologic and molecular methods for accurate blood type determination, which is crucial for managing hemolytic disease of the fetus and newborn. A mother, whose RBCs typed as D- and with a history of D isoimmunization, gave birth to a neonate who presented with anemia and jaundice. Initial D typ

Features of serum complement C3 and C4 levels in autoimmune hemolytic anemia patients.

INTRODUCTION: Abnormally activated complement system induces erythrolysis in a part of autoimmune hemolytic anemia (AIHA) patients. However, the alterations in serum complement levels in these patients are seldom reported. In this study, we aimed to evaluate the serum complement features of AIHA patients according to different clinical and laboratory characteristics and to find relationships between complement levels and hemolysis-associated laboratory indexes. METHODS: A retrospective analysis of 146 AIHA patients was performed, and serum complement C3 and C4 levels were compared between con

An infant with acute brucellosis presenting with Coombs-positive autoimmune hemolytic anemia: is breastfeeding guilty for transmission?

Brucellosis is a zoonotic infectious disease that can be transmitted to humans through infected milk and dairy products. There are limited cases with Brucella infection acquired via breastfeeding in infants in the literature. Also, Coombs-positive autoimmune hemolytic anemia as a result of the disease is comparatively rare when considering the other frequent hematologic complications. We report a mother who acquired the infection as a result of consuming infected milk and dairy products after delivery and of her 5-month-old baby, who had acquired the disease via breastfeeding and presented wit

Hemolytic anemia after kidney transplantation: case report and differential diagnosis.

A 58-year-old woman presented with hemolysis and thrombocytopenia 2 weeks after receiving a kidney graft. Hemolytic uremic syndrome was initially suspected, because in addition to hematological changes the graft function was missing. Unexpectedly, the results of the direct antiglobulin test became positive (4+), which is not normally observed in the hemolytic uremic syndrome. Differentiation of the eluted antibodies revealed anti-rhesus D specificity, which had to be interpreted either as an autoantibody of patient's origin or, hypothetically, as a "graft versus host" antibody of donor origin.

Coombs-positive giant cell hepatitis of infancy: effect of steroids and azathioprine therapy.

An 8-month-old boy and a 7-month-old girl presented with an acute, Coombs-positive auto-immune haemolytic anaemia and severe hepatitis. The clinical manifestations were pallor, jaundice and hepatomegaly. The liver histology revealed diffuse giant cell transformation and extensive necrosis with central-portal bridging. Combined immunosuppressive regimen with steroids and azathioprine led to prolonged clinical and biological remission with a respective 2 years and 7 months follow up. The girl, however, after 7 months developed a progressive encephalopathy of unknown aetiology, while liver and ha

[Prediction of the severity of fetal erythroblastosis using the erythrocyte phagocytosis test and evaluating anti-Rh antibody titers and their levels by autoanalysis].

Erythrophagocytosis++ test was performed in 45 pregnant Rh-negative women together with routine anti-Rh antibodies assay with indirect antiglobulin test and their concentration determined in an autoanalyser. It was found that the degree of ++phagocytosis of the standard erythrocytes immunized with maternal anti-Rh antibodies by the monocytes from healthy subjects is an important predictive factor enabling to foresee a severity of hemolytic disease in newborn babies. ++Phagocytosis over 50% predicts a severe course of the disease whereas under 20%--a mild form of the serologic conflict.

Severe delayed hemolytic transfusion reaction complicating an ABO-incompatible bone marrow transplantation.

A 26-year-old, blood group O bone marrow transplant recipient experienced a severe, delayed hemolytic transfusion reaction 6 days following transplantation of marrow from his HLA-mixed lymphocyte culture - identical, blood group AB sister. The patient's pretransplant serum contained both anti-A (IgG titer = 1:128; IgM = 1:32) and anti-B (IgG = 1:16; IgM = 1:64) which was reduced by a two-plasma volume plasma exchange followed by transfusion of four units of incompatible, donor type red cells. The patient experienced no immediate adverse reaction. On the 6th posttransplant day, he became acutel

A LISS spin enzyme method for the detection of red cell antibodies and its use in routine antibody screen procedures.

A technique for performing the enzyme phase of the antibody screen on red cells suspended in low-ionic-strength-salt solution (LISS) is described. The reliability of this LISS spin-enzyme (LSE) technique was compared with the two-stage papain-tile method, in the detection of 62 previously identified enzyme reacting antibodies. All 62 antibodies examined were detected by the LSE method, and no false-positive reactions were found. Using LSE and papain-tile methods in parallel, further assessment was obtained by screening 2000 sequential blood samples under routine service conditions. Fifty-six b

Urinary tract infection in term neonates with prolonged jaundice.

INTRODUCTION: The aim of this study was to evaluate the frequency of urinary tract infection (UTI) in neonates with prolonged jaundice. MATERIALS AND METHODS: Newborn infants with jaundice lasted more than 2 weeks were included in this study. Patients who had other signs or symptoms were excluded. Workup of prolonged hyperbilirubinemia was performed, including direct Coomb's test, blood group of the neonate and the mother, complete blood count, blood smear, glucose-6-phosphate dehydrogenase (G6PD), reticulocyte count, serum level of bilirubin (unconjugated and conjugated), thyroid function te

OBJECTIVE: To determine clinical and clinicopathologic features of a chronic intermittent severe hemolytic anemia characterized by erythrocy … tosis, mild hyperbilirubinemia, and high hepatic enzyme activities were common findings. Results of Coombs tests and tests for infectious diseases were negative. The erythrocytic osmotic fragility was high in affected cats (mean osmotic fragility, 0.66 to 0.78%), compared with healthy cats (0.48 to 0.58). No specific membrane protein abnormality, erythrocyte …
