[Typhoid fever--case report].

The paper describes a case treated at the Department of Infective Diseases in Novi Sad admitted during the remittance of typhoid fever. The course of the disease was atypical. Epidemiological and clinical features resembled those found in malaria but laboratory findings gave the true diagnosis. We found the case interesting because the diseased patient moved around a lot, making contacts with many people. Fortunately, contact diseases never happened to occur.

Multidrug resistant enteric fever.

Multidrug resistant typhoid fever (MDRT) is becoming an alarming public health problem in and around Pondicherry, South India. A retrospective review of the multidrug resistant typhoid fever cases admitted to the paediatrics ward of JIPMER Hospital, Pondicherry (India) during 1990 is presented. Prolonged pyrexia, chills and rigors, toxaemia, and tender hepatomegaly often more than 3 cm below the costal margin (often without splenomegaly) were striking features of MDRT cases. The incidence of complications was also greater. Positive blood cultures were observed even after weeks of antibiotic th…

Fever of unknown origin.

FUO is an uncommon problem in pediatric patients if one uses a strict definition of at least 2 weeks of fever. Perhaps the best definition, as suggested by Lorin and Feigin, is more than 1 week of fever and a negative initial evaluation, including examination and preliminary laboratory results. Most patients, especially younger ones, will spontaneously improve or have common disorders, usually respiratory-related infections. Most patients will have uncommon manifestations of more commonly recognized disorders. Mortality rates, even in those series that include a week in the hospital without a …

Rocky Mountain spotted fever.

Rocky Mountain spotted fever is an endemic tickborne disease found throughout the United States and other regions of the world. Exposure may result in a spectrum of disease from subclinical infection to severe or fatal multiorgan collapse. The disease is maintained in nature in Ixodid tick vectors and their hosts. The most important ticks in the United States are Dermacentor variabilis and Dermacentor andersoni. Small mammals are the natural reservoirs in the wild. Dogs become infected when a tick harboring Rickettsia rickettsii feeds on the dog. Dogs do not develop sufficient rickettsemia to …

Q fever in Israel.

BACKGROUND: Q fever was first described over fifty years ago. While in a proportion of cases there is a clear etiologic link to farm animals, their products, and fomites, in the majority of cases no such association is apparent, and the mode of infection is a mystery. METHODS: In this paper, we examine the reported incidence of Q fever in Israel since it was made notifiable in 1951, and in particular for the period 1981-1985, when special surveillance at national reference laboratories was carried out. RESULTS: Age and sex as well as urban-rural distributions suggest the influence of occupat…

Q fever in octogenarians.

Two of the 186 persons (1%) with acute Q fever examined in this study were 80 years of age or older. Both were males and both had Coxiella burnetti pneumonia. Q fever was not suspected during their hospital stay and would not have been diagnosed if these patients had not been part of a prospective study of community-acquired pneumonia.

Q fever - a review.

Q or "query" fever is a zoonosis caused by the organism Coxiella burnetii. Cattle, sheep and goats are the most common reservoirs of this organism. The placenta of infected animals contains high numbers (up to 10(9)/g) of C. burnetii. Aerosols occur at the time of parturition and man becomes infected following inhalation of the microorganism. The spectrum of illness in man is wide and consists of acute and chronic forms. Acute Q fever is most often a self-limited flu-like illness but may include pneumonia, hepatitis, or meningoencephalitis. Chronic Q fever almost always means endocarditis and …

[Rocky Mountain spotted fever].

After returning from a holiday in the USA a 24-year-old man fell ill with diarrhoea, high fever and marked rash including the palms of the hands and soles of the feet. When a history of a tick bite in the USA was elicited, a rickettsial infection was suspected. Treatment with doxycycline, 100 mg twice daily, was instituted finally and the fever slowly resolved. The patient became completely well again within four weeks. Serological tests confirmed the diagnosis of Rocky Mountain spotted fever.

[Fever, somnolence, vomiting, diarrhea].

A 69-year-old female was admitted because of a febrile state, somnolence, nausea and diarrhea. Addison's disease known to have developed several years earlier after adrenal tuberculosis suggested Addisonian crisis triggered by an acute infection. Therapy with steroids and substitution of electrolytes and fluid led to rapid improvement. Streptococcus bovis was identified as causative agent. The known association of this germ with cancer of the colon led to further investigation which revealed cancer of the sigmoid and incidental cancer of a salpinx. After surgical removal of both cancers, the p

[Complications of typhoid fever].

The authors report the complications noted in the course of 120 cases of typhoid fever. Intestinal hemorrhages and ileal perforations were the most frequent complications observed, respectively, among 12.5% and 3.3% of patients. Neuropsychiatric manifestations were also frequent: 6.7% of cases. Only one death was noted (0.8%). According to this high rate of complications, typhoid fever must be managed under hospital conditions.
