Preoperative systemic immune-inflammation index predicts seizure outcomes after anterior temporal lobectomy in drug-resistant temporal lobe epilepsy.

BACKGROUND: Anterior temporal lobectomy remains the most effective treatment for drug-resistant temporal lobe epilepsy, yet 20-40 % of patients experience persistent seizures postoperatively. Neuroinflammation plays a critical role in epileptogenesis, but the prognostic value of preoperative peripheral inflammatory markers has not been systematically evaluated. The systemic immune-inflammation index (SII), a composite marker integrating neutrophil, lymphocyte, and platelet counts, has demonstrated prognostic utility in cardiovascular disease and stroke but remains unexplored in surgical epilep

Baseline neuropsychological characteristics in patients with epilepsy with left temporal lobe encephaloceles compared with left mesial temporal sclerosis.

BACKGROUND: Temporal lobe encephaloceles (TE) are increasingly recognized as a cause of drug-resistant temporal lobe epilepsy. Improved recognition of these lesions offers an opportunity to treat them with a limited resection sparing the hippocampus. However, as they can be difficult to identify on imaging, additional clues pointing to the diagnosis can be helpful. We sought to understand the baseline cognitive/neuropsychological profile in patients with left temporal lobe epilepsy caused by encephaloceles compared with that caused by mesial temporal sclerosis (MTS), a common entity in the dif

Memory loss and memory reorganization patterns in temporal lobe epilepsy patients undergoing anterior temporal lobe resection, as demonstrated by pre-versus post-operative functional MRI.

This study was aimed to longitudinally assess memory function and whole-brain memory circuit reorganization in patients with temporal lobe epilepsy (TLE) by comparing activation potentials before versus after anterior temporal lobe (ATL) resection. Nineteen patients with medically-intractable TLE (10 left TLE, 9 right TLE) and 15 healthy controls were enrolled. Group analyses were conducted pre- and post-ATL of a novelty complex scene-encoding paradigm comparing areas of blood oxygen-level-dependent (BOLD) signal activations on functional magnetic resonance imaging (fMRI). None of the pre-oper

Intra-amygdaloid injection of kainic acid in rats with genetic absence epilepsy: the relationship of typical absence epilepsy and temporal lobe epilepsy.

We showed previously that genetic absence epilepsy rats from Strasbourg (GAERS) resist secondary generalization of focal limbic seizures after electrical kindling. We now investigate the effect of intra-amygdaloid injection of kainic acid, as another model of temporal lobe epilepsy, focusing on epileptogenesis, spike-and-wave discharges (SWDs), and the transition from basal to SWD states in GAERS. The EEG was recorded from the hippocampus and cortex of adult GAERS and Wistar rats before kainic acid injections into the basolateral amygdala and for 3 months thereafter. EEG and video recordings m

Divergent Propagation Networks Define Two Pathophysiologically Distinct Electroclinical Phenotypes in Mesial Temporal Lobe Epilepsy.

BACKGROUND AND PURPOSE: Temporal dissociation between electrographic and clinical seizure onset in mesial temporal lobe epilepsy (MTLE) implies network heterogeneity. We hypothesized that MTLE consists of distinct electroclinical phenotypes defined by divergent seizure propagation dynamics. METHODS: We retrospectively analyzed 167 seizures from 13 patients with drug-resistant MTLE who achieved Engel Class I surgical outcomes. Based on the predominant relationship between scalp electroencephalography (EEG) and clinical onset, patients were classified as EEG-first (n=7) or Clinical-first (n=6).

Brain connectivity signatures of cognitive impairment in temporal lobe epilepsy identified by robotic assessment.

BACKGROUND: Subjects with temporal lobe epilepsy (TLE) often experience cognitive impairment in different domains. Currently, the mechanisms underlying neuropsychological dysfunction in TLE remain poorly understood. The main objective is to characterize the multivariate relationship between brain connectivity patterns and cognitive impairment detected by robotic testing in subjects with TLE. METHODS: Kinarm robotic technology was used to evaluate motor, cognitive, and sensory domains of healthy controls and individuals with TLE. Structural connectivity (SC) and functional connectivity (FC) we

Metabolic Brain Network Reorganization in Temporal Lobe Epilepsy With Aware or Impaired Awareness Seizures.

BACKGROUND: Temporal lobe epilepsy (TLE) is increasingly recognized as a dysfunction of the brain network. However, the topological alterations of the metabolic brain network underlying impaired awareness seizures (IAS) remain unclear. In this study, we aimed to characterize metabolic network reorganization in patients with TLE and IAS (TLE-IAS) and to preliminarily investigate the specificity of these alterations by comparing the results with those of patients with TLE and aware seizures (AS). METHODS: This retrospective study included a total of 193 TLE-IAS patients, 30 patients with TLE-AS

Cross-species transcriptomic analysis of rodent model fidelity to human mesial temporal lobe epilepsy.

Mesial temporal lobe epilepsy (MTLE) is a common form of drug-resistant epilepsy, necessitating reliable rodent models for translational research. Although numerous rodent models reproduce hippocampal epileptogenesis, their molecular fidelity to human MTLE remains unclear. In this study, we conducted an integrative analysis of human MTLE transcriptomic data and evaluated rodent epilepsy models by comparing their transcriptomic profiles with those of human MTLE. By integrating 694 human samples (mean age 39.6 years, 40.3% female, 12 datasets) and 362 rodent samples (postnatal and adult mice and

Peripheral inflammatory markers and metabolic profiles in temporal lobe epilepsy and functional dissociative seizures.

BACKGROUND: Temporal lobe epilepsy (TLE) and functional dissociative seizures (FDS) are distinct conditions that share overlapping clinical, neuropsychiatric, and biological features, including depressive symptoms, obesity-related metabolic dysregulation, and alterations in systemic inflammatory markers. Understanding the interplay among these factors may provide insight into their shared biological burden. OBJECTIVE: This study aimed to investigate the relationships among systemic inflammatory markers, adipokines, obesity, seizure frequency, and depressive symptom burden in patients with dru

Characteristics of interictal MEG spike dipoles in temporal lobe epilepsy with unilateral amygdala enlargement.

OBJECTIVE: To clarify the characteristics of MEG findings in patients with temporal lobe epilepsy with amygdala enlargement (TLE-AE). METHODS: We retrospectively identified patients who showed unilateral amygdala enlargement (AE) on MRI and ≥ 5 interictal spikes localized to the ipsilateral temporal region on MEG. Patients were classified into an AE-only group with isolated AE and an AE-plus group with AE plus additional abnormalities. The equivalent current dipole (ECD) model was used to estimate the cortical sources of MEG interictal spikes. The mean pairwise angle (MPA) of the ECD orientat

Hippocampal AIM2 inflammasome regulate seizure and cognitive impairment in mice with temporal lobe epilepsy.

Epilepsy is a chronic neurological disorder characterized by spontaneous recurrent seizures. Approximately 30% of patients are resistant to current antiepileptic drugs, highlighting the need for further investigation into the underlying mechanisms of epilepsy. Studies have shown significant neuroinflammatory responses in specific brain regions of human epilepsy foci and in animal models. Absent in Melanoma 2(AIM2) is an important neuroinflammatory factor involved in regulating brain function, but its precise role in epilepsy remains unclear. This study aimed to explore the role of AIM2 in hipp

Direct MRI-guided stereotactic radiofrequency thermal ablation for mesial temporal lobe epilepsy: illustrative cases.

BACKGROUND: Despite rapid adoption among many epilepsy centers across the United States, stereotactic magnetic resonance imaging (MRI) thermometry-guided laser interstitial thermal therapy (LITT) is not universally available to treat medically refractory epilepsy in appropriately selected patients. OBSERVATIONS: The authors present two cases in which patients with mesial temporal lobe epilepsy (MTLE)-one with stereoelectroencephalography-proven left MTLE and one with right mesial temporal sclerosis-were advised to undergo LITT by a multidisciplinary team, but an insurance provider refused pre

Modulating limbic circuits in temporal lobe epilepsy: impacts on seizures, memory, mood and sleep.

Temporal lobe epilepsy is a common neurological disease characterized by recurrent seizures that often originate within limbic networks involving amygdala and hippocampus. The limbic network is involved in crucial physiologic functions involving memory, emotion and sleep. Temporal lobe epilepsy is frequently drug-resistant, and people often experience comorbidities related to memory, mood and sleep. Deep brain stimulation targeting the anterior nucleus of the thalamus (ANT-DBS) is an established therapy for temporal lobe epilepsy. However, the optimal stimulation parameters and their impact on

Interictal suppression in patients with mesial temporal lobe epilepsy: A simultaneous PET/fMRI study.

Previous stereotactic-electroencephalography (SEEG) results have suggested that seizure-onset zones (SOZs) could be suppressed by strengthened inward connectivity from the rest of the brain during interictal periods, which might explain why people with epilepsy did not have seizures continuously. However, the limited coverage of SEEG contacts and allocation bias hindered a more comprehensive survey of interictal suppression at the whole-brain level. Previous studies also lacked a direct comparison between patients and healthy controls due to the invasive nature of SEEG. In the present study, w

De Novo ACTB Variant Associated With Juvenile-Onset Temporal Lobe Epilepsy With Favorable Outcomes.

Genetic factors are estimated to contribute to 80% of people with epilepsy. However, only four genes were reported to be associated with temporal lobe epilepsy (TLE). This study is aimed at investigating the association between ACTB and TLE. Trio-based exome sequencing was performed in a patient, and a de novo ACTB variant was identified. The patient presented with TLE featuring by age of onset in juvenile, seizure-free status in adulthood, complications of memory decline and irritability, epileptic discharges in the bilateral temporal lobes, and bilateral hippocampal sclerosis. Transcriptionally downregulated GABAergic genes associated with synaptic density network dysfunction in temporal lobe epilepsy.

PURPOSE: Temporal lobe epilepsy (TLE) is a brain network disorder closely associated with synaptic loss and has a genetic basis. However, the in vivo whole-brain synaptic changes at the network-level and the underlying gene expression patterns in patients with TLE remain unclear. METHODS: In this study, we utilized a positron emission tomography with the synaptic vesicle glycoprotein 2 A radioligand [18F]SynVesT-1 cohort and two independent transcriptome datasets to investigate the topological properties of the synaptic density similarity network (SDSN) in TLE and its correlation with signifi

Potential molecular mechanisms explaining progressive alterations of the structural network in temporal lobe epilepsy.

The effect of recurrent seizures on the gradual deterioration of the white matter structural network and the potential molecular mechanisms that underlie the baseline and longitudinal changes in network topology in temporal lobe epilepsy (TLE) remain unclear. Therefore, we used diffusion tensor imaging (DTI) scans and neuropsychiatric assessments for 28 patients with unilateral TLE at baseline and follow-up, and for 28 healthy controls (HC). The topological properties of the structural network were calculated using graph theoretical analyses. Transcription-neuroimaging association analyses wer

Personalized neurometabolic signature predicts seizure outcomes of laser ablation in mesial temporal lobe epilepsy.

BACKGROUND: Mesial temporal lobe epilepsy (mTLE) is a common form of drug-resistant epilepsy and seizure outcomes after minimally invasive laser ablation remain suboptimal. Current imaging-guided strategies often fail to capture individual variability in seizure foci. This study aimed to develop a personalized neuroimaging biomarker to improve surgical planning and predict outcomes. METHODS: Thirty patients with mTLE (16 women, 53.3%; age range 17-59 years) who underwent magnetic resonance-guided laser interstitial thermal therapy were retrospectively analyzed.
