BACKGROUND: Anteromedial facet (AMF) fractures of the coronoid process are typically accompanied by tears of the lateral collateral ligament (LCL) and the posterior bundle of the medial collateral ligament (pMCL). Addressing the bone defect and repairing the LCL is essential in the acute setting to prevent persistent varus posteromedial rotatory instability (VPMRI) and subsequent progression to osteoarthritis. However, pMCL repair is not typically performed. Therefore, the present biomechanical study investigates the amount of stress experienced by the pMCL in cases of anteromedial facet defec

BACKGROUND: Displaced intraarticular distal humerus fracture has been conventionally treated operatively with various triceps disrupting approaches. These approaches are associated with several complications, such as triceps weakness, nonunion or delayed union of osteotomy, implant prominence, and delayed mobilization of the elbow. We present the functional outcome of intraarticular distal humerus fracture fixation using a triceps-sparing paratricipital approach which allows early elbow mobilization and preserving triceps strength.
MATERIALS AND METHODS: Twenty five patients with intraarticul

Elbow instability is common and may occur after a variety of injuries, including falls or direct blows. Instability can be classified as either acute or chronic. Acute instability is classified as simple (without fracture) or complex (with associated fracture). Chronic instability is classified as a chronically dislocated or recurrently unstable elbow. Recurrent instability commonly presents as isolated medial or lateral collateral ligament insufficiency. A chronically dislocated elbow is often more complex, involving both osseous and ligamentous injuries. The treatment of simple dislocations

BACKGROUND: Elbow dislocation is frequently associated with bony and osteochondral posterolateral capitellar lesions that are often under-reported. We aim to examine the radiologic signs of posterolateral lesions on computed tomography (CT).
METHODS: A retrospective analysis of CT scans was performed. Patients were classified into 4 groups: (1) simple elbow dislocation, (2) elbow dislocation with a fracture of the coronoid tip, (3) elbow dislocation with coronoid tip fracture and a radial head fracture inferior or equal to the anterior third, and (4) terrible triad defined as elbow dislocatio

BACKGROUND: Recommendations for cast immobilization of acute scaphoid fractures vary substantially. We reviewed data from randomized controlled trials comparing nonoperative treatment methods for acute scaphoid fractures to determine the best available evidence.
METHODS: A systematic search of the medical literature from 1966 to 2010 was performed. Two authors independently screened titles and abstracts, reviewed articles, assessed methodological quality according to the Grading of Recommendations Assessment Development and Evaluation system, and extracted data. The primary outcome parameter

INTRODUCTION: Capitellum fracture (CF) of the distal humerus is an unusual and rare pediatric fracture. CF is not only confined to the capitellum but can extend into the trochlea. Treatment options vary from casting to surgical fixation based on displacement and fracture characteristics.
CASE PRESENTATION: A 13-year-5-month-old boy who presented after a fall on his left elbow was found to have a displaced coronal shear CF extending into the trochlea, type 4 McKee fracture/type 2A Dubberley. An open reduction and internal fixation (ORIF) was performed through a lateral approach using headless

The optimal timing of postoperative mobilization following surgical fixation of distal humerus fractures remains controversial, particularly in complex intra-articular fractures where concerns regarding fixation stability and elbow stiffness influence rehabilitation strategies. Distal humerus fractures are complex injuries requiring meticulous surgical management to achieve optimal outcomes. This prospective study evaluated 31 patients with closed distal humerus fractures (AO Type 13-C) treated with pre-contoured locking plates, assessing functional recovery using the Mayo Elbow Performance Sc

Articular fracture of distal humerus account for 2% of all adult elbow fracture. The coronal shear fracture combined with olecranon fracture require stable anatomical reduction and stabilisation for early rehabilitation and mobility. Numerous reports have described various approaches in tackling this type of fracture and method of fracture fixation. Here, we shared a case of a lady with traumatic coronal shear capitulum and trochlear comminuted fracture fixed using posterior elbow approach with fracture site open window technique in managing distal humerus articular fracture with ipsilateral o

PURPOSE: The purpose of this study was to evaluate the objective and subjective results achieved after double tension band fixation in transverse extra-articular intracapsular supracondylar distal humerus fractures and nonunions in adults.
METHODS: Nine patients presenting six fractures and three nonunions of the distal humerus, treated with double tension band wiring between 1998 and 2011, were retrospectively evaluated. Two fractures were type A2 and four type A3, and the nonunions were oligotrophic; all nine lesions had a supracondylar intracapsular transverse orientation, that passed thro

The treatment of choice for proximal radial head fractures remains controversial. The goal of any treatment for an intra-articular fracture must be the complete restoration of the joint and its function. Nonoperative treatment leads to full motion in cases of less than 1-2 mm of fracture displacement. Resection of the radial head can be recommended only for very comminuted fractures. All other fracture types should be treated by open reduction and internal fixation. Our own personal follow-up observation of 19 patients who had surgical intervention demonstrated restoration of elbow function af

Intraarticular radial head (IARH) fractures are uncommon pediatric injuries with unpredictable yet poor outcomes. The aim of this study was to evaluate clinical outcomes of IARH fractures in pediatric and adolescent patients, by testing our hypothesis that surgically managed fractures would have less risk for an unplanned second surgery and better elbow range of motion at the final follow-up. A retrospective review of 53 IARH fractures was performed. Demographic and clinical data were recorded. Concomitant and associated injuries were documented. Initial management and any attempted reduction

Elbow dislocations are among one of the most frequently dislocated joints, with an incidence of five to six cases annually per 100,000 persons in the United States. The vast majority of elbow dislocations occur posteriorly, secondary to a disruption in the anterior elbow- and posterior elbow-stabilizing structures. Anterior elbow dislocations are rare injuries in both children and adults, occurring as the proximal ulna is forced anterior to the distal humerus with or without the proximal radius. As of 2019, only 21 anterior elbow dislocations without fractures have been reported between 1922 a

Distal humerus fractures are challenging injuries due to their soft tissue envelop, and the complex 3 dimensional anatomy including 3 articular surfaces. Modern fixation principles have allowed for improved outcomes even in previously unreconstructible fracture patterns. Despite these advances bone loss, and articular comminution may require alternative or adjunctive strategies such as grafting, fusion, hemi-arthroplasty and total elbow arthroplasty options.

CASE: An 18-year-old male polytrauma patient sustained a high-energy posterior fracture dislocation of his left elbow associated with a comminuted and irreparable O'Driscoll type 2 subtype 3 anteromedial facet coronoid fracture. He underwent early coronoid reconstruction using ipsilateral olecranon osteoarticular autograft with incorporation of the sublime tubercle attachment of the medial collateral ligament and repair of the lateral ulnar collateral ligament. A 3-year follow-up revealed a functional, painless, congruent, and stable elbow.
CONCLUSION: Early reconstruction of a highly comminu

BACKGROUND: We studied anteromedial varus angulation (VA) in the proximal third of the ulna. The importance of restoration of the anatomical orientation of the ulnar after a proximal fracture is unclear. The purpose of this study was to evaluate the impact of minimal proximal ulna malunion on elbow function after a proximal ulna fracture.
METHODS: We reviewed the follow-up of 60 patients who had undergone open reduction with internal fixation (ORIF) of a proximal fracture of the ulna. Patients were divided into two groups, defined as either more or less than 5° of the difference between the V

Since Speed first treated the radial head fracture with a metal prosthesis in 1941, the purpose of the treatment of radial head fracture with a radial head changed from the prevention of heterotopic ossification to the prevention of proximal migration of the radius and instability of the elbow. The optimal indication is gradually determined as a non-reconstructable radial head fracture with associated injuries that would leave the elbow unstable if the radial head were resected. Types of prosthesis changed from mono-block, bipolar to modular prosthesis. Material of prosthesis changed from acry
