[Facial bone fracture--statistical analysis and clinical aspects].

We experienced 102 cases of facial bone fracture during 16 months of 1986 to 1987. These cases were analyzed statistically concerning causes, age and locations of the fracture. These fractures have increased rapidly in number. The causes were classified into three types; occurrence during sport, traffic accident and fighting, which were equal in number. There were 85% males and 15% females in the patient cohort, which were concentrated at the ages of 10-20 years. A large part of the fractures was mostly consisted of maxillo-facial components (95%). These trends were similar to the previous rep

Keratoconjunctivitis sicca associated with fracture of the stylohyoid bone in a horse.

Keratoconjunctivitis sicca developed in a mare with fracture of the stylohyoid bone. Treatment was directed at preserving the health of the globe while local inflammation at the fracture site was resolved.

[Fracture types and fragment dislocations of the scaphoid bone of the hand].

Fractures of the scaphoid bone are nowadays better known and treated. However, the types of fractures and fragment dislocations are not well described. The authors analysed 325 fresh and old scaphoid fractures and describe six basic types, clearly defined by location and direction of the fracture line. In fresh cases, the dislocation is mainly rotational. In older cases there is an additional bone resorption. The six basic types of fracture lines are demonstrated radiologically and the course of untreated cases is shown.

[The Herbert system of screw osteosynthesis in fracture of the scaphoid bone].

Indications and different techniques in operating on fractures of the scaphoid are summarised. The "Herbert Scaphoid Bone Screw System" is described. Instrumentation, operative technique and typical clinical applications are shown. The advantages and disadvantages are discussed.

Cortical bone pegs in the treatment of osteochondral fracture of the knee.

Cortical bone pegs have been used in osteochondritis dissecans to fix the loosened or loose fragment. In osteochondral fractures of the knee the fragment has either been removed or reattached usually with headless pins or ASIF screws and only occasionally with cortical bone pegs. We report on four patients with osteochondral fractures of the knee, where cortical bone pegs were used as the means of fixation, with good results.

[Traumatic shock in multiple fractures of bones of the limbs in children].

An analysis of 11230 case histories of children aged to 15 who had fractures of the extremity bones has shown multiple fractures of the extremity bones in 3% of the patients. It was established that traumatic shock in children with multiple fractures of the extremities was diagnosed in 14,1% (I degree--in 51%, II--in 42,6% and III--in 6,4%). The article concerns the incidence and severity of shock in children of different age groups as well as fractures of bones of various character and localization.

The role of bone scans in the assessment of prognosis of scaphoid fractures.

Fifty patients with radiographic evidence of an acute scaphoid fracture were studied prospectively. Technetium bone scans were performed on all patients soon after injury in an attempt to detect fractures at risk of non-union or avascular necrosis. In three patients the scan showed a focal area of decreased radionuclide concentration in the region of the proximal pole of scaphoid implying avascularity, and each developed the radiographic signs of avascular necrosis. Two of these patients, each with a displaced fracture, developed non-union. Acute fractures of the scaphoid with evidence of avas

[Enlarging fractures of skull bones. Apropos of 2 cases].

The authors report two cases of children, who had a severe head injury with large skull fracture. If neurological evolution was good, on the other hand, skull fracture went on to develop, to give a bone defect with a leptomeningeal cyst of arachnoidal origin, as it is pointed out by the classical authors. The authors insist upon the specificity of this entity in childhood, and upon the necessity to observe by repeated clinical and radiological examinations any young child who have a large skull fracture.

Transmastoid decompression of the facial nerve in temporal bone fracture.

Among 41 cases of facial palsy caused by closed heart injury, temporal bone fractures were surgically confirmed in 36 cases, of which there were two mixed fractures and 34 longitudinal fractures. The fracture involved the geniculate ganglion area in 20 cases (55%). In 15 cases, decompression of the facial nerve was carried out using the transmastoid supralabyrinthine approach with disarticulation of the incus; in only five cases was the middle fossa approach used. The technique avoids the craniotomy for the middle fossa approach and is a reliable method of treatment in the majority of patients

Fractures of the carpal bones other than lunate and navicular.

Injuries to the carpals and their ligaments are difficult to diagnose unless one utilizes special X-ray techniques based upon a careful history of the injury and examination for tender points and painful joints. Use of only standard X-rays and cursory examinations caused many injuries to be missed in earlier years. Many fractures resolve with simple splinting or casting, but many do not, and late sequelae are now being diagnosed more commonly than in the past. Localization of pathologic conditions by diagnostic injections of local anesthesia or arthrography (or both) is very helpful. Salvage m

[Long-term peridural anesthesia in fractures of pelvic bones].

A continuous peridural anesthesia was used in 20 patients with pelvic bones injuries. Puncture of the peridural space was performed in an appropriate patient's posture. During 3--15 days 5--12 ml of 2% trimecain solution were infused via a catheter. For patients being in a shock condition the anesthetic dose was 3-4 ml less. 4-5 minutes following injection of the anesthetic in the peridural space pain at the site of fracture disappeared, there was noted a stabilization of hemodynamic indices, and the ventilatory and cardiac functions were improved. Anesthesia contributes to an early restoratio
