Perilymph fistula resulting from microfissures.

Perilymphatic fistulas are well documented in the otolaryngologic literature as a cause for both auditory and vestibular symptoms. The anatomic location of fistulas has been clinically confirmed in both the oval and round windows. Furthermore, membranous tears within the cochlea have been described post mortem and may be the cause of the remaining fistulas which are suspected but not confirmed surgically. A fourth source of perilymph fistula has previously been suggested by Okano and Harada. During the past several years, surgical confirmation of perilymph leakage from two microfissure areas h…

Arteriovenous fistula simulating arteriovenous malformation.

A 36-year-old man was thought (for 20 years) to have an arteriovenous malformation that could not be excised. Repeated ligations of proximal arterial supply to the vascular lesion were only transiently beneficial and may have caused a delay in correct diagnosis because of impaired angioaccess. Once it was discovered that he had an arteriovenous fistula--probably caused by a tonsillectomy at age 6--it was possible to occlude the fistula with detachable balloons. The mass and his headaches subsequently resolved. AV fistulas are caused by trauma. Growth of AVMs is often stimulated by trauma. Both…

Perilymphatic fistula: a histopathologic study.

Over the last two decades, clinical criteria for perilymphatic fistulae have been defined to the extent that differentiation can be made between such fistulae and other balance-affecting disorders such as Meniere's syndrome. On the assumption that the specimens in the temporal bone bank of the University of Chicago Medical School that had been obtained from patients having vertigo, hearing loss, or both, before those clinical criteria were so defined might have been classified incorrectly, we proposed a retrospective histopathologic study, with prediction of two independent variables: a clinic…

Spontaneous intrahepatic cutaneous biliary fistula.

A rare case of spontaneous intrahepatic cutaneous biliary fistula due to obstruction of the biliary tree by ampullary fibrosis is reported. The diagnosis was made clinically by the presence of constant flow of the bile through the fistulous opening and confirmed by roentgenography and by surgery. The definitive treatment consisted of removal of the obstruction by choledochoduodenostomy. The fistula healed spontaneously within two weeks of surgery.

Rectourethroperineal fistula in Crohn's disease.

We report the ninth case of a rectourethral fistula in Crohn's disease. The patient had undergone numerous surgical procedures for complications of severe ileocolitis before developing a rectourethroperineal fistula. His symptoms were confined to urine leaking from the perineal opening; the most common symptom of rectourethral fistula in Crohn's disease. He underwent surgical repair by an abdominoperineal approach and has been free of recurrence for a period of 2 years. The literature suggests that the incidence of rectourethral fistulae in patients with Crohn's disease is approximately 0.3% a…

Carotid artery-cavernous sinus fistula.

Carotid artery-cavernous sinus fistula is a lesion most often associated with massive head trauma. The maxillofacial surgeon must be able to diagnose this major complication of head trauma, use proper diagnostic techniques, and initiate appropriate therapeutic regimens. The symptoms of pulsatile headache, bruit, and visual change accompany the physical findings of bruit, conjunctival engorgement, chemosis, proptosis, and ophthalmoplegia. A number of procedures have been devised to correct this difficult lesion, ranging from cervical ligation to balloon occlusion. Two cases of carotid artery-ca…

Enterovesical fistula complicating Crohn's ileocolitis.

An enterovesical fistula, an infrequent complication of Crohn's ileocolitis which is considered to constitute a difficult surgical problem, was encountered in 29 patients. These patients had been treated for Crohn's disease by medical means for years and several had previously undergone bowel resections. An exacerbation of the Crohn's disease occurred in all patients coincident with the appearance of dysuria or the obvious clinical presentation (fecaluria) of the fistula. Other manifestations of active Crohn's disease frequently coexisted. Radiographic evaluation was of limited help in establi…

Esophageal atresia and tracheoesophageal fistula.

Associated anomalies are now the major cause of mortality in infants with esophageal atresia. Early diagnosis, preoperative management and appropriate referral for repair are challenges for the family physician. Timing and choice of surgical procedure depend on the type of malformation.

Popliteal arteriovenous fistula following meniscectomy.

A case of arteriovenous fistula between the popliteal artery and vein after injury during medial meniscectomy is presented. The diagnosis was made on the fifth postoperative day by angiography, and a successful reconstruction of the artery and vein was accomplished by primary anastomosis of the artery following resection of the damaged part; the vein was closed by lateral venorrhaphy.

Labyrinthine fistula in chronic mastoiditis.

A series of 79 otic capsule fistulae occurring in 792 consecutive cases of chronic mastoiditis undergoing mastoidectomy was analyzed. The incidence (10%) is the same as in other large series, the primary difference being the location of the fistula. In all series cited, 90% of fistulae were in the lateral semicircular canal whereas in this series the comparable incidence was 75%. The 20 (25%) fistulae located in areas other than the lateral semicircular canal enabled us to document a method of "reading" the fistula test preoperatively to establish its precise location. Eye responses to the tes…

Rectourethrocutaneous fistula in Crohn disease.

Fistulae from the gastrointestinal tract to the urinary tract are a well-known complication of Crohn disease. The majority of these are ileovesical. On rare occasion, patients with perianal complications of regional ileitis or granulomatous colitis may develop rectourethrocutaneous fistulae. The authors have encountered 2 such patients and add them to the 3 previously reported cases in the English literature. Retrograde urethrography and voiding cystourethrography established the diagnosis.

Rectalepidural fistula complicating Crohn's enterocolitis.

A previously unreported complication of a patient with Crohn's enterocolitis and internal fistulation is presented. The patient presented with meningeal signs in the lumbosacral region, fever, and sepsis. Computerized axial tomography revealed air in the epidural space, and a presumptive diagnosis of rectalepidural fistula was made. Surgical management included a diverting end sigmoid colostomy and presacral drainage.

Peranal repair of rectoprostatic fistula.

Rectoprostatic fistula is a rare complication of prostatectomy by any route including transurethral resection. Occasionally a fistula occurs congenitally or as a result of pelvic trauma. Five patients with rectoprostatic fistulas (4 post-prostatectomy, 1 traumatic) were repaired peranally using a full-thickness local flap of anterior rectal wall. The repair was protected by sigmoid loop colostomy. Primary healing occurred in all 5 patients. Anal and urinary continence are normal following colostomy closure in 3 patients. Two patients died of unrelated causes before colostomy closure.
