Autologous double-barrel vascularized fibula bone graft for arthrodesis of the shoulder after tumor resection.

INTRODUCTION: Arthrodesis of the shoulder is a straightforward and established alternative to reconstruct the shoulder function after tumor resection of the proximal humerus. In most cases, some kind of intercalary bone graft is used to bridge the bony defect. However, due to low stability of a single fibula autograft and disadvantages of exogenous graft material when performing combined allo- and autograft reconstruction, efforts to develop new surgical techniques, with the intention to lower the complication rates, are ongoing. MATERIALS AND METHODS: We present a detailed description of the

Intense start, painful consequences: a case report of fatigue fractures.

The aim of this study is to present a case of a patient with stress fractures. Stress fractures, otherwise known as fatigue fractures, result from abnormal and repetitive strain of healthy bone. This leads to micro damages and subsequent fractures. They are most commonly encountered after a sudden and excessive increase in physical activity and most frequently located in the metatarsal, heel, tibia, fibula and femur. This case presents a twenty-nine year old patient, diagnosed with obesity, not involved in sport ever before. Since the end of the spring, he started to exercise intensively at th

Stress fractures of the foot and ankle, part 2: site-specific etiology, imaging, and treatment, and differential diagnosis.

Stress fractures of the foot and ankle are a commonly encountered problem among athletes and individuals participating in a wide range of activities. This illustrated review, the second of two parts, discusses site-specific etiological factors, imaging appearances, treatment options, and differential considerations of stress fractures of the foot and ankle. The imaging and clinical management of stress fractures of the foot and ankle are highly dependent on the specific location of the fracture, mechanical forces acting upon the injured site, vascular supply of the injured bone, and the propor

Fracture risk among patients with Paget's disease: a population-based cohort study.

Localized disruption of bone architecture leads to an increased risk of pathological fractures in patients with Paget's disease, but the impact of the disease on overall fracture risk is unknown. We addressed this issue among 236 Olmsted County, Minnesota residents (107 women and 129 men) first diagnosed with Paget's disease from 1950 through 1994. These subjects (mean +/- SD age at diagnosis, 69.6+/-12.2 years) were followed subsequently for 2798 person-years. During this period of observation, 33 pathological fractures were attributed to Paget's disease (1 skull, 11 vertebra, 1 shaft/distal

Dropped hallux after the intramedullary nailing of tibial fractures.

We made a prospective study of 208 patients with tibial fractures treated by reamed intramedullary nailing. Of these, 11 (5.3%) developed dysfunction of the peroneal nerve with no evidence of a compartment syndrome. The patients with this complication were significantly younger (mean age 25.6 years) and most had closed fractures of the forced-varus type with relatively minor soft-tissue damage. The fibula was intact in three, fractured in the distal or middle third in seven, with only one fracture in the proximal third. Eight of the 11 patients showed a 'dropped hallux' syndrome, with weakness

Are Schatzker and AO classifications accurate enough to classify tibial plateau fractures in alpine skiers?

PURPOSE: The aims of this study were (1) to collect prospectively all tibial plateau fractures admitted to our department, over two ski seasons, and to classify them according to the Schatzker and AO classifications; (2) to assess if these classifications are accurate enough to include all types of fractures; and (3) to compare theses fractures with the series found in the literature, which included very few or no skiing accidents. METHODS: During the 2016-2017 and 2017-2018 ski seasons, we prospectively included 116 tibial plateau fractures caused by downhill skiing accidents. All patients u

Assessment and management of adult ankle fractures: understanding the evidence.

Ankle fractures are a common injury. Assessment should include looking at the mechanism of injury, comorbidities, associated injuries, soft tissue status and neurovascular status. Emergent reduction is required for clinically deformed ankles. Investigations should include plain radiographs and a computed tomography scan for more complex injuries or those with posterior malleolus involvement. An assessment of ankle stability determines treatment, taking into account comorbidities and preoperative mobility which need special consideration. Non-operative management includes splint or cast, allowi

A short single-incision approach for antiglide plate fixation of oblique fractures of the lateral malleolus: a technical tip.

Displaced fractures (> 2 mm) of the lateral malleolus, without medial or syndesmotic injuries, are often the subject of controversy regarding the best treatment option. In the past, these fractures were usually treated conservatively using a cast for 6 to 8 weeks without weightbearing. Currently, most of the patients desire a quick return to their previous activities, what makes surgical treatment a reasonable choice. It has benefits as earlier ankle mobilization and weightbearing. It also restores ankle biomechanics, preventing secondary osteoarthritis. However, postoperative complications wi

Tibial stress fracture after fibular graft harvesting: an unusual complication.

INTRODUCTION: Fibular grafts are commonly used in reconstructive head and neck surgery, and several complications have been reported. Tibial stress fracture, however, is rare. PATIENT: A 65-year-old woman presented with a tibial stress fracture 10 weeks after a vascularized fibular flap had been harvested for a mandibular defect due to osteoradionecrosis. The main symptom was activity-related pain after a period of no complaints. CONCLUSIONS: One should be aware of the complications, particularly after weeks or months, in cases of activity-related pain in the lower extremities. Prevention ca

Persistent diastasis in a Maisonneuve fracture--interposition of a tibial osteochondral fragment: a case report.

We report a case of failed reduction of the distal tibiofibular syndesmosis following a Maisonneuve fracture due to interposition of an osteochondral fragment in the syndesmosis from the tibial plafond. The displaced fragment was not initially appreciated. It is suggested that surgeons consider this as a possible cause of an incompletely reduced diastasis after deltoid ligament interposition has been excluded.

Reversal of the histology of bone after parathyroidectomy in patients with hyperparathyroidism.

Hyperparathyroidism is commonly seen in patients with end-stage renal disease and less commonly in the primary form. The skeletal manifestations of hyperparathyroidism are the same in both forms and are well described in the literature. We treated a patient from each category. Multiple bony lesions and pathologic fractures were observed. The clinical presentations and radiologic and histologic findings confirmed the diagnosis of hyperparathyroidism and osteitis fibrosa cystica in both patients. Subtotal excisions of the parathyroid glands were done in both patients. Appropriate treatment of th

Delayed union of fibular fractures accompanying fractures of the tibial shaft.

Among 440 adult patients with tibial shaft fracture and accompanying fibular fracture there were eight cases with radiographically ununited fibulae 4 months after the injury, each with uneventful tibial union. Fractures with severe soft-tissue injuries were excluded from this study. In 293 patients the treatment method of the tibial fracture was conservative, comprising closed reduction and immobilization by long plaster cast. In 147 patients it was intramedullary Küntscher nailing, and all the eight cases with delayed fibular union occurred among these, the frequency being 5.4%. The typical a

The Fibula Free Flap for Salvage of Complications After Orthopedic Extremity Bony Fixation in Oncologic Patients.

BACKGROUND: Advances in orthopedic oncology have significantly improved outcomes following extremity tumor resections; however, complications like nonunion, hardware failure, and radiation-induced fractures can occur after orthopedic fixation. While the vascularized fibula-free flap (FFF) is well-established in primary reconstructions, its effectiveness as a salvage option is not well described. We report our 25-year experience with the use of FFF in salvage extremity reconstruction. METHODS: This retrospective case series included patients undergoing extremity salvage with FFF after failure

Forensic morphological signs characterizing stability of the femur, tibia and fibula during effect of external destructive load.

OBJECTIVE: Introduction: Fractures of the lower limb bones very often become the subject of forensic expertise when experts have to determine the mechanisms of fracture formation, make retrospective restoration of conditions of their occurrence, solve the issues concerning the possibility of their formation under certain conditions. The aim of study is directed to investigation of morphological signs facilitating solidity of the osseous tissue of the lower limb long tubular bones, and therefore, promoting biochemical processes of their destruction in case of external traumatic impact. [Epithelioid hemangioendothelioma of bone complicated by femoral fracture].

A 54-year-old man was seen with a fracture of the left femur. Plain radiographs revealed a 40-mm lytic centromedullary lesion. Magnetic resonance T1- and T2-weighted sequences showed high and low signals. After stabilization of the fracture, the tumor was removed followed by reconstruction with a vascularized fibula. The pathological examination demonstrated proliferation of non atypical CD34 and CD31 positive epithelioid cells with few lumens, accompanied by abundant fibrous stroma, sometimes masking tumor cells. Satisfactory motion was achieved with no recurrence at 20 months follow-up. Bone

Vascularized fibular grafting for the treatment of congenital pseudarthrosis of the tibia.

Six patients with congenital pseudarthrosis of the tibia were treated with vascularized fibula transplantation after the abnormal pseudarthrosis site had been radically debrided. At the time of surgery, the patients' ages ranged from 18 months to 5.5 years, and follow up was from 9 months to 4.5 years. All the fibular transplants healed and the patients were all fully weight bearing in a brace 6-months postoperatively. Age at fracture or surgery, site of the fracture, or radiographic appearance were not contraindications to the use of this technique. Although three of the patients required one

Compartment syndrome without pain!

We report the case of a young male patient who underwent intra-medullary nailing for a closed, displaced mid-shaft fracture of tibia and fibula. He was commenced on patient controlled analgesia post-operatively. A diagnosis of compartment syndrome in the patient's leg was delayed because he did not exhibit a pain response. This ultimately resulted in a below-knee amputation of the patient's leg. We caution against the use of patient controlled analgesia in any traumatised limb distal to the hip or the shoulder.

Intraosseous lipoma of the calcaneus: a review and report of four cases.

Intraosseous lipomas have been reported as rare tumors in the lower extremity. They have been identified in the tibia, fibula, metatarsals, and calcaneus. They are frequently misdiagnosed as other tumors, especially unicameral bone cysts or aneutysmal bone cysts when found within the calcaneus. A detailed description of intraosseous calcaneal lipomas and their treatment is given. Surgical technique and four case reports are presented. One of the cases involved a pathologic calcaneal fracture, a finding to the authors' knowledge that has not been previously reported.
