Endocrine cells of the duodenal bulb producing adrenalin, noradrenalin, serotonin, histamine and beta-endorphine were assayed in 12 patients with preulcer and 39 patients with duodenal ulcer. It is shown that preulcer was mainly characterized by hyperplasia of endocrine cells in the duodenal bulb mucosa, secreting adrenalin and noradrenalin in the presence of the deficiency of beta-endorphine-producing cells. The recurrent duodenal ulcer was associated with a dramatic fall in the number of catecholamine-containing endocrine cells in the duodenal bulb.

Bleeding duodenal varices are a rare complication of portal hypertension. Upper gastrointestinal bleeding from other sources than varices is common in patients with cirrhosis. Most often the bleeding is misinterpreted as bleeding from duodenal ulcer, and the diagnosis is first apparent during emergency operation. From our experience with a case of bleeding duodenal varices we suggest that the diagnostic procedure of choice is percutaneous transhepatic portography.

Lymphatic malformations are benign tumors rarely located in the gastro-intestinal tract. They are usually asymptomatic; however, they can cause clinical discomfort when enlarged. Imaging leads to diagnosis, but the confirmation can only be done through histology findings. We report the case of a 59-year-old male patient, admitted in our ward for melena, to which esophago-gastroduodenoscopy showed budding formations in the second portion of the duodenum wall, with Magnetic Resonance Imaging features and histological examination confirming diagnosis of a duodenal lymphatic malformation.

BACKGROUND AND STUDY AIMS: Duodenal obstruction may prevent performance of endoscopic retrograde cholangiopancreatography (ERCP). Percutaneous transhepatic biliary drainage (PTBD) or Endoscopic ultrasonograhy-guided biliary access (EUS-BD) are alternative treatments but are associated with a higher morbidity and mortality rate. The aim of the study is to report overall technical success rate and clinical outcome with deployment of temporary fully or partially covered self-expanding duodenal stent (pc/fcSEMS) as a bridge to ERCP in case of inaccessible papilla due to duodenal strictures.

Duodeno-duodenal intussusception is often caused by an intraluminal tumour. The condition is rare owing to the retroperitoneal fixation of the duodenum, which is sometime absent in cases of intestinal malrotation. We describe the case of a 19-year old man admitted to hospital with abdominal pain and vomiting. A CT scan revealed a duodeno-duodenal intussusception including the head of the pancreas, which was confirmed by laparotomy. The cause was found to be a duodenal membrane with a pinhole passage combined with non-rotation of the duodenum.

It was proposed the method of organ-preserving surgical treatment of chronic ulcers of back wall of duodenal bulb complicated by decompensated stenosis and penetration in intrapancreatic part of common bile duct (RF patent number 2476164 from 27.02.2013). 12 patients were operated by using of this technique. One patient had post-operative pancreatitis. There were not lethal outcomes.

Causes of the incompetence of stump sutures in resection of the stomach observed in 90 out of 3479 patients with chronic duodenal ulcers were analyzed. Most patients were found to have a combination of general and local factors. Among the local factors the leading one was appearance of a large inflammatory infiltration around the "active" ulcers. The endoscopic autohemoapplication to the ulcer included in the complex of preoperative preparation of the patients reduced the frequency of incompetence of sutures from 2.58% to 1.33%.

38 patients with complicated duodenal ulcer were examined with polarographic method. The significant decrease of tissue blood flow was revealed in all segments of gastroduodenal zone. The increased level of free-radical oxidation of lipids was determined in duodenal mucosa and ulcer edges. Disorders of blood supply and activation of free-radical lipid oxidation play an important role in the mechanism of ulcer formation. The drugs improving blood supply and antioxidants should be used in combined therapy and preoperative preparation.
