Ménière's disease is a chronic illness characterized by intermittent episodes of severe vertigo, hearing loss, tinnitus, and aural pressure. The spells last from minutes to hours and are often associated with vegetative symptoms. Up to 50% of the patients are affected bilaterally over time. The etiology is unknown and the diagnosis is based on the history and the physical examination. The primary histopathological finding is endolymphatic hydrops. The treatment during spells is symptomatic and the preventive intervention can be separated into a conservative and a destructive regime.

OBJECTIVE: Structural features of the human cochlea may control early lesion formation in endolymphatic hydrops. This process may hinge on three structural features: the flattened spiral shape of the human cochlea, the toroidal configuration of the distended cochlea duct, and the distensibility characteristics of Reissner's membrane. Vertigo and dizziness are the most common complications of cochlear implantation. Data of a multicentric study about clinical aspects and cost utility were collected for vertigo and dizziness: incidence, clinical and treatment were analyzed among 469 adults and children. Results demonstrated that 16% of adults and 3% of children experienced dizziness postoperatively. In a few cases a specific mechanism as perilymphatic fistula was identified. In other cases the mechanism underlying delayed vertigo remains speculative and endolymphatic hydrops was suggested.

We report 2 cases of Cogan's syndrome. One female patient had a typical interstitial keratitis and bilateral hearing impairment, the other had dramatically progressive deafness, combined with failure of the left vestibule and severe conjunctivitis resistant to treatment and otitis externa. The typical Cogan's syndrome with interstitial keratitis developed only a few weeks later. The disease is caused by an auto-immune response. The hearing impairment is mainly due to endolymphatic hydrops. Corticosteroids are the treatment of choice.

Currently there appears to be a resurgence of interest in the measurement of the peripheral-most electrical potentials of the auditory system. These measurements, known collectively as electrocochleography, may be used (1) to assess and monitor patients with Meniere's disease or endolymphatic hydrops, (2) to enhance wave I of the surface recorded ABR, and (3) as a means of monitoring the peripheral auditory structures during surgery. Various recording techniques and electrode types are reviewed and suggestions are provided for response interpretation in several patient populations.

In 38 patients with cochleovestibular dysfunction and no cause other than syphilis (congenital in 15, acquired in 23), there was no characteristic pattern to the dysfunction. Sensorineural hearing loss was present in all (bilateral in 82%, unilateral in 18%), and 42% had episodic rotatory vertigo. Only 24% had the classic triad of symptoms of endolymphatic hydrops. The impairment was less in the first 5 years of hearing loss than in subsequent intervals; the average impairments were comparable in the second through fifth decades.

Since 1974 we have treated 59 patients suffering from unilateral and bilateral (two patients) Morbus Menière with intratympanal application of gentamycin-sulfate. In contrast to earlier attempts using this method, we did not try to destroy the vestibular apparatus, but taking into our considerations the recent findings about the pathophysiological mechanism of Morbus Menière, only to damage the secretory epithelium, thus preventing the endolymphatic hydrops. Releave from symptoms could be obtained in more than 90% of our patients.

The surface ultrastructures of membranous labyrinths in normal and hydropic guinea pig inner ears were studied. In normal specimens differences between Reissner's and saccular membranes were observed. Low frequency sensorineural deafness is a common subtype of idiopathic sudden deafness. Certain patients suffered recurrent attacks without vertigo, much alike Meniere's disease. Few of them developed into definite Meniere's disease during long-term follow-up in many clinical studies. Although the pathophysiology of recurrent low frequency deafness is yet unknown, the desease is considered associated with early state of endolymphatic hydrops or migraine. Otologists shall be aware of its clinical course and careful explanation is necessary at time of initial informed consent.

This investigation was performed to evaluate the efficacy of a new device, the Méniètt, in the treatment of Ménière's disease. The device delivers pressure pulses to the middle ear through a ventilating tube in the tympanic membrane. These pressure changes are conveyed to the inner ear, reducing the endolymphatic hydrops. A quantitative study of primary vestibular neurons was performed in temporal bones of individuals at different ages. The mean numbers of counts decreased beginning in the age group between 50 and 60 years. Neuronal populations from cases with Menière disease and otosclerosis were compared to the respective "normal" values. Endolymphatic hydrops could be excluded as the cause of degeneration of Scarpa's ganglion in Menière' disease. In cases of otosclerosis reduced neuronal counts were found if lesions were large enough to include the cribrose areas.
