Blood blister-like aneurysms of the internal carotid artery are uncommon lesions that often present with subarachnoid hemorrhage and are considered part of the spectrum of intracranial arterial dissections. Although endovascular treatment options have expanded in recent years, serious complications remain a clinical concern. We report the case of a 42-year-old woman with a ruptured blood blister-like aneurysm arising from the anterior wall of the internal carotid artery who was treated with stent-assisted coil embolization. The initial postoperative course was uneventful and follow-up imaging

Acute dissection of the ascending aorta is a life-threatening condition in which the aortic wall develops one or more tears of the intima associated with intramural rupture of the media layer with subsequent formation of a two lumina vessel. The remaining outer layer is just the adventitia, with high risk of complete rupture. Vital organs may be under-perfused. Mortality rate in this acute event is about 50% if an emergent surgical procedure is not performed as soon as possible to replace the tract affected by the primary rupture. Nevertheless, the emergent surgical procedure is affected by hi

We present a 66-year-old undergoing ostial-to-mid right coronary artery (RCA) percutaneous coronary intervention (PCI) lesion proximal to an existing stent in the mid-to-distal segment. The patient underwent PCI, with intravascular imaging using the Novasight Hybrid IVUS OCT (Conavi, Toronto, CA) which revealed a proximal edge dissection and distal dissection involving the media on OCT, less discernable on IVUS, which required an additional distal stent. Proximally, based on OCT minimal luminal area (MLA) measurements of the dissection, the size of the dissection flap and lack of involvement o

BackgroundIn this study, we sought to evaluate the early outcomes and aortic remodeling effects of the Frozenix Partial-ET in total arch replacement for acute type A aortic dissection, and its potential to reduce distal stent graft-induced new entry (dSINE).MethodsWe retrospectively analyzed 47 patients who underwent total arch replacement with a frozen elephant trunk (FET) between January 2017 and December 2024, among whom, 34 received the conventional Frozenix (Full-FET group), and 13 received the Frozenix Partial-ET (Partial-ET group). Clinical outcomes, CT-based morphological parameters, a

BACKGROUND: To prevent distal stent graft-induced new entry (SINE) after thoracic endovascular aortic repair (TEVAR) for double-barrel Stanford type B aortic dissection (TBAD), we adopted the PETTICOAT-Snowshoe technique. In this technique, a proximal stent graft is deployed, followed by the extension of a bare stent above the celiac artery and deployment of a second stent graft within the bare stent. This study aimed to evaluate its mid-term outcomes and feasibility in preventing distal SINE.
METHODS: This single-center retrospective study included patients who underwent the PETTICOAT-Snowsh

Carotid dissection is a major complication of blunt head and neck trauma. The authors describe a case report of sudden bleeding of a distal branch of the external left carotid artery caused by a fracture of the mandible. This event occurred a week after the trauma. After a first phase consisting in cardiopulmonary resuscitation and blood loss replacement, the placement of a Guglielmi detachable coil (GDC) was considered the best and safest way to stop the bleeding. Guglielmi detachable coil represents a good technique for emergency treatment of bleeding from distal carotid vessels. Given its h

As a treatment of rectal cancer, lateral lymph node dissection (LLND) is still a controversial issue. The argument against LLND is that the procedure is complicated, and consequently results in a high incidence of postoperative urogenital dysfunction. The surgical modality from fascia to space is adopted by lateral lymph node dissection in "two spaces". This operation has significant advantages of clear location of nerves and blood vessels and simplified surgical procedures, so the surgical procedure can be repeated and modulated. The fascia propria of the rectum, urogenital fascia, vesicohypo

Purpose To determine whether baseline clinical characteristics and cardiac MRI parameters predict major adverse cardiac events (MACEs) in participants with angiographically confirmed spontaneous coronary artery dissection (SCAD). Materials and Methods This prospective single-center study included participants with SCAD who were angiographically diagnosed between March 2018 and November 2023. Cardiac MRI was performed after a median of 4 days (IQR, 1.5-7) from symptom onset. SCAD types were classified according to the Yip-Saw system. Follow-up evaluation was performed at a median of 5.5 months

Spontaneous coronary artery dissection (SCAD) is a rare acute coronary artery event; however, it should always be considered in pregnant and postpartum patients as it can be life-threatening if untreated. Diagnosis of pregnancy-associated spontaneous coronary artery dissection (PSCAD) can be challenging when pathophysiological lines are blurred by changes related to pregnancy or concurrent obstetric processes such as pre-eclampsia or haemolysis elevated liver enzymes and low platelets. It has been reported that PSCAD has an increased incidence in women with pre-eclampsia, possibly due to their

During the past 4 years, from January 1991 to December 1994, we experienced 12 cases of aortic root reconstruction utilizing Carrel patch method. The cases included annulo-aortic ectasia (AAE), root aneurysm with aortic regurgitation (AR), and aortic dissection with AR. The aortic valve and aortic root were resected, the coronary ostia were dissected free, mobilized, and then implanted to the composite graft. Coronary anastmosis was performed by mattress suture and reinforced by Tefron felt strips. It was necessary in one case to undergo coronary artery bypass grafting for myocardial ischemia

OBJECTIVE: Management of aortic arch disease (AAD) is complex due to its involvement in multiple aortic segments and the variety of patient presentations. We compared the clinical outcomes of conventional aortic arch repair (CAAR) and the Frozen Elephant Trunk (FET) technique across the main clinical indications: acute type A aortic dissection (ATAAD), chronic dissection and atheromatous aneurysm.
MATERIALS AND METHODS: We conducted a retrospective, single-centre cohort study of 102 consecutive patients who underwent aortic arch repair between January 2012 and June 2023. Indication-specific s

Recently, several genome-wide association studies identified PHACTR1 as key locus for five diverse vascular disorders: coronary artery disease, migraine, fibromuscular dysplasia, cervical artery dissection and hypertension. Although these represent significant risk factors or comorbidities for ischemic stroke, PHACTR1 role in brain small vessel ischemic disease and ischemic stroke most important survival mechanism, such as the recruitment of brain collateral arteries like posterior communicating arteries (PcomAs), remains unknown. Therefore, we applied exome and genome sequencing in a multi-et

BACKGROUND: Ipsilateral mydriasis is known to accompany signs of cerebral ischemia in unilaterally compromised carotid blood flow. Mydriasis as the presenting sign of common carotid artery (CCA) dissection has not been reported thus far.
CASE DESCRIPTION: We report the case of a patient who presented with a mydriatic pupil after intraoperative injury of the ipsilateral CCA. Mydriasis preceded complete third-nerve palsy and symptoms of cerebral ischemia for 12 hours. Cerebral angiography revealed occlusion of the CCA with slow collateral flow to the internal carotid artery and fetal origin of

INTRODUCTION AND IMPORTANCE: Takayasu arteritis (TA) is a granulomatous, large vessel inflammation characterized by occlusion, stenosis or aneurysm of the aorta and its branches. Aortic dissection (AD) is one of the rare manifestations of TA, which can present as uncontrolled hypertension. This case illustrates the severe rare cardiovascular complication that arises in a 21-year male patient with TA, including hypertensive urgency, AD and significant renal artery involvement.
CASE PRESENTATION: A 21-year-old male presented with complaints of fever, malaise and dizziness. On clinical examinati

Introduction With the rising trends in breast cancer throughout the world, the traditional modus of intraoperative tissue dissection using a scalpel, scissors, or electrocautery needs to be re-evaluated in the wake of newer modalities, such as electrothermal bipolar vessel sealing (EBVS) devices, which may theoretically reduce the postoperative complications and morbidity in these patients. Aim and objective The objective of this study is to compare an EBVS device to electrocautery (diathermy) in modified radical mastectomy (MRM), based on selected intraoperative and postoperative outcomes.
