Necrotizing enterocolitis (NEC) is a leading cause of death in very preterm born infants. The most severe variant is NEC totalis (NEC-T), where necrosis of the small intestines is so extensive that curative care is often withdrawn. Mortality and NEC-T are difficult to predict before surgery, complicating counseling and decision-making. This study's aim was to identify preoperative risk factors for overall 30-day mortality and NEC-T in preterm born infants with surgical NEC.This multicenter retrospective cohort study included preterm born infants (<35 weeks) surgically treated for NEC between 2

PURPOSE: Necrotizing enterocolitis (NEC) is a life-threatening gastrointestinal disease of prematurity characterized by inflammation, necrosis, and high morbidity. Current therapies are limited, necessitating the development of novel treatments. Mesenchymal stromal cells (MSCs) have shown promise in murine NEC models. Given the anatomical and physiological similarities between premature piglets and human infants, we employed a preterm piglet model to evaluate MSC efficacy. We hypothesized that intraperitoneal MSC administration would reduce intestinal injury in NEC.
METHODS: Preterm piglets w

BackgroundNecrotizing enterocolitis (NEC) remains a significant cause of morbidity and mortality in preterm neonates. Identifying early markers of impaired intestinal perfusion may aid detecting an association with the development of NEC. This study aims to evaluate the role of Doppler ultrasound of the superior mesenteric artery (SMA) and celiac artery (CA) and its association with NEC in preterm neonates.MethodsWe conducted a retrospective, single-center case-control study. Eligible infants were born at <29 weeks' gestation; we excluded those with chromosomal abnormalities, major anomalies,

PURPOSE: Necrotizing enterocolitis (NEC), an inflammatory intestinal disease common in premature infants, has been associated with the development of lung damage. Toll-like receptor 4 has been shown to regulate inflammation in the NEC lungs, however, other important inflammatory mechanisms have not been thoroughly investigated. In addition, we reported that milk-derived exosomes were able to attenuate intestinal injury and inflammation in experimental NEC. This study aims to (i) investigate the role of the NLRP3 inflammasome and NF-κB pathway in regulating lung damage during experimental NEC;

BACKGROUND: Necrotizing enterocolitis (NEC) is a rare, but potentially fatal intestinal inflammatory condition most often arising in premature infants. Infants provided formula are also at greater risk of developing this disease. Although the majority of formula-fed, preterm infants do not develop NEC, up to 30% of infants with the disease do not survive. Thus, identifying additional, currently unrecognized factors, which may predispose a specific infant to NEC development would be a significant clinical advancement. In this regard, we have previously reported that offspring of female or male

Prenatal inflammation is a risk factor for necrotizing enterocolitis (NEC), and it increases intestinal injury in a rat NEC model. We previously showed that maldevelopment of the intestinal microvasculature and lack of vascular endothelial growth factor (VEGF) receptor 2 (VEGFR2) signaling play a role in experimental NEC. However, whether prenatal inflammation affects the intestinal microvasculature remains unknown. In this study, mouse dams were injected intraperitoneally with lipopolysaccharide (LPS) or saline at embryonic day 17. Neonatal intestinal microvasculature density, endothelial cel

INTRODUCTION: Neonates with hypoplastic left heart syndrome (HLHS) are at increased risk for necrotizing enterocolitis (NEC). Initial hospital outcomes are well described, but minimal midterm data exist. Goal of this study was to compare outcomes of HLHS infants with NEC (HLHS-NEC) to HLHS without NEC (HLHS-nNEC) during the interstage period.
METHODS: Data were reviewed from 55 centers using the NPC-QIC database. Case-control study with one HLHS-NEC matched to HLHS-nNEC neonates in a 1:3 ratio based on institutional site, type of surgical repair, and gestational age ±1 week was performed. Bas

OBJECTIVE: We examined the effect of two strategies to prevent necrotizing enterocolitis (NEC) in extremely low birth weight (ELBW) infants-adherence to a standardized feeding protocol and use of a hospital-based milk bank to provide exclusive preterm human milk feedings.
STUDY DESIGN: We conducted a single-center observational study from 2010 to 2015. Infants received preterm human milk, initially trophic feeds from days 7 to 14 after birth, followed by advancement of 15 mL/kg/day to reach a goal of 180 mL/kg/day. Fortification was used selectively for weight gain < 15 g/kg/day. We determine

PURPOSE: Necrotizing enterocolitis (NEC) is a devastating gastrointestinal disease of premature infants. Daikenchuto, a Japanese herbal drug, has several effects on the digestive system, so we investigated its preventive effects in a rat model of NEC.
METHODS: NEC was induced in newborn rats via asphyxia (100% N2 for 90 s; every 4 h) + LPS (4 mg/kg/day [administered orally on days 0 and 1]). The effects of Daikenchuto were evaluated in four groups (control: 0 g/kg/day, I: 0.3 g/kg/day, II: 0.6 g/kg/day, and III: 1.0 g/kg/day). Daikenchuto was administered into the stomach through a microcathe

Preterm infants are susceptible to infection and necrotizing enterocolitis (NEC) and are often treated with antibiotics. Simultaneous administration of enteral and parenteral antibiotics during the first days after preterm birth prevents formula-induced NEC lesions in pigs, but it is unknown which administration route is most effective. We hypothesized that only enteral antibiotics suppress gut bacterial colonization and NEC progression in formula-fed preterm pigs. Caesarean-delivered preterm pigs (90-92% of gestation) were fed increasing amounts of infant formula from birth to day 5 and given

BACKGROUND: Necrotizing enterocolitis (NEC) is associated with changes in the luminal gut microbiota. It is not known whether the mucosa-associated microbiota is affected by NEC and stimulates inflammatory lesions.
OBJECTIVE: We hypothesized that the density of the mucosa-associated microbiota correlates with NEC severity in preterm pigs and that in vitro infection with increasing densities of Clostridium perfringens, which has been associated with NEC in preterm infants, would lead to a transcriptional response related to the inflammatory conditions of NEC.
METHODS: First, we determined the

OBJECTIVE: To evaluate epidermal growth factor (EGF) levels in cord blood as a possible marker for predicting the subsequent development of necrotizing enterocolitis (NEC) in very low birth weight (VLBW) infants.
PATIENTS AND METHODS: A total of 241 VLBW infants (38 infants developed NEC and 203 had no NEC) were enrolled. EGF concentrations were measured by enzyme-linked immunosorbent assay.
RESULTS: VLBW infants who subsequently developed NEC had significantly lower cord serum concentrations of EGF than those who did not (p < 0.0001). Moreover, cord blood concentrations of EGF were signific

BACKGROUND: Indomethacin is the most frequently used pharmacological agent for closure of a patent ductus arteriosus (PDA) in premature infants. However, reports of complications, particularly, necrotizing enterocolitis (NEC) and isolated gastrointestinal perforation have generated concerns about the use of this medication.
OBJECTIVES: A retrospective study to compare the incidence of NEC, NEC-related gastrointestinal complications and isolated gastrointestinal perforation among premature infants treated for a PDA with either, indomethacin alone (I), surgical ligation alone (L), or indomethac

Survival of extremely premature infants (< 27 weeks' gestational age) has improved over the past two decades. Indomethacin prophylaxis was used in these infants, who have the highest mortality and morbidity rates, to reduce the incidence of intraventricular hemorrhage and patent ductus arteriosus (PDA). Medical records of 65 extremely premature infants born at our institution between 1995 and 2001 were reviewed retrospectively to determine whether treatment of PDA with indomethacin in the first 48 hours of life reduces the need for PDA ligation or increases neonatal morbidity, when compared to

PURPOSE: Early identification of surgical necrotizing enterocolitis (S-NEC) before clinical deterioration remains challenging in extremely low-birth-weight infants (ELBWIs). We evaluated whether coagulation and fibrinolysis markers at birth could predict subsequent S-NEC.
METHODS: We retrospectively analyzed 250 ELBWIs (< 1000 g) treated from 2013 to 2022; 22 developed S-NEC. Eleven coagulation and fibrinolysis markers were measured within 24 h of birth. Multivariable logistic regression models and a simple birth-time risk score were developed.
RESULTS: Relative to controls, S-NEC infants ha

OBJECTIVES: To evaluate the ability of abdominal radiographs-based radiomics features to predict surgical risk and timing in neonates with necrotizing enterocolitis (NEC), providing an objective basis for clinical decision-making.
METHODS: A retrospective cohort of 262 NEC patients (2016-2022) was divided into surgery and nonsurgery groups. Radiomics features were extracted from abdominal radiographs at NEC diagnosis using LASSO and Cox regression to generate a radiomics risk score (Riskscore). The cohort was split into training (n = 184, 42 surgical cases) and test sets (n = 78, 18 surgical

BACKGROUND: Regional splanchnic oxygen saturation (rsSO2) is lower in preterm infants <28 weeks of gestation, who later develop necrotizing enterocolitis (NEC). Evidence is lacking for preterm infants born between 28 and 32 weeks. We aimed to investigate whether patterns of rsSO2 in the first week after birth differ between preterm infants who later developed NEC and matched controls with a gestational age between 28 and 32 weeks or >32 weeks and a birth weight <1200 g.
METHODS: We included infants with a gestational age between 28 and 32 weeks or >32 weeks and a birth weight <1200 g. NEC inf

Necrotizing enterocolitis (NEC) remains one of the most devastating gastrointestinal emergencies in preterm infants and is associated with high morbidity, mortality, and long-term complications. Increasing evidence suggests that breast milk and probiotics may act synergistically to reduce NEC risk by modulating the intestinal microbiota, its metabolites, and host immune responses. As the optimal nutritional source for preterm infants, breast milk provides a wide range of bioactive components, including human milk oligosaccharides (HMOs), lactoferrin, and immunoglobulins, which promote intestin
