Bare Metal Stenting for Residual Arch Dissections: A Computational Analysis.

PURPOSE: Type A Thoracic Aortic Dissections are a highly morbid and complex clinical challenge often managed with hemiarch or total arch repair. Hemiarch repair is more commonly performed due to improved neurologic morbidity profile however it leaves behind a residual dissection flap which can lead to aneurysmal degeneration. Bare metal stent placement in conjunction with hemiarch repair is a novel technique which can theoretically avoid leaving a residual dissection flap. In this paper we analyze the biomechanical changes observed after in silico deployment of a bare metal stent in a post-hem

The Fbn1 gene variant governs passive ascending aortic mechanics in the mgΔlpn mouse model of Marfan syndrome when superimposed to perlecan haploinsufficiency.

INTRODUCTION: Ascending thoracic aortic aneurysms arise from pathological tissue remodeling that leads to abnormal wall dilation and increases the risk of fatal dissection/rupture. Large variability in disease manifestations across family members who carry a causative genetic variant for thoracic aortic aneurysms suggests that genetic modifiers may exacerbate clinical outcomes. Decreased perlecan expression in the aorta of mgΔlpn mice with severe Marfan syndrome phenotype advocates for exploring perlecan-encoding Hspg2 as a candidate modifier gene. METHODS: To determine the effect of concurre

A novel organ culture model of aorta for vascular calcification.

Vascular calcification is a characteristic feature of aging, atherosclerosis, diabetes mellitus, and end-stage renal disease. The use of organ culture provides complementary information that may bridge the gap between traditional cell culture and animal models, and establishes easily controlled experimental conditions. Therefore, we investigated whether organ culture of the aorta could be used as a model of vascular calcification, applying it to animal models of other conditions. Thoracic aortas were dissected from C57BL/6 mice and cultured. To induce vascular calcification, stimulation with i

Identification of three particular morphological phenotypes in sporadic thoracic aortic aneurysm: phenotype III as sporadic thoracic aortic aneurysm biomarker in aged individuals.

Aging has a striking impact on the heart and the vascular system, and particularly on the large elastic arteries (i.e., aorta), resulting in a multitude of changes at different structural and functional levels. As result, medial degeneration (MD) occurs. A characteristic example of MD is sporadic thoracic aortic aneurysm (S-TAA), whose patho-physiological mechanisms remain unclear. In this study, typical MD morphological phenotypes were researched in S-TAA cases and control aorta specimens using histopathological and mainly immunohistochemical analyses. Three phenotypes (I, II, and III) were d

The distal aorta in the Marfan syndrome.

Prolonged survival of patients with Marfan syndrome after aortic root replacement has led to an increased number of patients with aortic complications beyond the root. Elective replacement of the aortic root removes the most important predilection site for aneurysms, but the distal aorta remains at risk. Predictors for aortic growth and adverse events in the distal aorta include aortic diameter, aortic distensiblity, previous aortic root replacement, hypertension and aortic regurgitation. After aortic dissection, the initial false lumen diameter is an independent predictor for late aneurysm fo

Re-interventions on the thoracic and thoracoabdominal aorta in patients with Marfan syndrome.

The advent of multi-gene panel genetic testing and the discovery of new syndromic and non-syndromic forms of connective tissue disorders have established thoracic aortic aneurysms as a genetically mediated disease. Surgical results in patients with Marfan syndrome (MFS) provide an important benchmark for this patient population. Prophylactic aortic root surgery prevents acute dissection and has contributed to the improved survival of MFS patients. In the majority of patients, re-interventions are driven by a history of dissection. Patients undergoing elective root repair have a low risk for re

Case of isolated thoracic aortic aneurysm as a manifestation of undiscovered giant cell arteritis.

A 73-year-old woman was referred to our hospital to investigate dilatation of an aortic arch which had been detected by a chest roentgenogram and severe aortic valve regurgitation detected by echocardiography. On admission, a computed tomography scan of the chest showed a large fusiform ascending aortic aneurysm. She had not shown any symptoms such as headache or polymyalgia rheumatica and had no significant coronary atherosclerosis. She underwent aneurysmectomy and reconstruction of the ascending aorta using cardiopulmonary bypass without aortic valve replacement, and pathological examination

Single graft "peninsula-style" transverse aortic arch replacement in patients with type A acute aortic dissection: case report.

Acute type A aortic dissection, despite treatment advances, remains a critical emergency with markedly high morbidity and mortality rates. The primary goals of immediate surgical intervention are to ensure survival, prevent severe complications, and avoid subsequent interventions. We present a case of a 55-year-old male who presented with new-onset chest pain, dyspnea, and hypotension. Emergent transthoracic echocardiography and subsequent computed tomography revealed an ascending aortic dissection. The patient underwent immediate surgical repair using a "peninsula-style" technique for transve

Acute type A aortic dissection involving the left main coronary artery and the aortic valve--a case report.

The authors report a case study of a 60 year-old-male admitted to the Coronary Care Unit with severe chest pain, hypertension and ST depression on the anterior and lateral leads of the electrocardiogram. A diastolic murmur was heard in the aortic area. The chest X-ray showed an enlarged superior mediastinum. The transthoracic echocardiogram revealed an image that indicated an intimal flap above the aortic plane with severe aortic regurgitation. An aortography was performed and confirmed the diagnosis of acute type A aortic dissection with partial obstruction of the left main coronary artery, p

Aortic disease and interventions in adults with tetralogy of Fallot.

BACKGROUND: Aortic dilation and aortic valve disease are known long-term complication of tetralogy of Fallot (TOF), but the risk of aortic dissection and the indications for prophylactic aortic surgery are unknown in this population. The purpose of this study was to: (1) determine the prevalence of significant aortic valve disease and/or significant aortic aneurysm (AVD-AA); (2) determine the incidence of progressive aortic dilation and aortic dissection in patients with TOF. METHODS: Retrospective review of adults with repaired TOF, and no prior aortic valve/aorta surgery, who had ≥2 measure

Clamshell thoracosternotomy for single-stage repair of select aortic arch pathologies.

OBJECTIVE: Although median sternotomy is widely used for aortic arch repair, the distal extent of arch replacement is limited with this approach. Bilateral thoracosternotomy (clamshell) represents an alternate and underappreciated strategy that allows for single-stage repair of the aortic arch and descending thoracic aorta. We report our institutional experience with this approach. METHODS: Patients who underwent clamshell thoracosternotomy for aortic arch surgery from 2005 to 2022 were identified from a prospectively maintained institutional aortic surgery database. The primary outcomes were

Graft replacement of the descending thoracic aorta: results of "open" distal anastomosis.

With the advent of rapid autotransfusion, we began to repair aneurysms of the descending thoracic and thoracoabdominal aorta by using an "open" technique, in which a single cross-clamp is placed proximal to the aneurysm to exsanguinate the lower body. To determine whether open distal anastomosis effectively protects against spinal cord injury, we studied 71 consecutive patients (50 men, 21 women) who underwent this procedure beginning in April 1989. The patients ranged in age from 31 to 83 years (mean, 63.3 years). Most patients were hypertensive (n = 61; 86%) and symptomatic (n = 54; 76%). Mo

Genetics of Thoracic and Thoracoabdominal Aortic Dissections and Aneurysms.

Acute aortic dissection is a life-threatening event that requires immediate medical attention and surgical intervention. Aortic dissections affect 4 to 5 per 10,000 individuals in the USA. These emergency events can have as high as a 50% mortality risk. Twenty percent of patients who develop a thoracic aortic dissection have an underlying genetic cause for their increased risk for aortic aneurysm and dissection, which can be identified using genetic testing. Genes associated with abdominal aortic aneurysm and dissection have been identified but have not been reported for clinical use or manage

Hypogastric Artery Occlusion with Evoked Potentials Monitoring as Bailout Technique to Assess the Risk of Postoperative Spinal Cord Ischemia.

A 65-year-old man, with previous open surgical repair of an infrarenal abdominal aortic, presented with acute complicated (paraplegia) Type B aortic dissection. He successfully underwent endovascular repair of the descending thoracic and abdominal aorta. Following the procedure, the neurological manifestations resolved. As he had a concomitant aneurysm of the right hypogastric artery (HGA), we executed a 10-minute balloon occlusion of this artery with evoked potential measurements to assess the risk of spinal cord ischemia after exclusion of the right HGA. The examination was interpreted as ne
