Whole exome sequencing identifies somatically mutated genes in bladder cancer: A pilot study from Bangladesh.

Bladder cancer, ranking as the tenth most common cancer globally, lacks genomic characterization in Bangladeshi patients. This study is an exploratory analysis of the genomic landscape of bladder cancer in this population by conducting whole-exome sequencing (WES) on tumor (median coverage = 65X) and matched blood (median coverage = 68X) samples from four patients at the National Institute of Cancer Research and Hospital in Dhaka, using the Illumina HiSeq platform. Somatic mutations were identified using GATK Mutect2 and analyzed with bioinformatics tools to explore potential genetic alteratio…

Clinical utility of prenatal exome sequencing after normal chromosomal microarray in fetuses with structural anomalies.

Congenital anomalies detected by ultrasound occur in approximately 2-4% of pregnancies. Cytogenetic testing allows detection of chromosomal abnormalities, but the majority of fetuses remain without a diagnosis. It is in this context that exome sequencing was introduced into prenatal medicine. The objective of this study was to analyze the indications for this test, determine its diagnostic yield, and assess its impact on pregnancy outcome. Among the 68 fetuses included, exome sequencing identified pathogenic variants in 22 cases, corresponding to an overall diagnostic yield of 32%. Termination…

Monogenic Findings in Early Pregnancy Loss: Whole-Exome Sequencing Study of Euploid Products of Conception.

Early pregnancy loss (EPL), particularly when recurrent, represents a profoundly distressing experience for affected couples. Although chromosomal abnormalities are the most common cause of EPL, a substantial proportion of cases, especially those involving euploid embryos, remain unexplained. In this study, we investigated the potential contribution of rare monogenic variants to euploid EPL using whole-exome sequencing (WES). WES was performed on 66 euploid products of conceptions (POCs) from EPLs occurring before 12 gestational weeks. A molecular diagnosis with a high level of confidence, def…

Unmasking rare thalassemia variants through whole-exome sequencing in Huadu District, China: An observational study.

In regions with a high prevalence of thalassemia, conventional diagnostic methods may fail to detect atypical or complex genetic variants. Whole-exome sequencing (WES) provides a comprehensive strategy to identify such variants, allowing more accurate genotype-phenotype correlation. Nevertheless, its optimal integration into clinical workflows and its incremental value over standard diagnostic approaches remain uncertain. This study aimed to evaluate the clinical utility of WES in a heterogeneous patient cohort and to propose a framework for its rational use in endemic settings. WES was perfor…

Exome sequencing-based identification of DNAAF1 variants implicated in male infertility and primary ciliary dyskinesia.

Primary ciliary dyskinesia (PCD) is a rare, autosomal recessive disorder caused by impaired cilia and flagella function. Despite advances in molecular diagnostics, pathogenic variants remain to be detected in a subset of clinically diagnosed individuals. In the present case, abdominal ultrasonography revealed situs inversus of the liver and spleen, and chest X-ray demonstrated dextrocardia. Semen analysis showed markedly reduced sperm motility, consistent with ciliary dysfunction, and the patient exhibited additional clinical features characteristic of PCD. Exome sequencing (ES) revealed biall…

Exome Sequencing Uncovers Phenotypic and Genotypic Heterogeneity in 196 Indian Families Evaluated for Autoinflammatory Disorders.

Autoinflammatory disorders (AIDs) are a clinically heterogeneous group of inborn errors of immunity primarily caused by dysregulation in the innate immune system. Clinical diagnosis is often challenging due to clinical heterogeneity and the overlapping phenotypes with other inborn errors of immunity and monogenic conditions that mimic AIDs. Worldwide, and especially in India, there is a lack of systematic large cohort studies employing genomic testing early in the course of evaluation for individuals with autoinflammatory disease. Herein, we describe our experience using exome sequencing as a …

Computational exploration of TITIN variations: insights from whole exome sequencing and molecular dynamics simulation study.

Titin (TTN), the largest known human protein (∼4 MDa), is considered as a key component for sarcomere integrity and function. Mutations in the TTN gene play a pivotal role in the genetic underpinnings of Dilated Cardiomyopathy (DCM). In the present study, we have conducted whole exome sequencing (WES) on 15 patients (5 familial and 10 sporadic) diagnosed with idiopathic DCM and identified 88 exonic variants. Here, we also report for the first time four novel variants comprising two frame-shifts, one missense, and one stop-codon variant. These variants are predominantly located in the A-band re…
