The aneurysmal bone cyst (ABC) is a benign tumour of children and young adults. It represents approximately 1-2% of all bone tumours. The ABC may develop on all skeletal bones, but the proximal end of the femur is the most common location. The authors report a ABC femoral neck in a child of 13 years. This location is pretty special. Indeed, the fragility of the femoral neck due partly to the pathology itself and secondarily curettage requires a judicious attitude surgical (excisional curettage + bone graft + screw) to prevent the risk of high local recurrence and pathological fracture.

A retrospective review was performed on 27 consecutive patients with total hip arthroplasty (THA) following failure of internal fixation of fractures of the proximal femur. The results were comparable to primary THA in femoral neck fractures. Considerably less satisfactory results were obtained in THA in intertrochanteric fractures. Bone loss and medial displacement of the femoral shaft led to high incidence of intraoperative complications and postoperative dislocations. Extreme care must be taken to avoid fracture and penetration of the femoral shaft. Autograft, allograft, or head and neck re

BACKGROUND: The appropriate treatment in mild slipped capital femoral epiphysis (SCFE) should not only prevent further slipping of the epiphysis but also address potential femoroacetabular impingement by restoring the anatomy of the proximal femur. The aim of this study was to quantify length of the remodeling phase mediated by growth of the femoral neck, after treatment of SCFE with a screw designed to prevent premature closure of the physis and provide stability.
METHODS: Between 2001 and 2011, 38 patients with unilateral mild SCFE were treated by fixation in situ using a modified screw whi

BACKGROUND: The initial goals of the treatment of slipped capital femoral epiphysis (SCFE) are to stabilize the epiphysis, prevent slip progression, and avoid complications. In situ fixation with a single screw is the most accepted procedure to provide an optimal fixation, but fixation failure and slip progression suggest that the procedure might be improved. The aim of the present study was to biomechanically compare partially threaded screws (16 mm and 32 mm) and fully threaded screws in an in vitro porcine model.
METHODS: An unstable/acute SCFE was created in 18 skeletally immature porcine

Osteoporosis international : a journal established as result of cooperation between the European Foundation for Osteoporosis and the National Osteoporosis Foundation of the USA2000eng
An inappropriate reference range for peak bone mineral density (BMD) may result in identification of an incorrect proportion of subjects with osteopenia and osteoporosis at dual-energy X-ray absorptiometry (DXA). In this study, we assessed the prevalence of low BMD in Turkish young adults with respect to local population reference range T-scores and the US reference range T-scores. The BMD values of lumbar spine (L1-L4) and proximal femur (femoral neck, intertrochanter, trochanter, Ward's triangle and total) were measured by DXA in 323 healthy young adults (171 women, 152 men) aged 19-25 years

A 63-year-old woman presented with a low energy pelvic fracture, which showed no signs of healing. Initial fractures were to the right hemipelvis, later followed by a right fractured neck of femur. We present a complicated patient journey, management dilemmas and highlight the growing concern with long-term bisphosphonate treatment.

Reports of long-term evaluation of the viability of the femoral head after a hemiresurfacing arthroplasty (HRA) are scarce. We report the case of a 41-year-old female with lupus and right HRA (performed 24 years previously for avascular necrosis) that was revised to a total hip arthroplasty for worsening right groin pain due to chondrolysis. We present a histologic evaluation of the resurfaced femoral head retrieved 24 years after HRA. To the authors' knowledge, this is the longest reported histologic follow-up of such a case. There was no evidence of fracture or inflammation and the underlyin

The stability of intertrochanteric fractures depends upon multiple factors including the fracture displacement, location and pattern, the type of fixation used for stabilization, and the loading that the fracture undergoes postfixation. Traditional classification systems are of limited utility because they typically stratify intertrochanteric fractures as stable or unstable based purely on the fracture pattern without consideration of the stability after fixation. Biomechanical studies evaluating the stability of various fixation constructs should include physiologic loading, including rotatio

I report on two cases of cerebral infarction caused by fat embolism during the orthopedic surgeries. The first patient was a 77-year-old woman with a femur neck fracture, who developed coma after orthopedic operation. The other was a 70-year-old woman with open fractures in the femur and the fibula, who developed hemiplegia after operation. By echogram, no embolus was demonstrated in the heart, in the carotid arteries or in deep veins, also paradoxical cerebral infarction was denied in the both cases. Diffusion-weighted MR image and FLAIR MR image showing multiple hyperintense signals in the h…

Cannulated screw fixation is the main therapy for femoral neck fractures, especially in young patients. The traditional surgical procedure uses C-arm fluoroscopy to place the screw freehand and requires several guide wire adjustments, which increases the operation time and radiation exposure. Repeated drilling can also cause damage to the blood supply and bone quality of the femoral neck, which can be followed by complications such as screw loosening, nonunion, and femoral head necrosis. In order to make fixation more precise and reduce the incidence of complications, our team applied robot-as

INTRODUCTION: Most geriatric hip fractures occur in the femoral neck (FN) and intertrochanteric (IT) regions of the femur, while a minority occur in the subtrochanteric (ST) region. Relative outcomes based on the anatomical subtype of fracture are not well studied. This study characterizes postoperative complications and outcomes of hip fractures distinguished by anatomic region.
MATERIALS AND METHODS: The targeted hip fracture series of the American College of Surgeons National Surgical Quality Improvement Program database was queried to identify geriatric (≥ 65 years) patients who sustained

Femoral neck fractures are of great interest for challenging several problems to indicate surgical procedures. Even though ages and general or local conditions of the patient, and types of fracture or some other influencing factors are sufficiently taken into consideration, it is very difficult to foresee the fate of femoral head on the radiogram at the time of injury. This viability of fractured femoral head is the most important factor to choose the method of treatment, but no adequate method of assessing the viability has been established. One hundred and twelve femoral heads removed in the

Within the last decade, intramedullary nailing systems have gained enormous popularity in the treatment of fractures of the trochanteric region. However, these implants are not free of any complications. Commonly reported treatment failures include stress-derived femur fracture, rotational displacement, varus deformity of the femoral neck and head, and implant cutout through the femoral neck. So far, there are only five cases known in the literature describing dislocation of the medial leg screw into the pelvis. The patient history, operative intervention, and course are reported. The incidenc

Clinical, experimental and post-mortem studies suggest that avascular necrosis of the femoral head following fractures of the neck of the femur, is due to compression, torsion, stretching or rupture of the blood vessels by displacement of the fragments. If this is true then urgent reduction and internal fixation are important in the prevention of this complication. This study shows that those cases which were submitted to early reduction and internal fixation had a reduced incidence in severity of avascular necrosis compared with those in whom operation was delayed. Therefore it is proposed th

INTRODUCTION: Ipsilateral femoral neck and shaft fractures (IFN-SFs) caused by high-energy trauma pose a significant risk of complications related to bone healing. Prompt identification of fracture types and maintenance of fracture fixation stability can mitigate this risk. This study employed finite element analysis to evaluate biomechanical parameters for the stability of fixation in IFN-SFs and quantify differences in biomechanical stability among various fracture types.
METHODS: Patient-specific femur models were constructed from computed tomography data of 10 individuals. Simulating comb
