[Clinical phenotypes of classic Rett syndrome].

INTRODUCTION: Rett syndrome (RS) is a progressive neurological disorder that is diagnosed by essential, supportive and exclusion clinical criteria, and development takes place in four stages. It has been shown to be caused by de novo mutations of a gene located in the long arm of the dominant X chromosome that codes for the methyl CpG binding protein (MECP2). It has been observed that girls with classic RS (CRS) present distinguishing nuances with respect to the age of onset of the different criteria and as regards the progression of the disorder. Taking the ability or failure to walk as a ref

Novel therapies for mucopolysaccharidosis type III.

Mucopolysaccharidosis type III (MPS III) or Sanfilippo disease is an orphan inherited lysosomal storage disease and one of the most common MPS subtypes. The classical presentation is an infantile-onset neurodegenerative disease characterised by intellectual regression, behavioural and sleep disturbances, loss of ambulation, and early death. Unlike other MPS, no disease-modifying therapy has yet been approved. Here, we review the numerous approaches of curative therapy developed for MPS III from historical ineffective haematopoietic stem cell transplantation and substrate reduction therapy to t

The early diagnosis and management of mixed delirium in a patient placed on ECMO and with difficult sedation: A case report.

Delirium represents a serious problem that impacts the physical and cognitive prognosis of patients admitted to intensive care units and requires prompt diagnosis and management. This article describes the case and progress of a patient placed on Extracorporeal Membrane Oxygenation with difficult sedation criteria and an early diagnosis of mixed delirium. During the case report, we reflect on the pharmacological and non-pharmacological strategies employed to cope with delirium paying special attention to the non-use of physical restraint measures in order to preserve vital support devices (end

Bilateral Early Knee Osteoarthritis Treated With Unilateral Proximal Tibial Osteotomy And Contralateral Non-Surgical Technique.

Some non-surgical techniques that decrease the mechanical load of the knee may be effective in treating early primary knee osteoarthritis. Thirtysix consecutive patients with bilateral early primary knee osteoarthritis and genu varum were treated with unilateral proximal tibial osteotomy in the more degenerated knee. After the osteotomized bone healed, ambulation with protected weight bearing using a cane in the contralateral hand was advised continuously for at least three months or until knee pain subsided bilaterally. Thirty-one patients were followed for an average of 4.6 years (range, 2.1

Ultrasound guided proximal adductor canal and pes anserinus blocks improve early recovery after arthroscopic ACL reconstruction.

To explore the impact of ultrasound-guided proximal adductor canal and pes anserinus tendon block on early recovery after anterior cruciate ligament reconstruction surgery via daytime knee arthroscopy. A total of 127 patients, aged 18-60 years, with ASA class I-II, undergoing anterior cruciate ligament reconstruction via knee arthroscopy under general anesthesia with laryngeal mask airway intubation, were selected. These patients were randomly divided into three groups: Group C (43 cases), Group N1 (41 cases), and Group N2 (43 cases). Control group C: received proximal block of the adductor ca

Propensity score-matched comparison of unicompartmental and total knee arthroplasty in older patients: Lower invasiveness and faster early recovery after UKA.

BACKGROUND: Unicompartmental knee arthroplasty (UKA) may offer lower surgical invasiveness and faster recovery than total knee arthroplasty (TKA), but comparative studies in older patients remain limited by selection bias. We therefore asked whether, in propensity score-matched older patients, UKA would (1) reduce perioperative invasiveness, (2) improve early postoperative recovery, (3) maintain comparable short-term patient-reported outcomes, and (4) show stable radiographic outcomes compared with TKA. HYPOTHESIS: UKA would be associated with lower surgical invasiveness and faster early post

Optimal, Early Postoperative Management of Cardiac Transplant and Durable Left Ventricular Assist Recipients.

PURPOSE OF REVIEW: Summarize developments in the early postoperative care of patients undergoing cardiac transplantation or left ventricular assist device implantation. Provide a practical approach with personal insights to highly complex patients at risk for prolonged hospitalization. RECENT FINDINGS: Advancements in technology allow for percutaneous mechanical circulatory support of both the right and left ventricles either isolated or combined via subclavian and neck vessels. Since the adult heart allocation system has been changed to reduce waitlist mortality, the use of temporary mechani

Strategies to overcome challenges of transradial coronary angiography and intervention.

The transradial approach has become the preferred route for performing coronary angiography and interventions. Several studies reported that radial access is associated with significant reduction in vascular complications compared with the femoral access. This technique allows also early ambulation, improves the patient's well-being, and is less expensive. One important limitation of radial access is that coronary engagement from transradial approach is more challenging than transfemoral approach. The increased susceptibility of the radial artery to spasm, the radial-brachial artery tortuositi…

Stasis ulcers: update on a common geriatric problem.

The impact of chronic venous insufficiency on the activity and well-being of the elderly patient can be devastating. Acute complications such as cellulitis and venous ulcers often require extended hospitalization at a great cost to both patient and society. However, it is the long-term loss of ambulation that has the most profound effect on the elderly. It is clear that despite hundreds of medical papers the optimal management still has not been delineated. In general, early intervention to improve the hemodynamics before irreversible damage occurs is most beneficial. It is the responsibility

Femoral fractures in patients with lower extremity amputations.

Femoral fracture in patients with lower extremity amputations is relatively rare (reported incidence in 341 amputee patients, 2.35%). Lack of adequate support and balance, and osteoporosis on the amputation side, may explain the predisposition to fracture of the ipsilateral extremity. The prosthesis was in use at the time of the accident in only 3 of 9 cases studied, most falls occurring during transfer activity without a prosthesis. Proximal femoral fractures in below-knee amputee patients responded to open reduction with internal fixation and early ambulation. Others were treated with closed…

A multifaceted approach to prevention of thromboembolism: a report of 529 cases.

The morbidity and incidence of thromboembolic complications can be reduced by patient awareness, nursing staff concern, and physician responsibility using mechanical measures and drugs. Preoperative exercise instrutions, early ambulation, calf exercise, antiembolism hose, postoperative circle-bed turning and use of the Trendelenburg position, the use of intravenous dextran postoperatively and, in selected cases, low doses of sodium warfarin (Coumadin) form the foundation of our approach to the prophylaxis of thromboembolic phenomena. In the presence of thrombophlebitis, phenylbutazone (Butazol…

Altered states of consciousness in disorders of daytime sleepiness.

Patients with daytime sleepiness present altered states of consciousness. The occurrence of these states impairs their professional, social and familial activities and may threaten life itself. The automatic behavior syndrome is characterized by continuation of mechanical activity and complete amnesia. Episodes lasting from a few seconds to several hours are correlated with repetitive micro-sleep periods. During cataplectic attacks, patients may have a meshing of reality with hallucinatory dream contents. Sleep-induced apnea may lead to abnormal movement and abnormal ambulation during sleep as

[Anesthesia and delivery].

Close collaboration of an informed anaesthetist with the obstetrician, and respect of the security protocols in every anaesthesia must guarantee the well-being of pregnant women. The development of loco-regional anaesthesia for use in labour analgesia and caesarean section has reduced the indications and the mortality related to general anaesthesia. In the last 30 years, analgesia and control of loco-regional anaesthesia effects have been greatly improved. Loco-regional anaesthesia protocols are now safe for the mother and various. Epidural anaesthesia using low concentrations of local anaesth

Autosomal dominant familial spastic paraplegia: report of a large New England family.

A large New England family with autosomal dominant familial spastic paraplegia is described. In a pedigree of 173 individuals, 71 affected individuals are identified. Seventeen cases examined by the authors are described with regard to the natural history of FSP in this family. A staging system for following progress and planning interventions is proposed. Three illustrative cases are presented. In this family, FSP is found to have a homogeneous clinical course with nearly complete penetrance. Onset occurs at or before 3 years of age with involvement limited to the lower extremities. After the

Pressure recordings in patients with femoral fractures in cast-braces and suggestions for treatment.

Five patients with femoral-shaft fractures were treated by cast-brace application and early ambulation. As fracture healing progressed, pressures between the thigh and the cast were recorded with the patients lying still, contracting the quadriceps while lying down, and in four weight-bearing postures. Control results were gained by application of a second cast-brace to three of the patients when they had recovered completely. Resting pressures averaging sixteen millimeters of mercury were recorded under the whole cast. Much higher values were recorded under the proximal two-thirds of the cast…

Anaesthesia for laparoscopic surgery.

Laparoscopic surgery offers patients a significant number of advantages, including a reduction in pain and cosmetic injury that facilitates early ambulation and a rapid return to normal activities. Although the surgery is of a minimally invasive nature, there are a number of specific physiological alterations occurring as a result of creating the pneumoperitoneum and the postural changes involved in optimizing patient position. These may be particularly in patients with pre-existing respiratory and cardiovascular disease. In addition, there may be surgical hazards related to trocar insertion o…

The Head Injury Transportation Straight to Neurosurgery (HITS-NS) randomised trial: a feasibility study.

BACKGROUND: Reconfiguration of trauma services, with direct transport of traumatic brain injury (TBI) patients to neuroscience centres (NCs), bypassing non-specialist acute hospitals (NSAHs), could potentially improve outcomes. However, delays in stabilisation of airway, breathing and circulation (ABC) and the difficulties in reliably identifying TBI at scene may make this practice deleterious compared with selective secondary transfer from nearest NSAH to NC. National Institute for Health and Care Excellence guidance and systematic reviews suggested equipoise and poor-quality evidence - with
