Intraoperative arthroscopic classification tool for posterolateral elbow instability.

BACKGROUND: Introducing and implementing an arthroscopic classification tool for posterolateral elbow instability. METHODS: Thirty arthroscopies were performed on 30 patients, and all recordings were collected, blinded, and labeled. Three orthopedic surgeons reviewed and scored all 30 recordings three times with a period of at least seven days in between to analyze the intraobserver and interobserver reliability. The classification consisted of five different grades. RESULTS: Indications for elbow arthroscopy included impingement (n = 7), osteochondritis dissecans (n = 5), pain (n = 7), oste…

Treatment Options for Tennis Elbow - An Umbrella Review.

INTRODUCTION: The main goal of the present umbrella review was to provide the most up-to- date and evidence-based results regarding the various treatment options for tennis elbow (TE), which hopefully will significantly decrease the confusions existing in the literature. Furthermore, our study differs from past analytical studies because, as to the best of the authors' knowledge, is the first to provide independent (not in comparison to other treatment) statistical results regarding the effectiveness of each TE treatment. MATERIALS AND METHODS: Major medical databases such as PubMed, Scopus, …

[MEDIAL EPICONDYLITIS (GOLFER'S ELBOW) - CLINICAL PRESENTATION AND TREATMENT].

Medial epicondylitis, which is also called "Golfer's Elbow" is a pathology which typically presents as medial elbow pain. It affects 1% of the general population, yet affects 3.8% to 8.2% of work-related complaints. Golfer's elbow is common in the 40 to 60-year-old age group so those suffering from it are part of the workforce and hence, its economic impact. Women and men alike suffer from golfer's elbow, and microtrauma combined with attritional changes in the common flexor tendon origin at the medial aspect of the elbow are the culprit of this pathology. The first line of treatment is conser…

The Van Gorder approach for total elbow arthroplasty.

BACKGROUND: Surgical management of the triceps during exposure for total elbow arthroplasty (TEA) is critical to a successful outcome. Previously described techniques include elevating the triceps insertion from one side or leaving the triceps insertion attached and dislocating the joint. Another approach to the elbow, first described in 1933 by Willis Campbell, MD, and subsequently modified by George Van Gorder, MD, involves turning down the triceps tendon without disrupting the triceps insertion. This approach offers complete visualization of the joint and provides excellent exposure for TEA…

Sonication improves microbiologic diagnosis of periprosthetic elbow infection.

BACKGROUND: Periprosthetic joint infection (PJI) is a relatively frequent and oftentimes devastating complication after total elbow arthroplasty (TEA). Its microbiologic diagnosis is usually based on periprosthetic tissue culture (hereafter referred to as tissue culture), but the sensitivity of tissue culture is variable. Although implant sonication culture has been shown to be superior to tissue culture for the diagnosis of hip and knee PJI, only a single small study (of fewer than 10 infected implants) has assessed sonication for PJI diagnosis after elbow arthroplasty. METHODS: We retrospec…

Effects of elbow immobilization on upper extremity activity.

BACKGROUND: It has been dogma that handedness greatly impacts daily activities. Interruptions in the ability to utilize the dominant arm due to neuromusculoskeletal injuries could negatively impact performance of activities of daily living. Daily activity can be measured using activity monitors. This study aimed to examine how arm dominance impacts function by immobilizing the arms of healthy individuals. METHODS: Ten individuals wore four upper extremity activity monitors for three days-one day without immobilization, one day with their dominant arm immobilized, and one day with their non-do

Bilateral Collateral Ligament Reconstruction for Chronic Elbow Dislocation.

We report the case of a 37-year-old male with autism spectrum disorder who was hospitalized for chronic elbow dislocation. He had suffered a posterior elbow dislocation 5 months ago. His elbow dislocation was easily reduced, but the reduction position could not be maintained. Severe varus and valgus instabilities were observed in his right elbow. He was diagnosed with chronic elbow dislocation due to bilateral collateral ligament dysfunction and was surgically treated. Bilateral ligament reconstruction using the bilateral palmaris longus (PL) tendon combined with a temporary ulnohumeral joint …

Treatment of complete ankylosed elbow with total arthroplasty.

We reported the case of a 38-year-old patient who in 2017 presented to our institution after post-traumatic complete ankylosis of the elbow. He complained of stiffness and pain, and the radiographs revealed complete fusion of the humeroulnar joint. The Mayo Elbow Performance Score (MEPS) on admission was 31, and the elbow was fused at 90° flexed position. The patient underwent semiconstrained total elbow arthroplasty with Bryan-Morrey approach; after a 2.1 years follow-up, active arc of movement was 120°. The MEPS was excellent with a 100 points score, the patient was able to complete all acti…

Neglected Iatrogenic Elbow Joint Dislocation. Didactic Case Report.

We present a case of a neglected dislocation of the elbow joint. A 28-year-old patient suffered elbow joint dislocation with fracture of the radial head after a fall from a considerable height. Following a failed attempt at manual repositioning, the dislocation was reduced surgically and free fragments of the radial head were removed without stabilisation with a Kirschner wire. The joint was immobilised for 6 weeks in an arm-to-hand plaster cast, followed by two weeks during which an elbow joint orthosis with adjustable flexion was worn. The patient then attended rehabilitation. The first foll…

Acute and overuse elbow trauma: radio-orthopaedics overview.

The correct management of acute, subacute and overuse-related elbow pathologies represents a challenging diagnostic and therapeutic problem. While major trauma frequently requires a rapid surgical intervention, subluxation and minor trauma allow taking more time for diagnostics and planning the correct elective treatment after careful clinical and radiological investigation. In these conditions, communication between orthopaedic surgeon and radiologist allow to create a detailed radiology report, tailored to the patient's and surgeon's needs and optimal to plan proper management. Imaging techn…

Risk factors for reoperation after total elbow arthroplasty.

BACKGROUND: Total elbow arthroplasty (TEA) is a treatment option for arthritic conditions of the elbow and for complex distal humerus fractures in the elderly. Complications are common, however, and rates of survivorship vary. The goal of this study was to describe the factors associated with reoperation and revision after TEA. METHODS: We retrospectively reviewed primary TEAs performed at 2 tertiary academic medical centers. We identified 102 primary TEAs in 82 patients by 9 surgeons. The average age of the patients was 61 years. Female patients represented 81% of TEAs performed. The mean fo…

Complex Elbow Dislocations and the "Terrible Triad" Injury.

BACKGROUND: The elbow is the second most commonly dislocated joint in adults and up to 20% of dislocations are associated with a fracture. These injuries can be categorised into groups according to their mechanism and the structures injured. METHODS: This review includes a literature search of the current evidence and personal experiences of the authors in managing these injuries. RESULTS: All injuries are initially managed with closed reduction of the ulno-humeral joint and splinting before clinical examination and radiological evaluation. Dislocations with radial head fractures should be t…

Analysis of complications after a floating elbow injury.

BACKGROUND: The aim of the present study is to analyse complications after a floating elbow injury, attempting to establish which of them act as a poor prognosis factor regarding clinical and functional results. MATERIALS AND METHODS: Twenty-three patients who suffered a floating elbow injury, treated at our institution from 2004 to 2013, were retrospectively reviewed. Patients were divided into four groups depending on the type of injury. An analysis of demographic data, associated injuries, treatment options and complications was carried out. Clinical evaluation was made by a conventional g…

Successful Closed Reduction of a Lateral Elbow Dislocation.

In this report, we present a case of lateral elbow dislocation treated with closed reduction. Lateral elbow dislocation is rare, and a closed reduction is reported with even less frequency. The reduction can be hindered by swelling and soft tissue interposition, and we describe the use of a nonoperative reduction technique performed under mild sedation with early physiotherapy to avoid joint stiffness. No additional complication was observed, and the normal range of elbow movement and function was obtained by early physiotherapy.

Surgical Treatment of Tennis Elbow; Minimal Incision Technique.

BACKGROUND: When non-operative treatment of tennis elbow fails; a surgical procedure can be performed to improve the associated symptoms. Different surgical techniques for treatment of lateral epicondylitis are prescribed. The purpose of this study was to evaluate the clinical outcomes of surgical treatment for tennis elbow based on small incision techniques. METHODS: This technique was performed on 24 consecutive patients between June 2011 and July 2013. Outcomes were assessed using the Patient-Rated Tennis Elbow Evaluation (PRTEE), Nirschl's staging system and visual analog scale (VAS) for …

Triceps-sparing ulnar approach for total elbow arthroplasty.

To prevent insufficiency of the triceps after total elbow arthroplasty, we have, since 2008, used a triceps-sparing ulnar approach. This study evaluates the clinical results and post-operative alignment of the prosthesis using this approach. We reviewed 25 elbows in 23 patients. There were five men and 18 women with a mean age of 69 years (54 to 83). There were 18 elbows with rheumatoid arthritis, six with a fracture or pseudoarthrosis and one elbow with osteoarthritis. Post-operative complications included one intra-operative fracture, one elbow with heterotopic ossification, one transient ul…

Transphyseal elbow fracture in newborn: review of literature.

Transphyseal elbow fracture is a rare entity in newborns, and in the last century, only case reports or small case series have been published; however, precise epidemiological data lack. Such fractures occur more often in emergency Caesarian section or vaginal delivery. The differential diagnosis with elbow dislocation can be challenging. Radiography, arthrography, magnetic resonance, ultrasound or a combination of these have already been described to make diagnosis, but guidelines for the management of this injury in the neonate are not well established. A review of the literature of the last…

"Popeye muscle" morphology in OBPI elbow flexion contracture.

BACKGROUND: The pathophysiology of elbow flexion contracture (EFC) in obstetrical brachial plexus injury (OBPI) is not established. In basic science models, neonatal denervation leads to impaired muscle growth. In clinical studies, diminished growth is correlated with extent of denervation, and improved with surgical repair. In EFC, the biceps are clinically short and round vs the contralateral size, termed the "Popeye muscle". The objective of this study was to determine if the biceps morphology (muscle belly and tendon length) in arms with EFC secondary to OBPI is different vs the contralate…
