Ebstein Anomaly Fetal Echocardiogram Parameters are Associated with Illness Severity on Admission to the Cardiac Intensive Care Unit.

BACKGROUND: Ebstein anomaly is associated with a broad spectrum of presentations. Many fetal factors have been associated with intrauterine fetal demise and early neonatal mortality. Little is described about how fetal factors may influence illness severity in the immediate postnatal period. OBJECTIVES: To identify the relationship between fetal echocardiogram measures and severity of illness on admission utilizing pSOFA scores. METHODS: Retrospective chart review of 32 neonates with prenatally diagnosed Ebstein anomaly. Fetal echocardiographic factors assessed included right ventricular press…

Exercise Performance and Right Ventricular Remodeling After the Cone Repair in Patients With Different Severity of Ebstein Anomaly.

BACKGROUND: Although the cone repair has shown good outcome regarding valve competence, its effect on exercise performance and right ventricular remodeling are unclear. METHODS: Between January 2010 and June 2021, 60 patients underwent a cone repair at our center. We excluded patients with previous operation (n = 6), age <5 years (n = 3), in-hospital death (n = 2), replacement before discharge (n = 1), and missing follow-up (n = 2), resulting in a study population of 46 patients. Median age was 27 years (interquartile range, 5.6-68.2 years), and 28 (61%) were female. Patients were divided bas

SUCCESSFUL ABLATION OF CONCEALED ACCESSORY PATHWAY MEDIATED SUPRAVENTRICULAR TACHYCARDIA IN A 5-YEAR-OLD CHILD WITH EBSTEIN ANOMALY.

Here we report a case of a 5-year-old boy with Ebstein anomaly and supraventricular tachycardia. He was diagnosed with Ebstein anomaly at the age of 3 months during the workup for afebrile convulsions, and has been followed by a pediatric cardiologist since. Electrocardiography recorded a small Rr` pattern in V1 on a few occasions, without other abnormalities. At the age of 2.5 years, the first episode of supraventricular tachycardia was recorded and stopped with adenosine. He was started on atenolol and was asymptomatic until a year later when he presented with the second recorded supraventri…

Right Ventricular Electromechanical Dyssynchrony and Its Relation to Right Ventricular Remodeling, Dysfunction, and Exercise Capacity in Ebstein Anomaly.

BACKGROUND: Abnormal atrioventricular and intraventricular electrical conduction and dysfunction of the functional right ventricle (fRV) are common in Ebstein anomaly (EA). However, fRV mechanical dyssynchrony and its relation to fRV function are poorly characterized. We evaluated fRV mechanical dyssynchrony in EA patients in relation to fRV remodeling, dysfunction, and exercise intolerance. METHODS: We retrospectively analyzed data from nonoperated EA patients and age-matched controls who underwent echocardiography, cardiovascular magnetic resonance imaging, and cardiopulmonary exercise test…

Tailored strategy to match anatomy and physiology with intervention can improve outcomes of symptomatic neonates with Ebstein anomaly.

OBJECTIVE: Neonatal presentation of Ebstein anomaly (EA) represents the most severe form of this condition. Despite significant advances, operative mortality remains high and management decisions represent a formidable challenge. We used a strategy aimed to match anatomy and physiology with type and time of intervention to increase survival. METHODS: We performed a review of all patients with fetal or neonatal diagnosis of EA managed at a single center between 2007 and 2020. RESULTS: Among 18 patients with EA, 8 underwent neonatal intervention. The most common indication included cyanosis an…

Strain and Strain Rate To Evaluate Right Heart Function of Ebstein Anomaly (EA) Patients Before and After Operation.

OBJECTIVE: This study was aimed to elucidate the feasibility of using right ventricular (RV) strain and strain rate to evaluate right heart function of Ebstein anomaly (EA) patients before and after operation. METHODS: Sixty EA patients and 30 healthy controls underwent echocardiography (UCG) for evaluation of right heart function. Preoperative UCG and 1-week and 3-month postoperative UCG were performed in EA patients. RV strain and strain rate were measured on the four-chamber section of tissue Doppler imaging (TDI). RESULTS: The strain and strain rate representative of right ventricle syst…

Severity Scores for Ebstein Anomaly: Credibility and Usefulness of Echocardiographic vs Magnetic Resonance Assessments of the Celermajer Index.

BACKGROUND: The severity score of Ebstein anomaly (EA) that corresponds to clinical status is still under research, with the Celermajer index (Cel-ind) being one of those. The agreement between echocardiographic and cardiac magnetic resonance (CMR) assessment of Cel-ind is not known. We determined the agreement between echocardiography- and CMR-derived Cel-ind and its relationship with heart failure markers. METHODS: A total of 37 unoperated adults with EA (mean age, 43.0 ± 14.4 years) underwent echocardiography, CMR, and cardiopulmonary tests. For the Cel-ind, end-diastolic areas in echocard…

Prevention preferable to treatment: 3 case reports of patients experiencing right-sided heart failure after Ebstein anomaly correction.

RATIONALE: Ebstein anomaly is a common congenital heart disease that may induce severe tricuspid regurgitation and dilation of the "atrialized" portion of the right ventricle. Patients who undergo surgery to correct Ebstein anomaly are at high risk of postoperative right-sided heart failure, yet little is known about what pre-, peri-, or postoperative procedures may help reduce this risk. PATIENT CONCERNS: Here, we describe 3 cases of adults with Ebstein anomaly who underwent corrective surgery and in whom right-sided heart failure occurred with severe tricuspid regurgitation detected by tran…

Total 3d endoscopic approach to complete repair of Ebstein's anomaly: a case series.

BACKGROUND: Ebstein anomaly is a rare congenital malformation of the tricuspid valve. Although robotic-assisted minimally invasive repair has shown promising results, its high cost limits accessibility in developing countries. We report our initial experience with a totally endoscopic cone repair using three-dimensional (3D) visualization and standard thoracoscopic instruments, performed without robotic assistance, for the treatment of Ebstein anomaly. METHODS: This retrospective case series included five adult patients who underwent total 3D endoscopic tricuspid valve cone reconstruction at E…

Leadless AV Synchronous Pacing in a Patient With Ebstein's Anomaly and Prior Glenn Shunt.

BACKGROUND: Leadless pacing offers an important alternative to transvenous systems in patients with complex congenital heart disease and limited venous access. CASE SUMMARY: We report the case of a 29-year-old woman with Ebstein anomaly, hypoplastic right ventricle, prior bidirectional Glenn shunt, multiple septal defect repairs, and transcatheter pulmonic valve replacement who presented with symptomatic paroxysmal atrioventricular block. Given preserved biventricular function and complex right-sided anatomy, a leadless pacing strategy was selected. Using transesophageal echocardiography, 3-d…

Atriofascicular pathway in Ebstein's anomaly: Revisiting the role of preoperative EP studies in pediatrics.

We present a case of a 6-year-old with Ebstein's anomaly (EA) who was found to have a right lateral decremental, antegrade only accessory pathway (AP), presumed atriofascicular AP, during a pre-operative electrophysiology study (EPS) prior to da Silva Cone repair (CR). Atriofascicular APs represent <3 % of APs but have an association with EA. To the best of our knowledge, this is the youngest reported case of EA with an atriofascicular AP. Providers should be aware of this possibility, as CRs are being done at younger ages, and they may be discovered in asymptomatic patients.

Fetal circular shunt in Ebstein's anomaly and non-steroidal anti-inflammatory treatment.

A circular shunt is a poor prognostic factor associated with Ebstein's anomaly. Targeting the constriction of the ductus arteriosus (DA) in order to limit or resolve the circular shunt, has been shown to improve fetal outcomes. Prenatal non-steroidal anti-inflammatory drugs (NSAIDs) have been known to constrict the DA. Recently, prenatal NSAIDs have been used for that purpose in the treatment of circular shunt. Limited research shows that it may be an effective treatment leading to improved fetal outcomes. In this article, we did an extensive review of literature to describe this therapy's eff

Intraoperative Transesophageal Echocardiography Evaluation of Tricuspid Valve Re-repair for Ebstein's Anomaly.

Postoperative stenosis or regurgitation of the tricuspid valve is common and affects the prognosis after repair surgery of Ebstein's anomaly. However, it is unclear how intraoperative echocardiography influences the postoperative course. We report a longitudinal echocardiography course including intraoperative transesophageal echocardiography in a cone reconstruction procedure for Ebstein's anomaly in a 17-year-old woman. Tight tricuspid valvuloplasty was preferred, but the tricuspid annulus enlarged rather after surgery. The evaluation of the tricuspid valve form and function using intraopera

Ebstein's Anomaly: From Fetus to Adult-Literature Review and Pathway for Patient Care.

Ebstein's anomaly, first described in 1866 by Dr William Ebstein, accounts for 0.3-0.5% of congenital heart defects and represents 40% of congenital tricuspid valve abnormalities. Ebstein's anomaly affects the development of the tricuspid valve with widely varying morphology and, therefore, clinical presentation. Associated congenital cardiac lesions tend to be found more often in younger patients and may even be the reason for presentation. Presentation can vary from the most extreme form in fetal life, to asymptomatic diagnosis late in adult life. The most symptomatic patients need intensive

Ebstein's Anomaly: An Unanticipated Differential Diagnosis of Egg-on-Stick Appearance and the Diagnostic Dilemma.

BACKGROUND Ebstein's anomaly (EA) is a rare congenital cardiac abnormality with diverse anatomic and spectra of clinical presentations. This heart anomaly occurs in approximately 1 per 200 000 live births and accounts for <1% of congenital cardiac diseases. The main pathologic finding is "atrialization" of the basal region of the right ventricle. EA has been described extensively in the literature; however, to the best of our knowledge, this is the first reported case of EA presenting with "egg-on-stick" appearance. CASE REPORT A 42-year-old woman presented on account of 2 years of progressive

A case of capillary malformation-arteriovenous malformation and Ebstein's anomaly in a child with EphB4 mutation.

Capillary malformation-arteriovenous malformation (CM-AVM) is a rare condition characterized by multiple cutaneous capillary malformations with potential associated arteriovenous malformations. RAS p21 protein activator 1 (RASA1) and ephrin type-B receptor 4 (EPHB4) genes are implicated. We present a child with CM-AVM, due to EPHB4 mutation, and Ebstein's anomaly. Although EPHB4 is a known effector of vascular remodeling, its contribution to cardiogenesis is still being explored. Further research is needed to determine causality of Ebstein's anomaly in the setting of CM-AVM due to EPHB4 mutati
