Intraperitoneal fluid administration to neonatal calves.

OBJECTIVE: To determine if Intraperitoneal fluid therapy (IPFT) is an effective rehydration therapeutic option for dehydrated neonatal calves. Perceived risks and unknown efficacy have limited IPFT implementation into practice. It is hypothesized that IPFT will be an efficient and safe method of fluid administration to healthy neonatal calves. METHODS: 4 L of isotonic saline was administered IP to healthy calves (n = 4). Control animals received no treatment (n = 2). Clinical and blood parameters were collected before and after fluid administration in all animals. RESULTS: Girth diameter and…

Reduced oncotic pressure after shock: a proposed mechanism.

Hemorrhagic shock plus resuscitation leads to expansion of both interstitial fluid space (IFS) and intracellular fluid (ICF). THe IFS expansion is thought to reflect reduced serum colloid oncotic pressure (COP) from capillary leak of serum albumin (SA). This hypothesis was analyzed in 138 injured patients who received an average of 13.6 units of blood and 10.7 L of saline solution for correction of shock. Plasma volume (PV), extracellular fluid (ECF) by inulin space technique, SA level, percent albumin leak (PAL), total intravascular albumin content (TIAC), COP, IFS, weight gain, and time of i

Managing the dehydrated child.

Hypotonic fluid losses lead to hypertonic dehydration. Because the extracellular fluid compartment has the ability to draw fluid from the intracellular compartment, intravascular volume is better maintained in hypertonic dehydration than in isotonic or hypotonic dehydration. When the amount of weight loss is known, the fluid deficit can be easily calculated; otherwise, a clinical assessment of the percentage of dehydration is required.

Severe leptospirosis: treatment with intravenous corticosteroids and supportive care.

Leptospirosis is a common zoonotic infection worldwide and is recognized as an emerging public health problem. Although commonly thought of as a tropical disease, incidence in temperate climates is increasing, with recent outbreaks in the United States and Germany, among other countries. The disease presents with symptoms ranging from fever, headache, nausea, and vomiting to life-threatening multiorgan failure characterized by acute liver failure, nephritis, pulmonary hemorrhage, meningitis, and cardiac arrhythmia. We describe a case of an otherwise healthy 28-year-old man who had just returne

Reversal of oligohydramnios with subtotal immersion: a report of five cases.

OBJECTIVE: The purpose was to summarize five cases where subtotal immersion was associated with an increase in the amniotic fluid index. STUDY DESIGN: Five women with oligohydramnios, as defined by an amniotic fluid index < 8 cm, who underwent subtotal (shoulder-deep) immersion therapy are described. RESULTS: The mean pretreatment amniotic fluid index was 4.9 +/- 3 cm. After immersion therapy was instituted, the amniotic fluid index increased an average of 6 +/- 2.2 cm. In three subjects whose immersion therapy was discontinued, the amniotic fluid index fell an average of 4.7 cm. CONCLUSION

New tools for optimizing fluid resuscitation in acute pancreatitis.

Acute pancreatitis (AP) is a frequent disease with degrees of increasing severity responsible for high morbidity. Despite continuous improvement in care, mortality remains significant. Because hypovolemia, together with microcirculatory dysfunction lead to poor outcome, fluid therapy remains a cornerstone of the supportive treatment. However, poor clinical evidence actually support the aggressive fluid therapy recommended in recent guidelines since available data are controversial. Fluid management remains unclear and leads to current heterogeneous practice. Different strategies may help to im…

The syndrome of inappropriate secretion of antidiuretic hormone and fluid restriction in meningitis--how strong is the evidence?

In patients with meningitis, fluid restriction is recommended to counter the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) and to reduce cerebral oedema. However, any effects of an increased plasma level of ADH upon cerebral oedema would be due not to fluid retention but to hypoosmolality. In a literature review of fluid and electrolyte disturbances and the effect of fluid therapy in bacterial/tuberculous meningitis, the prevalence of hyponatraemia, hypoosmolality and SIADH varied considerably; apparently, non-osmotic stimuli for the secretion of ADH, e.g. intracranial hy…

The usual suspects: a pragmatic framework to identify and address reversible contributors in apparent refractory septic shock.

BACKGROUND: A subset of patients with septic shock remains hypoperfused or requires escalating vasopressor support despite initial resuscitation. Although this pattern may suggest progression toward refractory septic shock, early high vasopressor requirements or persistent hypoperfusion may still reflect potentially modifiable contributors, including unresolved infection, inadequate fluid administration, metabolic derangements, iatrogenic factors, or cardiac dysfunction. The recent SCCM/ESICM consensus provides expert-derived criteria for refractory septic shock, but the preceding bedside proc

Fluid Resuscitation in the Treatment of Acute Pancreatitis: Rate and Volume Controversies.

The aim of the Research to Practice column is to enhance the research critique abilities of both advanced practice registered nurses and emergency nurses, while also aiding in the translation of research findings into clinical practice. Each column focuses on a specific topic and research study. In this article, we used a scenario of left upper quadrant pain to explore the study by de-Madaria et al., titled "Aggressive or moderate fluid resuscitation in acute pancreatitis".

Conservative versus liberal fluid resuscitation for septic patients at risk for fluid overload: A systematic review with meta-analysis.

BACKGROUND: Intravenous fluid resuscitation forms a crucial part of the sepsis bundle. However, the perception is that patients with comorbidities such as congestive heart failure, chronic kidney disease, and cirrhosis receive lower volumes due to concerns regarding potential for fluid overload. We review outcomes relating to resuscitation with conservative versus liberal volumes in specific patient populations. METHODS: We searched MEDLINE, Embase+Embase Classic, Cochrane library, Web of Science, CINAHL Complete, and ClinicalTrials.gov for studies that compared outcomes related to different

Predictive accuracy of changes in the inferior vena cava diameter for predicting fluid responsiveness in patients with sepsis: A systematic review and meta-analysis.

BACKGROUND: Existing guidelines emphasize the importance of initial fluid resuscitation therapy in sepsis management. However, in previous meta-analyses, there have been inconsistencies in differentiating between spontaneously breathing and mechanically ventilated septic patients. OBJECTIVE: To consolidate the literature on the predictive accuracy of changes in the inferior vena cava diameter (∆IVC) for fluid responsiveness in septic patients. METHODS: The Embase, Web of Science, Cochrane Library, MEDLINE, PubMed, Wanfang, China National Knowledge Infrastructure (CNKI), Chinese Biomedical (C

[Two rare cases of severe hyponatremia with consciousness disorder following bowel preparation and high fluid intake].

Symptomatic hyponatremia due to bowel preparation is extremely rare, but it can cause severe neurological symptoms and require hospitalization. We report our experience with two cases of symptomatic hyponatremia after bowel preparation. Our findings suggest that the cause of hyponatremia may be not only oral bowel cleansing agents but also high fluid intake. Adjusting the dose and pace of oral bowel cleansing agents and fluid intake;rehydration should be considered to prevent any recurrences.

Fluid management in hospitalized pediatric patients.

The proper use of intravenous fluids has likely been responsible for saving more lives than any other group of substances. Proper use includes prescribing an appropriate electrolyte and carbohydrate solution, at a calculated rate or volume, for the right child, at the right time. Forming intravenous fluid plans for hospitalized children requires an understanding of water and electrolyte physiology in healthy children and how different pathology deviates from the norm. This review highlights fluid management in several disease types, including liver disease, diabetic ketoacidosis, syndrome of i

Vasoactive and/or inotropic drugs in initial resuscitation of burn injuries: A systematic review.

BACKGROUND: According to current guidelines, initial burn resuscitation should be performed with fluids alone. The aims of the study were to review the frequency of use of vasoactive and/or inotropic drugs in initial burn resuscitation, and assess the benefits and harms of adding such drugs to fluids. METHODS: A systematic literature search was conducted in PubMed, Embase, Cochrane Database of Systematic Reviews, Cochrane Central Register of Controlled Trials, UpToDate, and SveMed+ through 3 December 2021. The search included studies on critically ill burn patients receiving vasoactive and/or
