Approximately 30% of patients with diabetes mellitus will have disease-related dermatological problems. Dry skin can be associated with autonomic neuropathy and may be fragile, promoting bacterial invasion. Any potentially infected 'diabetic foot' must be taken seriously, and non-painful deep sepsis suspected if there is evidence of sensory loss. Consideration should be given to eliminating nasal carriage of staphylococci if recurrent superficial sepsis occurs in the presence of poor diabetic control. Fungal infections, both of skin and nails, are common but usually not serious in the absence …

Rheumatoid hindfoot deformity presents with hindfoot eversion, flattening of the longitudinal arch and abduction of the forefoot. Splayfoot, as the typical rheumatoid forefoot deformity, is mostly associated with various toe malformations, i.e. hallux valgus, hammer toe and claw toe, which may either be attributed to hindfoot malalignment or develop as a separate entity. The algorithm of treatment, comprising clinical assessment of both lower limbs, includes both orthotic shoe devices and surgical treatment. In rheumatoid flatfoot, arthrodesis of the hindfoot with lengthening of the lateral colu…

Synovial sarcoma of the foot and ankle frequently is misdiagnosed, which leads to delays in treatment. The clinical records of 14 patients with synovial sarcoma of the foot and ankle were reviewed. Common findings at presentation were an enlarging mass with a variable incidence of pain, tenderness, and edema. The patients tended to be younger than patients with other soft tissue sarcomas (30 years) and had a median duration of symptoms of 14 months. Of the 14 patients, 12 underwent an attempted curative surgical procedure. Ten patients had partial foot amputations or below knee amputations, wh…

Numerous reconstructive procedures have been used to address the manifestations of rheumatoid arthritis in the foot and ankle. Clinical studies have documented that these procedures relieve pain, however they often sacrifice motion essential to the normal function of the foot. In the forefoot, metatarsophalangeal joint resection arthroplasty shortens the lever arm of the foot, defunctions the toes, and disables the plantar plate and fat pad. Arthrodesis of the ankle and hindfoot alters gait and the effective transmission of weight-bearing stresses through the foot and ankle. These ablative pro…

The goal of this study was to characterize Charcot neuroarthropathy of the foot and ankle by specific sites of involvement (ankle, hindfoot, midfoot, and forefoot), modes of presentation, methods of management, and outcome. A summary of treatment and results for 50 ankles, 22 hindfeet, 131 midfeet, and 18 forefeet is presented. Nondisplaced neuropathic ankle fractures typically healed uneventfully with casting and bracing. For displaced ankle fractures, closed reduction and casting generally resulted in loss of reduction and progressive deterioration; better results were obtained with open red…

Polydactyly of the foot is not an uncommon foot problem. However, it is not well understood and not often reported. Its clinical manifestations vary, as do treatment methods and results. During the period of 1979 to 1994, 54 children with polydactyly of 65 feet were treated in our clinic. Bilateral involvement was seen in 11 patients, and six had accompanying polydactyly of the hand. By Blauth and Olason's classification, duplication of the fifth ray (43 feet) was most common, followed by first ray duplication (20 feet) and other ray duplication (2 feet). A rudimentary form was found in one fo…

Man's susceptibility to the virus of foot- and-mouth disease (FMD) was debated for many years. Today the virus has been isolated and typed (type O, followed by type C and rarely A) in more than 40 human cases. So no doubt remains that FMD is a zoonosis. Considering the high incidence of the disease (in animals) in the past and in some areas up to date, occurrence in man is quite rare. In the past when FMD was endemic in Central Europe many cases of diseases in man showing vesicles in the mouth or on the hands and feet were called FMD. The first suggestion of a human infection with FMD was repo…

Occupational problems of the foot and ankle have received less attention than those of other body regions, namely the hand, neck, and lumbar spine. Functional requirements of the foot and ankle differ from other body regions, necessitating a different clinical approach to injuries than those indicated in the upper extremity or back. This article reviews the clinical characteristics of the foot and ankle pertinent to the worker and highlights the unique features of the management of foot and ankle problems encountered in occupational orthopedic practice.

Congenital varus deformity of the first metatarsal can be secondary to the abnormal insertion of abductor hallucis, a space-occupying lesion of the first web space, or hypoplasia of the metatarsal. We report a unique case where all three factors were present in one foot. This was combined with talipes equinovarus, which gave the appearance of a simian or hand-like foot.

Compartment syndrome of the foot following trauma is well known in adults but has not been discussed frequently in texts on pediatric trauma. In the current study, seven children and teenagers were identified as having had compartment syndrome of the foot during a five-year period. The average age at the time of the diagnosis was ten years (range, four to sixteen years). The cause of the compartment syndrome was a crush injury in six patients and a motor-vehicle accident in one. All patients had swelling and pain with passive motion, but none had neurovascular deficits. Only the two oldest chi…

Rheumatoid arthritis of the foot and ankle can be a debilitating problem, particularly for patients who have undergone successful hip or knee arthroplasty. Optimal medical management, use of orthotic devices, and surgical intervention are essential components of patient care. Forefoot involvement with hallux valgus and lesser metatarsophalangeal joint subluxation and dislocation are the most common findings. Reconstruction usually requires lesser metatarsophalangeal joint excisional arthroplasty and first metatarsophalangeal joint arthrodesis. Midfoot tarsometatarsal and intertarsal involvemen…

A new antimycotic drug lamisil (Sandoz, Switzerland) was given as pills to 35 patients with pedal mycosis (23 of them suffered from onychomycosis). Pedal mycosis was cured for 2 weeks, onychomycosis for 12 weeks. In 2 cases the drug was discontinued because of gastrointestinal toxicity. By tolerance and efficacy, lamisil is superior to griseofulvin and nizoral.

A case of severe bilateral trench foot is presented in a patient who lived rough for 3 weeks without removing his boots. Non-operative management yielded no clinical improvement and bilateral below-knee amputation was necessary. Histology revealed subcutaneous and muscle necrosis with secondary arterial thrombosis.

Lower extremity problems, specifically in the feet and ankles, are common in dancers. This is not surprising considering the repetitive and acute stresses applied to lower limbs during this rigorous performing arts activity. When evaluating a dancer with lower extremity complaints, the nature of these demands must be appreciated. In addition to routine physical examination, static and dynamic biomechanical evaluation is paramount to analysis and treatment of leg, foot, and ankle conditions. Although a particular problem, such as painful bunion, may seem localized, it is often related to other …
